In Nerd’s Chalk’s August 21, 2026 guide, the 14 listed pickups split between active effects you trigger and passive effects that apply while assigned to that role. The guide reports Familiarity landmarks at 1, 10, and 50 copies, but does not give a complete set of effect values or formulas. Item behavior can also change with patches, so treat these details as guide-reported and check the current game build where precision matters.
What Permanent Pickups do
Nerd’s Chalk describes two roles for a Permanent Pickup: an active effect, such as curing an ailment or restoring Health, and a passive effect, such as mitigation or a transformation. The guide says a pickup is assigned to one role at a time; it does not describe the active and passive effects as working together. It also summarizes the Familiarity mechanic this way: Collecting more copies of the same Permanent Pickup raises its Familiarity.

The guide lists 14 items. Use the table to match a pickup to the problem in front of you. It does not supply a complete numerical table of effect strength, duration, or proc chances, so none are inferred here.

How to choose for the current threat
Counter a named ailment
Match the pickup to the ailment: Bloodseed for Bloodcurse, Burnt Effigy for Burn, Galvanic Shard for Lightning, Martyr’s Ash for Frost, Paradoxical Scripture for Stasis, Seedbearer’s Scripture for Cosmic Disease, or Tarspore for Poison. The guide reports that each active use cures its corresponding ailment and grants temporary mitigation. It does not provide mitigation amounts or durations.

Pick the healing pattern you need
Mango is the listed instant Health restore. Weltcap restores Health gradually when used, and its passive also refills Health over time. The guide does not provide exact recovery rates or caps.
Support Resolve or defense
Common Moonshine actively restores Resolve and passively provides damage reduction that scales with copies. Special Moonshine is reported to increase maximum Resolve, but its active effect is not stated in the guide. Choose based on whether you need Resolve now or a larger maximum, and verify Special Moonshine’s current behavior in-game.

Sheephead Totem offers sheep transformation, grazing, and a jump attack, alongside its enemy-Health-reduction passive. Mango also unlocks transformations. Trollweed changes you into a bush and is reported to grant an unspecified total immunity; the guide does not establish its exact scope.

What the Familiarity milestones mean
Nerd’s Chalk reports 1, 10, and 50 copies as broad Familiarity landmarks—Tier I, Tier II, and Tier III, respectively. These are not presented as universal formulas for every effect. The guide says some proportional passive effects may continue increasing after 50 copies, but does not identify exact values or provide a full per-item progression table. Do not infer percentages, proc rates, healing amounts, or durations from the tier labels alone.

What is known about replenishment and New Game Plus
The guide claims active charges replenish by resting at a Beacon and that using a pickup does not consume the item itself. It also claims pickups respawn and that inventory and Familiarity persist into New Game Plus. These claims were not corroborated by official documentation in the sources reviewed, so check your current build before planning a farming route or relying on carryover.

Item behavior is patch-sensitive. An official Steam community announcement snippet reports an adjustment to the Grisha Permanent Pickup and says Weltcap became stronger in both active and passive forms. The available announcement detail does not establish exact current values for the full item list. Consult the official Steam community announcements or check the item in your build before relying on exact mechanics.
