Some studies find a lower risk of antibiotic-associated diarrhea when selected probiotics are used; they do not show that every probiotic works for everyone. Evidence supports starting certain studied regimens within two days of the first antibiotic dose, but it does not establish a universal number of hours to separate a probiotic from an antibiotic.
Can you take probiotics while taking antibiotics?
For many people, taking a probiotic during an antibiotic course is an option. In a 2021 systematic review of 42 studies involving 11,305 adults, probiotic co-administration was associated with a lower risk of antibiotic-associated diarrhea (AAD): pooled relative risk 0.63 (95% CI 0.54–0.73). The review rated the overall evidence as moderate, and its authors cautioned that the results may not apply when a person’s baseline risk of diarrhea is low. BMJ Open, 2021

That result is an average across studies, not a guarantee for an individual or proof that any product labeled “probiotic” will help. Trials varied in organisms, doses, populations, and how they defined diarrhea. A 2017 review of 17 studies and 3,631 nonhospitalized participants also found an association with about half the likelihood of AAD, but NCCIH describes the conclusion as tentative because study quality was moderate. For some studied probiotic regimens, starting within two days of the first antibiotic dose may work better than starting later. This is evidence about when to begin those regimens; it is not a rule that applies to all products. NIH Office of Dietary Supplements

The sources summarized here do not establish a universal spacing interval, such as taking every probiotic a fixed number of hours before or after an antibiotic. Follow the product label and ask a pharmacist or clinician about a specific antibiotic and probiotic combination rather than relying on a general timing rule.
Which probiotics have been studied?
Evidence summaries name particular organisms, including Lactobacillus rhamnosus GG and Saccharomyces boulardii. Guidelines also identify specific organisms or combinations for certain C. difficile prevention recommendations. A generic label does not show that the contents match a studied intervention.

- Organism and strain: Check the full name and strain designation where given; products sharing a broad species name are not necessarily equivalent.
- Dose and preparation: Compare the labeled dose and formulation with the studied regimen. A matching species name alone does not establish that a retail product is the same as the one tested.
- Purpose: Evidence about preventing general AAD, preventing C. difficile-associated diarrhea, or treating diagnosed C. difficile infection concerns different outcomes.
The American Gastroenterological Association’s 2020 guideline suggests selected probiotic products for adults and children receiving antibiotics in the context of C. difficile infection prevention. This is a conditional, product-specific recommendation—not a directive for every antibiotic user to take a probiotic. AGA guideline

It can. The adult meta-analysis found evidence of benefit in studies with moderate or high baseline AAD risk, but not in low-risk studies. Cochrane describes a possible small short-term benefit among antibiotic users without weakened immunity and calls for more research among people at low risk of C. difficile infection. Cochrane review

These findings do not provide a personal risk estimate. Age, health, antibiotic and care setting can differ between a reader and the people in the trials, so a clinician or pharmacist can help judge whether a probiotic is relevant in an individual case.
Are probiotics a treatment for C. difficile infection?
Prevention evidence should not be used as a reason to self-treat active infection. The AGA recommends probiotics for people with diagnosed C. difficile infection only in the context of a clinical trial. If diarrhea develops during or after antibiotics, contact a healthcare professional for advice, especially if symptoms are severe or persistent.

The 2017 Infectious Diseases Society of America infectious diarrhea guideline says probiotic preparations may be offered to reduce symptom severity and duration in immunocompetent adults and children with infectious or antimicrobial-associated diarrhea; it grades this as a weak recommendation with moderate-quality evidence. That recommendation concerns symptom management in that population, not proof that all products prevent AAD. IDSA guideline

People who are critically ill, severely debilitated, or have impaired immune systems should get individualized medical advice before taking probiotics. Although probiotics are generally considered safe for many people, rare bloodstream or fungal infections linked to probiotic organisms have been reported in vulnerable patients. NCCIH and IDSA describe this safety concern, including case reports in critically ill or immunocompromised people. NCCIH IDSA guideline
