ADHD Medications Compared: Stimulants, Nonstimulants, Benefits, and Risks

Stimulants and nonstimulants differ in how quickly they work, their formulations, and their risks. Here’s what to compare with a prescriber.

ADHD medications fall into two broad categories: stimulants and nonstimulants. Stimulants are the most widely used and can reduce symptoms such as inattention, hyperactivity, and impulsivity; nonstimulants generally take longer to work, and their effects may last up to 24 hours. Neither category is right for everyone. The useful comparison is how a medicine may help, how it is taken, what risks apply, and how its effects will be monitored with a prescriber.

How do stimulant and nonstimulant ADHD medications differ?

Stimulants are the best-known and most widely used ADHD medications. The FDA says prescription stimulants can increase attention and decrease impulsivity and hyperactivity in people with ADHD. Nonstimulants are another treatment option; according to the CDC, they generally do not work as quickly as stimulants, though their effects may last up to 24 hours. That duration is a broad summary, not a drug-by-drug timeline.

Comparison Stimulants Nonstimulants
Role Most widely used ADHD medication category; may improve attention and reduce impulsivity and hyperactivity. An alternative ADHD medication category; generally slower to take effect.
Timing and formulation Immediate-release products are usually taken two or three times daily, while extended-release products are generally taken once daily. These are broad patterns, not directions for an individual prescription. Effects may last up to 24 hours, according to the CDC; the CDC overview does not give a timeline for each drug.
Side effects and cautions FDA lists appetite loss, sleep problems, headache, stomachache, irritability, fast heart rate, and high blood pressure as common side effects. Prescription stimulants also carry class-wide warnings about misuse, abuse, addiction, overdose, and death. The sources cited here do not provide a complete side-effect comparison by nonstimulant drug. Check the current official label for the exact medicine’s warnings.

A slower onset does not by itself mean a nonstimulant is safer or a better fit. Nor should stimulant side effects be assumed to apply to every nonstimulant. Specific benefits, side effects, and warnings depend on the medicine and the person taking it.

What benefits can ADHD medication offer?

Medication may reduce ADHD symptoms and make it easier to sustain attention or manage impulsivity and hyperactivity. The response varies from person to person. The CDC reports that 70–80% of children with ADHD have fewer symptoms while taking stimulant medication. That figure applies to children as described by the CDC; it is not a prediction for an individual child, an estimate for adults, or a comparison with nonstimulants.

The National Institute of Mental Health describes stimulants as the most common medication type for ADHD and says research shows they are highly effective. Medication still needs to be prescribed and monitored by a qualified health professional. Finding an option that helps while remaining tolerable can involve trying different medications or doses and tracking both benefits and adverse effects.

What side effects and safety risks should you discuss?

Common stimulant side effects

The FDA lists decreased appetite, trouble sleeping, headache, stomachache, irritability, fast heart rate, and high blood pressure among common stimulant side effects. People respond differently, and the likelihood of a particular effect depends on factors such as health and other medicines. These listed effects are not a complete drug-specific profile; consult the current prescribing information for the medication in question.

Misuse, sharing, and safe storage

On May 11, 2023, the FDA announced class-wide prescription-stimulant labeling updates addressing misuse, abuse, addiction, and overdose. Misuse can include taking your own medicine differently from the prescription or taking medication prescribed to someone else. FDA warns that misuse can lead to addiction, overdose, and death. Do not share a prescription. The FDA directs prescribers to assess and monitor for misuse, addiction, and diversion; patients and caregivers should discuss safe storage and any concerns with the prescriber.

Warnings are medicine-specific

There is no basis in these broad class summaries to rank nonstimulants as universally safer than stimulants, or vice versa. Contraindications, interactions, and serious warnings need to be checked against the current label for the specific product and considered alongside a person’s health and other medicines.

How does age affect ADHD medication decisions?

For children younger than six, two points from U.S. guidance are important but distinct. The CDC describes parent training in behavior management as the first-line treatment before medication is tried. Separately, the FDA says extended-release stimulants are not indicated for treating ADHD in children under six because young children may have higher exposure and higher rates of adverse reactions, including weight loss. These statements concern U.S. guidance and product labeling; they do not substitute for individualized clinical advice or establish the right approach for an older child or adult.

What should you compare with a prescriber?

Bring the discussion back to the specific medicine being considered rather than treating either category as a single interchangeable option. Useful questions include:
