A production fix is not simply a larger memory store: persist the right state, give each memory a clear scope and source, revise it as facts change, and verify that retrieval and deletion work across the whole lifecycle.
Why enterprise agents forget
“Memory” can mean anything from data held in a running process to a durable record retrieved in a later conversation. An agent may seem forgetful for several different reasons, so diagnose the failure before changing the storage layer.

State disappears when a process restarts
If a session exists only in application memory, a process restart can erase it. Google Cloud’s architecture guidance describes external persistent stores—including Memorystore, Firestore, and Agent Platform Sessions—as ways for production applications to retrieve state and write updates back after each turn. The appropriate choice depends on the application’s durability, recovery, and operational requirements; the key architectural property is that needed state does not depend on one live process.
The memory exists, but retrieval misses it
An agent cannot use a record that retrieval does not surface. Retrieval may not run at the right point, or ranking may put a less useful result first. Microsoft’s agent-memory guidance also describes “fading memory”: as a store grows, retrieval precision can decline and useful details can be obscured by noise. More stored material therefore does not automatically produce better recall.

Summaries lose detail or preserve obsolete facts
Compression can omit a qualification, and extraction can introduce a claim that was never in the source conversation. An append-only store can also retain an old value beside a newer, incompatible one. Orogat and Mansour’s 2026 preprint identifies missing semantic revision as a recurring failure mode: reliable memory depends on how stored state changes, not just whether individual records were written correctly.
Scope and deletion do not follow the data
A fact relevant to one person or project may be inappropriate in another user’s context. And deleting a raw record may leave the same information in an embedding, cached result, archive, or generated summary. Both problems arise when a system treats memory as a single undifferentiated store rather than data with boundaries and a lifecycle.
Choose the right kind of memory
Separate temporary working state from durable memory, then distinguish durable memories by the job they serve. Microsoft’s guidance describes three useful categories:

| Memory type | What it represents | How to handle it |
|---|---|---|
| Semantic | Facts, preferences, or other relatively stable knowledge about a user or context. | Keep provenance and freshness information; revise or supersede a fact when it changes. |
| Episodic | A particular event or interaction, associated with a time. | Preserve timestamps so the agent can distinguish what happened when, rather than treating every event as current. |
| Procedural | A workflow or way of carrying out a task. | Store and retrieve it as a procedure, with appropriate versioning when the workflow changes. |
Active session or thread state serves a different purpose: it supports ongoing work and may not deserve long-term retention. Persist it externally when recovery after restart or coordination across application instances matters. Promote information into durable memory only when it has a defined future use and meets the system’s sensitivity, scope, retention, and ownership rules.
Build a write lifecycle that can correct itself
Memory should be managed as production data, not as an ever-growing transcript. Assign ownership for each stage so a record can be checked, updated, and ultimately removed.
- Set eligibility and policy before capture. Define which information may be retained, for whom or what scope, for how long, and under whose ownership. Avoid storing credentials or restricted data. Provide appropriate ways to review, correct, and delete retained information.
- Extract with a link to the source. Preserve the originating conversation or event reference and its timestamp. Validate extracted facts against the source turns, especially when a compressed summary could change the meaning.
- Consolidate rather than only append. Identify duplicates and contradictions. Version changes or mark superseded records so retrieval can favor the current state without erasing the history needed for provenance.
- Reinforce or demote deliberately. Use importance and access frequency to help prioritize useful records. Apply decay or move stale, rarely used information to a lower-priority tier when policy permits; do not let age alone silently rewrite a fact.
- Make deletion end-to-end. Follow a deletion request through raw records, indexes, embeddings, caches, archives, and derived summaries. Record completion in a way that allows the system to verify that copies are no longer recoverable.
Retrieve selectively, with scope and freshness checks
Do not inject a large memory history into every request by default. Retrieve on demand when it is useful, and constrain the search before ranking results.

Everything stays effortlessly organized, powered by intelligent tagging and recognition.
- Filter by boundary first: apply metadata constraints for user, tenant, channel, project, and sensitivity at both write and retrieval time. Relevance ranking is not a substitute for isolation.
- Rank for usefulness and freshness: consider the current task, the source and timestamp, and whether a newer record has superseded the candidate.
- Keep the context bounded: monitor how much retrieved memory enters the model context and its associated token cost. Include only what helps the current task.
- Keep memory subordinate to instructions: a retrieved record is context to assess, not an instruction that can override system rules or authorization checks.
Microsoft’s security guidance puts the distinction plainly: “Memory is candidate context, not authoritative truth.” Validate sensitive or potentially malicious content before injection, and make sure the agent cannot use retrieved text to bypass its governing instructions.
Test security, isolation, recovery, and deletion
Ordinary happy-path recall tests will not reveal many production failures. Include cases that challenge the system’s boundaries and its ability to change state:
- Restart an application instance and verify that the required session state can be recovered.
- Change a remembered fact, then check that retrieval favors the current version rather than surfacing conflicting values as equally valid.
- Seed stale or contradictory records and inspect what the agent actually receives.
- Attempt multi-turn memory poisoning, delayed tool invocation, cross-context leakage, and payload assembly across sessions—the attack patterns called out in Microsoft’s security guidance.
- Request deletion of a fact copied into derived tiers, then test whether it remains recoverable through summaries, indexes, or other retrieval paths.
- Test whether an adversary can extract secrets through the agent’s memory behavior.
Log memory creation, reads, updates, and deletion with the actor, identity, source, and provenance needed for audit. Logs should make operations traceable without becoming an uncontrolled second store of sensitive content.

Measure memory quality as the store grows
Track both whether memory helps and what it costs. Microsoft’s architecture guidance recommends measuring:
- Retrieval accuracy, precision, and recall, including whether the retrieved item was relevant and current.
- Context retention across sessions and detection of fading memory as the store grows.
- Memory-related token cost with and without retrieval, plus p95 retrieval-and-inference latency.
- Repetition reduction, resolution-time improvement, and change in user satisfaction.
The Microsoft page suggests a target above 90% for context retention across sessions. Treat that as the page’s proposed target, not a universal industry standard; define a threshold that fits the task and evaluate it against representative conversations.
Privacy needs its own tests, not just a policy statement. The 2026 preprint Deployment-Time Memorization in Foundation-Model Agents reports a 76% reduction in canary extraction on Gemma 3 12B and 64% on GPT-4o-mini, and says approximately 20% of instances still had derived summary copies recoverable after raw-only deletion. These are benchmark-specific study results, not guarantees for production workloads. They illustrate why adversarial extraction and residual recoverability after deletion belong in an evaluation plan.
Choose an architecture by operational requirements
Storage products and agent frameworks are not interchangeable on the questions that determine production fit. Compare candidate designs against the workload and lifecycle requirements:
- Durability and recovery: confirm what survives restart and whether recovery meets the application’s required point and scope.
- Retrieval and freshness: assess whether the system can find relevant records without surfacing stale information or losing precision as the store grows.
- Revision and deletion: check support for versioning, consolidation, and verified deletion across derived data.
- Isolation and auditability: establish how user, project, agent, and tenant boundaries are enforced, and whether memory operations can be traced to an actor and source.
- Latency and cost: measure retrieval plus inference latency and memory-related token or service costs on the real workload.
- Operational complexity: account for lifecycle jobs, monitoring, retention controls, and failure recovery—not just the initial storage setup.
A simple external store may be sufficient when the application needs durable session recovery and straightforward records. More elaborate memory management is justified when facts change frequently, retrieval noise grows, deletion must reach derived representations, or strict isolation and auditing are required. Decide using measured workload behavior rather than assuming that a larger or more specialized memory system will solve forgetfulness by itself.
