Treat the first five minutes as triage—not a universal acceptance test. Customer forms, portals, deadlines, and approval rules differ.
1. Can the reviewer identify the problem and its scope?
The report should define the nonconformance precisely enough to assess customer exposure. Look for the affected part or process, where and when the issue occurred, and quantified evidence such as the affected quantity or rate. A label like “quality issue” does not show what is wrong or how far it may have spread.

Siemens describes D2 as building an information base around who, what, where, when, and how much. A supplier guideline from Vestas also emphasizes distinguishing what the problem is from what it is not. 2. Does containment protect the customer now?
Containment is an interim safeguard while the team investigates and develops a permanent correction. Check which population it covers, what actions were taken, and what evidence shows those actions are effective. For example, the report should make clear what affected stock or production is covered and how the supplier verified the containment process.

Containment does not prove the root cause has been fixed. Siemens describes D3 as protecting internal and external stakeholders and validating containment effectiveness; Graco’s supplier corrective-action instructions also include containment in the SCAR response. These are useful examples, not a universal customer checklist.
3. Are both the occurrence and escape causes verified?
Two different questions need answers: why was the defect created, and why did the supplier’s controls fail to prevent it from reaching the customer? The first is the occurrence cause; the second is the escape cause. A report that addresses only how the defect was made leaves the failure of detection or prevention unexplained.

Look for evidence that tests the proposed causes, rather than a plausible narrative alone. Siemens describes checking whether the problem appears when the proposed cause is present and is absent when the cause is absent. Process records, control data, or a documented test can help establish that connection.
4. Do corrective actions follow from the causes?
Trace each verified cause to an action. The report should identify who owns the action, when it is due, evidence that it was implemented, and a measure used to validate its effect. If an action does not connect to a cause, or its success cannot be checked, the report does not yet show that the investigation has produced an effective correction.

Siemens says selected actions should eliminate the root cause, be checked for unintended effects, and be validated using the original measurable indicator. That link between the original problem measure and the validation result makes it easier to judge whether the correction worked.
5. Is recurrence prevention reflected in the system?
Look for changes to the relevant process, procedure, control, or standard—not only a one-time fix for the reported incident. The report should also show that the team considered similar products or processes where the same failure could recur. Siemens describes D7 as addressing the systems and standards that allowed recurrence and standardizing process changes.

6. Does the report follow this customer’s instructions?
Before judging format or approval status, check the actual SCAR, customer portal, required form, due date, and requested evidence. Acceptance requirements belong to the requesting customer; one supplier’s process should not be assumed to apply to another.

For example, Graco’s published supplier corrective-action instructions require a complete 8D by the specified due date, submitted to its Divisional Supplier Quality Engineer for validation and approval. That is Graco’s stated process, not an industry-wide deadline or approval rule. The VDA QMC 2021 English-language reference describes the 8D report as an information document from supplier to customer.

What this first pass can—and cannot—tell you
A fast review helps identify missing evidence and obvious gaps in customer protection, cause analysis, and corrective action. It does not replace a technical audit of the full investigation. The sources describe 8D disciplines and customer-specific practices, but they do not establish a universal five-minute protocol or common acceptance threshold. Use the current customer request and supplier manual to determine what is required for a particular case.
