Propranolol is being studied as an add-on to cancer care, with a possible signal around surgery, but current evidence does not establish that it prevents recurrence or extends survival. A 2025 review found mixed or inconclusive results with chemotherapy and radiotherapy, while pooled studies of beta blockers used with immune checkpoint inhibitors did not show longer survival.
What do clinical studies show?
The evidence varies by cancer type, drug, treatment timing and outcome measured. A change in a laboratory marker is not the same as fewer recurrences, slower progression or longer life.

| Evidence | What it found | What it does not establish |
|---|---|---|
| 2025 systematic review of propranolol | Included 31 publications: 7 randomized controlled trials, 4 systematic reviews and 20 meta-analyses. The authors described a possible benefit signal, particularly around surgery. | The findings do not prove reduced recurrence or longer survival. Results alongside chemotherapy or radiotherapy were inconclusive, and the review called for further clinical trials. |
| 2020 randomized, placebo-controlled phase II breast cancer study | Preoperative propranolol changed biomarkers associated with metastatic potential. | Biomarker changes do not demonstrate fewer metastases, recurrences or deaths. The authors called for larger phase III trials designed to measure recurrence and survival. |
| 2025 systematic review and meta-analysis of beta blockers with immune checkpoint inhibitors | Across 12 clinical studies and 4,293 patients, the pooled estimate showed no association with longer overall survival (HR 1.02; 95% CI 0.84–1.23) or progression-free survival (HR 0.98; 95% CI 0.80–1.20). | The results do not establish benefit for the combination overall, but they also do not prove that every beta blocker or cancer setting has no effect. The review reported no apparent increase in toxicity and noted that prospective trials are still awaited. |
For the two survival estimates, a hazard ratio (HR) of 1 represents no difference between groups; these confidence intervals include 1. The pooled findings therefore do not support presenting beta blockers with immune checkpoint inhibitors as a proven survival-improving strategy.
Why does timing and the measured outcome matter?
Studies of propranolol have examined it at different points in cancer care and alongside different treatments. The 2025 propranolol review describes the perioperative evidence—the period around surgery—as a possible signal, but says results with chemotherapy or radiotherapy remain inconclusive. A promising signal is a reason to test a question, not proof that the treatment works.

When comparing a study or a claim, check:
- Cancer type and stage: a finding in one cancer cannot automatically be applied to another.
- Which beta blocker was used: propranolol is not interchangeable with every beta blocker; studies may also differ in whether the drug is selective or non-selective.
- Timing and co-treatment: taking a drug around surgery is a different question from combining it with chemotherapy, radiotherapy or immunotherapy.
- What was measured: biomarkers, tumor response, recurrence, progression-free survival and overall survival are distinct outcomes.
- How the evidence was gathered: a randomized treatment trial differs from an observational study of patients already taking a beta blocker for a heart condition.
Can beta blockers have a role in cancer care for the heart?
Yes, a clinician may consider a beta blocker for cardiovascular risk management during some cancer treatments. That is separate from using it to treat a tumor. A 2024 JACC: CardioOncology expert panel recommends considering ACE inhibitors, ARBs, and/or beta blockers to help prevent a decline in left ventricular ejection fraction (LVEF) in patients with breast cancer receiving HER2-targeted therapies. The panel says it remains unclear whether these medicines prevent heart failure.
A review cited by the panel pooled nine randomized trials with 1,362 participants receiving trastuzumab-based therapy. Across groups receiving ACE inhibitors, ARBs or beta blockers, pooled mean LVEF was 2.3 percentage points higher than in control groups (95% CI 0.0–4.6). Because the pooled intervention combined drug classes, this figure does not isolate a beta-blocker effect; the panel also said the clinical benefit remains unclear.

What cancer trials are studying beta blockers?
The National Cancer Institute’s beta-adrenergic antagonist index listed 15 trials on the page accessed in October 2026. Examples marked active there included studies of propranolol for Kaposi sarcoma; propranolol with pembrolizumab and chemotherapy for PD-L1-positive advanced or metastatic triple-negative breast cancer; the same broad combination for advanced esophageal or gastroesophageal-junction adenocarcinoma; propranolol with chemoradiation for esophageal cancer; and naltrexone plus propranolol with standard immunotherapy for stage II–III melanoma.

Trial status and locations can change, so check the NCI listing for current details. A trial’s presence on a listing means the question is being studied; it is not a recommendation or evidence that the treatment is effective.

Should you start or stop a beta blocker because of cancer?
No. Do not start, stop or change propranolol or another beta blocker to try to affect cancer outcomes. The right medicine, dose, interactions and monitoring depend on the individual and require assessment by the treating clinician. If you already take one, discuss any concern with your oncology or cardiovascular care team rather than changing it on your own.
