A licensed dentist or orthodontist needs to assess your teeth, bite, dental history, and goals before recommending a plan. The practical decision is whether the proposed movements are appropriate and predictable for you—and whether you can wear and care for removable trays as directed.
Who may be a candidate for clear aligners?
Clear aligners are prescription, custom-made removable trays, not a one-size-fits-all cosmetic product. The U.S. Food and Drug Administration (FDA) says they are used under the supervision of a licensed practitioner. An evaluation is needed to decide whether they fit your particular teeth and bite.

The American Association of Orthodontists (AAO) lists crowding, gaps, overbite, underbite, open bite, crooked teeth, and crossbite among conditions aligners may treat. The FDA describes common use for mild-to-moderate crowding, gaps, and bite problems. These lists identify reasons to seek an evaluation; they are not an eligibility checklist or proof that aligners alone will work for an individual.

More complex orthodontic conditions may need traditional braces, another treatment, or a combination. Some cases are more predictable with braces. The decision depends on the specific movements required and the clinician’s assessment, not simply on the name of a condition.

What an evaluation should consider
A practitioner assesses your teeth, bite, dental history, and treatment goals. The evaluation may include X-rays; scans or impressions can be used to plan customized trays. Ask who reviews your records and approves the plan, what movements it is designed to make, what limitations may remain, and what alternatives are reasonable.
What treatment involves—and what it asks of you
- Evaluation and planning: A licensed practitioner examines your teeth and bite and discusses your goals and treatment options.
- Records and custom trays: Scans or impressions are used to plan a series of trays made for your treatment.
- Daily wear and tray changes: The FDA says patients typically wear each tray for one to two weeks and generally wear aligners at least 20 hours a day, or as directed by their practitioner.
- Progress checks: The practitioner monitors treatment. Ask how often checks are needed and when an in-person visit is appropriate.
- Retention: A retainer may be recommended after active treatment.
The FDA’s consumer comparison gives a broad possible treatment duration of six to 18 months. That range is not a promise or an estimate for a particular patient; the time depends on the movements needed and should be discussed as part of an individualized plan.
Because trays are removable, making progress depends on following the wear instructions. Removability also lets you take them out to eat, brush, floss, and clean the trays. That flexibility comes with a daily responsibility to put them back in as directed.

How aligners compare with braces
Neither option is universally faster, safer, or better. Ask the clinician to compare the approaches for your case, including whether the planned tooth and bite movements are predictable with aligners and whether braces or combined treatment would be preferable.
| Consideration | Clear aligners | Braces |
|---|---|---|
| Appliance | Custom trays that can be removed; the FDA says aligners are prescription devices used under a licensed practitioner’s supervision. | Fixed appliances; the AAO notes that some cases are treated more predictably with braces. |
| Eating and hygiene | Can be removed for eating, brushing, flossing, and cleaning the trays. | Fixed during treatment; ask the practitioner what eating and cleaning routines apply to the proposed braces. |
| Wear and adherence | Requires the patient to follow the prescribed daily wear instructions. | Ask how a fixed appliance affects your routine and what care is required. |
| Appearance | Less noticeable than traditional braces for many users, according to the FDA. | Ask which appliance options are available and how they compare in appearance. |
| Suitability and predictability | Depends on the movements required and the individual treatment plan. | May be more predictable for some cases; combined treatment may also be considered. |
| Duration, cost, refinements, and retention | Ask for a case-specific estimate and plan. | Ask for a case-specific estimate and plan. |
When comparing plans, include follow-up and in-person monitoring, what happens if treatment does not progress as expected, and any costs associated with refinements, retention, correction, or replacement. General patient guidance cannot settle those questions for your case.
Risks, supervision, and direct-to-consumer aligners
The FDA lists several possible problems: treatment may not progress as intended if wear instructions are not followed; trays can be lost or damaged; speech may change temporarily; and some people may experience irritation or an allergic reaction. Existing crowns, fillings, or implants may loosen or be dislodged and need repair or replacement. Adverse events can include swelling and cuts.

The FDA warns that inadequate supervision can raise the chance of bite problems, unintended tooth movement, pain, and tooth loss. It recommends a practitioner evaluation, preferably in person, discussion of benefits and risks, following the prescribed plan, and contacting the practitioner about pain or bite changes.
In its 2026 patient guidance, the FDA says it has not authorized clear aligners sold directly to consumers without a prescription and supervision by a dentist or orthodontist. The agency says virtual visits may be useful in some care plans, while periodic in-person visits can help reduce risks and check that treatment is progressing appropriately. Remote monitoring within supervised care is therefore not the same as treatment without practitioner prescription and supervision.
The FDA has received reports involving unauthorized direct-to-consumer aligners of new or worsening dental pain, jaw pain, bite problems, worsening misalignment, and new gaps. These reports do not establish how often such problems occur. If symptoms appear or worsen, contact a dentist or orthodontist for evaluation.

The American Dental Association (ADA) describes direct-to-consumer dental laboratory services as eliminating the dentist’s role in diagnosis, treatment planning, and treatment management. That is the ADA’s professional association policy; the FDA’s statements above are its separate regulatory guidance.
What “FDA registered” does—and does not—mean
FDA registration or listing is not the same as FDA authorization of a specific product. Ask the practitioner which product is being used and what its authorization status is. The FDA says consumers can search its 510(k) database using product code NXC; its patient guidance explains the distinction at Clear Dental Aligners.
Questions to ask at a consultation
- Based on my teeth, bite, dental history, and goals, am I a candidate for aligners alone? What findings support your recommendation?
- Which movements are realistic, and which parts of my bite or alignment may remain untreated?
- Would braces, a combination approach, or no treatment be reasonable alternatives? Why?
- What examination and records do you need, and who reviews and approves the treatment plan?
- How many hours a day should I wear each tray, how often should I change trays, and how will you check progress?
- How often should I have an in-person visit, and whom should I contact if a tray does not fit or symptoms arise?
- What are the likely duration and total cost, and what is included for refinements, retainers, correction, or replacement?
- Which aligner product will be used, and how can I check its FDA authorization status?
