No—antibiotics do not sort bacteria into “good” and “bad.” They act on bacteria susceptible to a particular drug, which can include helpful gut microbes as well as bacteria causing an infection. That disruption may weaken some natural defenses and give organisms such as C. difficile an opportunity to grow.
What antibiotics target
Antibiotics are medicines that kill certain kinds of bacteria or stop them from growing. Their relevant distinction is whether a bacterium is susceptible to the medicine—not whether it helps or harms the person. As the CDC explains, antibiotics can kill harmful bacteria that cause infections and helpful bacteria in the gut.

“Good” and “bad” are useful shorthand, but microbes do not fall into permanent categories. Some can live harmlessly in one setting and cause problems in another. Antibiotics also differ in what they affect; this general explanation does not establish the effects of any particular drug.

The gut microbiome is a community of microbes that contributes to breaking down food, producing nutrients, supporting natural defenses, and protecting against infection. Antibiotics can reduce the number and variety of helpful microbes, disrupting that community. The CDC says those effects can last several months.

When beneficial microbes are suppressed, other organisms may face less competition. This can allow C. difficile—a bacterium that can cause serious diarrhea and colitis—to grow. Resistant organisms that survive may also have an advantage in some circumstances. These are possible ecological effects, not outcomes that happen to everyone who takes an antibiotic.
How antibiotic use relates to C. difficile risk
The CDC says people are up to 10 times more likely to get a C. difficile infection while taking an antibiotic and during the month after finishing one. This is a CDC estimate for that comparison period, not a calculation of any one person’s likelihood. The CDC also says longer antibiotic courses can potentially double C. difficile risk.

These risks are a reason to use antibiotics appropriately, not a reason to stop a prescribed medicine on your own. If you have concerns about a prescription or develop symptoms, contact a healthcare professional for advice specific to your situation.

Colds and flu are caused by viruses, and antibiotics do not work against viruses. They treat certain bacterial infections. Taking an antibiotic when it is not needed can cause side effects and contribute to antimicrobial resistance without treating a viral illness.

What to do if you are prescribed an antibiotic
- Take it exactly as prescribed. Ask the prescriber or pharmacist if you are unsure how to take it or have questions about the instructions.
- Do not share your medicine with someone else or save it for a future illness.
- Do not stop, extend, or otherwise change treatment based on general information about gut bacteria. Ask a healthcare professional what to do about your specific medicine or symptoms.
These points reflect CDC guidance on antibiotic use. They do not replace advice from the clinician who knows your infection and health history.
