AI Agent Got a Fact Wrong? How to Correct Its Memory

An AI assistant may remember a false fact from more than one place. Identify the source, correct or remove the relevant record, and check whether the stale claim returns.

First find where the incorrect fact is stored. An AI assistant’s “memory” might be a saved note, a summary of earlier chats, the original conversation or file, a connected app, or a developer-managed data store. Correcting one layer may leave the others untouched, so identify the source before you delete or edit anything.

Find which memory source contains the wrong fact

Start by noting the exact claim the agent repeated and where it appeared. If the product can identify its sources, ask what it used for that answer; then inspect the relevant memory controls and source material. In ChatGPT, for example, response sources may identify a past chat, saved memory, custom instruction, file, or connected email, but OpenAI cautions that source information may not show every factor that influenced an answer. Its memory summary is also high-level and may not include every detail or source available to memory: OpenAI’s Memory FAQ.

“What do you remember about me?” can be a useful diagnostic prompt, but an answer to that question is not a complete audit of every source the system can use. Treat it as a clue, not proof that the false fact has been found.

  • Saved memory: A separate record the product retains for future conversations.
  • Chat summary or recalled context: A compressed or retrieved account of earlier interactions.
  • Original source: The chat, uploaded file, email, or other connected content from which the claim came.
  • Application-managed state: Data a developer’s agent stores in files, checkpoints, or a database.

Correct or remove the record in a consumer assistant

ChatGPT

Open the memory controls available in your ChatGPT account and review the saved information or memory summary. Depending on your plan, region, platform, and workspace, you may be able to tell ChatGPT to correct a memory, edit a saved memory or summary, or delete it. OpenAI describes these controls and their account-dependent availability in its Memory FAQ.

If you want the false claim removed rather than merely corrected, check where else it may remain: the saved memory, the chat in which it appeared, other chats, files, and connected apps. OpenAI makes an important distinction: “Deleting a chat alone does not necessarily delete a separate saved memory created from that chat.” Turning Memory off also does not itself delete existing chats. OpenAI says that turning Memory back on can allow new remembered information to be created from chats still in history. Review the product’s current instructions before deleting anything you may need.

OpenAI also says memory updates and deletions can take time to propagate. Logs of deleted saved memories may be retained for up to 30 days for safety and debugging; this is an OpenAI-specific statement, not a general rule for other services. See the ChatGPT Memory FAQ for the current details.

Claude

Anthropic’s consumer support guidance describes controls for reviewing memory topics, editing a topic, or deleting it. It also discusses pausing or resetting memory and importing or exporting it. Use the controls in the Claude product to change the relevant topic, and check the Claude personalization support article for the current interface and availability.

Claude’s consumer memory controls are separate from its developer-facing API memory tool. A correction made in one should not be assumed to change the other.

Remove the false fact from the source, too

Changing a summary or saved note may not remove the claim from the original chat, an uploaded file, or a connected service. If the agent keeps repeating the claim, inspect the source it may be retrieving and correct or remove the false information there as well. For ChatGPT, OpenAI specifically advises checking other places where the information might appear, including chats, files, and connected apps; its Memory FAQ explains why deleting only one item may not be enough.

Before deleting an entire conversation or file, consider whether you need its other contents. Where possible, edit the source to fix the specific fact, or remove only the material you no longer want the assistant to use.

Update the store used by a custom agent

For a developer-built agent, a correction in the active conversation may not rewrite persistent data. Find the record the application retrieves and update or delete it through the application’s own storage logic.

Claude API memory files

Anthropic’s API memory tool is built around file operations that the application implements, allowing Claude to create, read, update, and delete files across sessions. Inspect the files the application actually supplies or retrieves, then change the incorrect record there. The tool’s documentation describes this developer-managed arrangement: Claude API memory tool.

LangGraph checkpoints and stores

LangGraph distinguishes checkpointed state associated with a conversation thread from longer-term stores that can hold application-defined information across threads. If the error is confined to one thread, inspect its checkpointed state; if it recurs across threads, look for a long-term store or other retrieval source. This distinction helps locate the relevant data, but LangGraph’s persistence documentation does not define one universal correction interface: LangGraph persistence.

Verify the correction without treating it as a guarantee

  1. Reopen the product’s memory controls or inspect the application’s stored record to confirm the intended change.
  2. Ask a neutral question that would reveal whether the agent still relies on the incorrect fact. Avoid embedding the correction in the question, since that can supply the answer rather than test what the agent retrieves.
  3. If the false claim returns, check another layer: the source chat or file, a connected app, a summary, or developer-managed state.

This check can show whether the agent still uses the claim in that situation; it cannot prove that the fact has been erased from every system or guarantee that it will never recur. The cited product and framework documentation does not establish a universal cross-platform test or correction process.
