Review the complete skill package and each server’s exposed actions, test untrusted components in isolation, limit credentials and network access, and revisit approvals when versions or remote behavior change. A scan or confirmation prompt can help, but neither replaces those controls.
Why agent skills and MCP servers need review
These integrations combine two kinds of risk: conventional software supply-chain risk and instructions that can try to redirect an agent. Anthropic’s engineering authors describe any external resource provided to an agent as both “a code execution risk” and “a prompt injection vector” (Anthropic engineering article). Anthropic defines prompt injection as an attempt to get an agent to ignore its original instructions, disclose information, or take unintended actions by presenting those actions as useful to its task; vulnerabilities in tools or sub-agents can create additional exposure (Anthropic security guidance).

A skill is not just its entry-point instructions: it may reference files, run scripts, invoke tools, or make network requests. An MCP server exposes actions that can use the identity and permissions granted to it. In either case, file access combined with network access may create a route for sensitive data to leave the environment. OpenAI’s sandbox guidance notes that agent-generated code can access the files, credentials, and network available in its environment (OpenAI sandbox guidance).

Build a gate before installation or publication
Apply the gate before a component is installed, published, or made available to an agent. Record the decision and its scope rather than treating approval as a permanent property of a name such as a skill or server.
1. Identify the component and intended use
For each skill or MCP server, record its source, maintainer, version or revision, installation method, purpose, and the identity used for access. For remote integrations, include the endpoint and authentication method. List the tools and actions the agent will receive. Revisit the record when the code, endpoint behavior, exposed actions, or permissions change. Anthropic recommends version pinning and source review for dependency risk, while cautioning that remote behavior can change after approval (Anthropic engineering article).
2. Audit the complete skill package
Read the entire directory, not only SKILL.md. Follow referenced Markdown, scripts, and bundled resources, then check whether actual behavior fits the stated purpose. Look for instructions to ignore safeguards or conceal actions, conditional behavior, unexpected tool calls, external URL fetches, unexpected destination domains, and code that reads sensitive data and sends or encodes it elsewhere. Test scripts in a sandbox and inspect their outputs. Anthropic specifically calls out scripts, instruction manipulation, and MCP server references as risk indicators, and says: “Never deploy Skills from untrusted sources without a full audit” (Anthropic enterprise Skills guidance).

- WORKS WITH 1000+ ACCOUNTS: Compatible with popular accounts like Google, Microsoft, and Apple. A single YubiKey 5 NFC secures 100+ of your favorite accounts, including email, password managers, and more
- FAST & CONVENIENT LOGIN: Plug in your YubiKey 5 NFC via USB and tap it, or tap it against your phone (NFC), to authenticate. No batteries, no internet connection, and no extra fees required
- MOST SECURE PASSKEY: Supports FIDO2/WebAuthn, FIDO U2F, Yubico OTP, OATH-TOTP/HOTP, Smart card (PIV), and OpenPGP. That means it’s versatile, working almost anywhere you need it
- PRIMARY & SPARE KEYS: Just like having a spare house key, we recommend buying two YubiKeys - one for daily use and one as a spare. That way you’ll never get locked out of your accounts
3. Review MCP tools and actions individually
Inspect the tools a server advertises and ask what each one can read, create, change, or send; whether the workflow actually needs that action; and which identity or credentials it uses. Expose only the minimum set of tools and actions required. Review changes to those definitions rather than assuming that an earlier approval covers them.
Some platforms provide controls for this. OpenAI’s API documentation describes allowed_tools as a way to limit which MCP tools an agent can discover and call (OpenAI remote MCP guide). ChatGPT’s Enterprise/Edu administration workflow describes selecting actions and user groups; new actions are disabled by default when refreshed, and changes to existing actions are shown for review (ChatGPT Enterprise and Edu apps guidance). These controls and availability are product-specific: verify the equivalent feature and plan coverage on the platform you use.
Contain execution, credentials, and data movement
Isolate testing and workloads
Test untrusted skills and servers with fake or non-sensitive data in a contained environment before connecting them to production resources. Separate environments when users or workloads must not share data. Review permissions before testers try write actions, and restrict outbound traffic to destinations needed for the workflow. Isolation limits the damage an unexpected script or tool can do; it does not make the component trustworthy.

Keep credentials out of agent-readable code
Where possible, keep long-lived application credentials and third-party secrets outside environments accessible to the agent. OpenAI notes that a secret injected into an environment can still be read by agent-generated code; a trusted proxy can provide credentials to approved destinations without placing the real secret in the sandbox (OpenAI sandbox guidance).
When an MCP integration needs credentials, use an appropriate protected credential mechanism and scope permissions narrowly. Do not put secrets in reusable agent definitions, plugin archives, or logs. In particular, code running in a stdio server’s environment can read its environment values (OpenAI remote MCP guide).
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Use confirmation as one layer, not the security gate
For consequential actions, human confirmation can add a useful checkpoint. ChatGPT may request confirmation based on app permissions, action context, and potential impact, and may block especially risky actions; administrators remain responsible for deciding whether a connector is suitable (ChatGPT Enterprise and Edu apps guidance). The confirmation behavior described is context-dependent, not a guarantee that every risky action or outcome will be caught. Keep least privilege, isolation, and review in place even when prompts are enabled.

Check what scanning and administration actually cover
Do not assume a platform’s built-in scanner covers every way a skill can enter an organization. Anthropic says organization-level Skills scanning applies to custom skills uploaded or edited in Claude.ai and Cowork, but not Skills API uploads; some pre-existing skills and certain organizational data-handling configurations are also outside the described coverage. For API deployments, Anthropic advises review and version pinning (Anthropic enterprise Skills guidance).
Maintain an explicit coverage record for uploaded skills, API-created skills, local and remote MCP servers, refreshed tool definitions, script execution, and runtime network access. Assign an owner to approve changed versions, permissions, endpoints, and actions before they are used. A clean scan only speaks to the material and paths it actually covered.

Evaluate a security gate by its coverage
When choosing or designing a gate, compare what it actually controls rather than relying on a “secure” label. These are evaluation criteria drawn from official guidance, not comparative product-test results.
- Content: Does review cover every skill file, script, and referenced resource, or only selected entry files?
- Behavior and injection: Does it examine suspicious instructions and tool use as well as code and dependency risks?
- Action scope: Can reviewers restrict MCP tools and write actions to what a workflow needs?
- Credentials: Are tokens narrowly scoped and kept out of logs, reusable definitions, and agent-readable environments?
- Isolation and egress: Can testing and production workloads be separated, with outbound access limited to approved destinations?
- Change review: Are remote behavior and refreshed tool definitions surfaced for approval before use?
- Coverage boundaries: Which platforms, plans, upload methods, and existing installations are included in scanning or administration controls?
No directly applicable prevalence or effectiveness statistic, vendor ranking, or scanner benchmark is established by the cited guidance. Treat the gate as a layered admission and containment process, not a claim that any one scan or feature makes an integration safe.
