Liposomes can change how a chemotherapy drug circulates and reaches tissues, which may reduce or alter some toxicities. They do not make chemotherapy harmless, guarantee that enough drug reaches every tumor, or prevent all side effects. Pegylated liposomal doxorubicin illustrates both the promise and the limits: serious risks still include hand-foot syndrome, mouth sores, infusion reactions, and heart damage.
How does liposomal chemotherapy work?
A liposome is a tiny, fat-based carrier. In liposomal doxorubicin, the active chemotherapy drug doxorubicin is enclosed in pegylated liposomes and given intravenously. The carrier changes the drug’s distribution and clearance; it is not itself the cancer-killing drug.

The National Cancer Institute describes liposomal doxorubicin as lasting longer in the body than other forms. The U.S. prescribing information explains that its distribution differs substantially from free doxorubicin: its small steady-state volume of distribution suggests that much of the drug remains in the vascular fluid, with doxorubicin becoming available after liposomes move out of blood vessels into tissues. PEG coating and encapsulation can slow clearance and change tissue exposure.

These properties provide a rationale for improving delivery or changing toxicity, including some heart-related effects associated with anthracyclines. They do not ensure selective delivery to every cancer cell. Tumor access varies, and a carrier’s intended targeting effect is not proof of uniform or sufficient drug exposure in an individual tumor.

Can liposomes reduce chemotherapy side effects?
They may reduce or change selected toxicities, but the result depends on the specific drug formulation, cancer, treatment regimen, and patient. The NCI notes that liposomal doxorubicin may have fewer side effects than other forms; that does not mean it avoids adverse effects.
The U.S. DailyMed prescribing information lists fatigue, nausea, mouth sores (stomatitis), vomiting, diarrhea, hand-foot syndrome, rash, and blood-count abnormalities among reported reactions. It also warns of cardiomyopathy and potentially serious infusion reactions.

One illustration of why the details matter: in ovarian-cancer Trial 4 in the U.S. prescribing information, 51% of patients receiving doxorubicin hydrochloride liposome injection experienced hand-foot syndrome, and 24% had grade 3 or 4 cases. Those figures describe that trial and regimen; they are not a general rate for every patient, cancer, or liposomal product.

What does the evidence show about tumor targeting?
“Targeted” can be misleading if it suggests that a liposome seeks out every cancer cell or spares all healthy tissue. A 2024 review of pegylated liposomal doxorubicin describes the enhanced permeability and retention effect—the tendency of some tumors to accumulate larger particles—as variable across human tumors and potentially lower in metastases. That variability helps explain why a design rationale does not guarantee a clinical delivery benefit in every tumor.

The same review identifies hand-foot syndrome and stomatitis as persistent, dose-limiting toxicities for pegylated liposomal doxorubicin. Liposome design can shift the balance of exposure and harm; it cannot remove the need to weigh both treatment benefit and risk.
What is pegylated liposomal doxorubicin used for?
In the United States, the National Cancer Institute lists FDA-approved uses of doxorubicin hydrochloride liposome for ovarian cancer that has progressed or recurred after platinum-based chemotherapy, AIDS-related Kaposi sarcoma, and multiple myeloma in combination with bortezomib for appropriately pretreated patients. Exact eligibility, restrictions, dose, and schedule depend on the current product label and treatment setting.

The liposomal product is not interchangeable with other doxorubicin products. Its prescribing information explicitly warns against substitution, and gives regimen-specific dosing instructions. A treatment decision must therefore identify the formulation precisely rather than treating “doxorubicin” and “liposomal doxorubicin” as synonyms.

There is no universal winner across drugs and cancers. A useful comparison focuses on the evidence for the exact treatment rather than assuming that one carrier design is safer or more effective in general.

Can clinicians remove liposomes from circulation to change exposure?
A small clinical investigation explored “kinetic targeting”: giving pegylated liposomal doxorubicin and later using double-filtration plasmapheresis to remove circulating liposomes. In the 2011 CARL study, about 62% of circulating PLD was removed—corresponding to roughly 45% of the total dose—across 57 treatment cycles. The investigators reported a 50% reduction in doxorubicin area under the curve. The report involved 12 patients receiving neoadjuvant breast-cancer treatment and three with recurrent ovarian cancer.
This is a limited clinical investigation, not routine self-care or evidence that patients can safely alter chemotherapy exposure on their own. It does not establish that this approach is appropriate for other patients, drugs, or treatment settings.
What should patients discuss with their oncology team?

- What exact drug formulation and regimen are being considered, and why?
- Which benefits and risks are established for this cancer and treatment setting?
- Which adverse effects require prompt reporting, and how will they be monitored?
- How should a suspected side effect be handled without changing or delaying treatment independently?
Do not switch formulations, change a dose, or pursue extracorporeal drug removal without the oncology team. Formulation-specific risks and schedules require clinical oversight.
