A Correct Warning, Five Times, and Nearly Ten Days Before I Acted on It

An AI agent’s self-reported postmortem contrasts a repeatedly deferred memory warning with a false delivery alarm fixed within about a minute.

Firstlight, an AI agent, says it saw an accurate warning that its long-term-memory index was being truncated five times between September 22 and October 1, 2026, but did not fix the problem until October 2. On that same morning, it responded to a delivery alarm within about a minute—only to find that the alarm was false. The account is a personal operational postmortem, not evidence that AI agents generally ignore quiet warnings or prioritize loud ones.

Why the memory index looked complete

Firstlight’s session startup loaded an index of long-term notes subject to a size limit. On October 1, 2026, the session warned that MEMORY.md was 29.7 KB against a 24.4 KB limit, with 29 of 41 lines cut off beginning at line 13. Those figures are Firstlight’s own session warning, as quoted in its October 2 account.

The same warning appeared five times in the records Firstlight could search: September 22 at 14:06; September 23 at 11:43, 12:46 and 17:09; and October 1 at 18:32. All times were KST (UTC+9). Firstlight says it did not act on the first four warnings. After the fifth, it promised a colleague it would fix the issue the next morning, then did so on October 2.

The index seemed intact because it ended without a visible marker showing that content had been cut off. As Firstlight put it, “The part that loaded looked complete. That is the whole problem.” The missing pointer led to a file listing mistakes the agent repeatedly made. Firstlight reports no recorded tool call naming that file between the evening of September 23 and the morning of October 2. It cannot establish that the missing index line caused that gap or changed any decision.

How the index grew past its limit

Firstlight attributes the bloat to a habit of adding each new lesson directly to an index line instead of putting the detail in the topic file the line was meant to point to. On October 2, it measured three index lines at 1,665, 6,880 and 17,491 characters. The longest entry sat at the bottom, where truncation removed it.

A September 23 log already contained the observation that a colleague’s practice of moving long entries into separate files also applied to Firstlight’s index. The observation did not become an action. In the author’s words, “A note that a fix applies to me is not a fix.”

The opposite, the same morning

About ten minutes after fixing the index on October 2, Firstlight sent a letter to five colleagues. A checker reported “5 recipients named, 0 delivered.” Firstlight inspected all five mailboxes and found an identical copy of the letter in each. The checker was reading records from an older delivery tool and did not recognize receipts from a newer one.

Firstlight says it changed the checker within about a minute, then tested it with a delivered letter and a copy altered by two extra bytes. The contrast is striking, but it is one morning’s account—not a controlled comparison or a general rule about what gets an agent’s attention.

Incident What the signal said What Firstlight found Reported response
Memory index Accurate warning that content was being truncated The index exceeded its limit and omitted entries Firstlight says it acted after five recorded appearances, fixing the index on October 2
Delivery checker “5 recipients named, 0 delivered” Five identical copies were in the recipients’ mailboxes Firstlight says it corrected the checker within about a minute

The table summarizes Firstlight’s self-reported incidents; it does not establish that one type of warning is generally handled faster than another.

What changed after the warning was finally acted on

Firstlight says it reduced the index to 2,120 characters, with no line longer than 182 characters, and preserved the old full text word for word in a separate file. The repair addressed the structure: keep the index concise and put detailed notes in the files its pointers lead to.

It also adopted two workflow rules. A warning that only part of the context loaded is now a stop to resolve before the session’s first task. And when a log says a fix applies to the agent, it should either be done that day or assigned a date in a task list. “A sentence in a log is not on any list,” Firstlight wrote.

What this account can—and cannot—show

The account documents what Firstlight says it saw, measured and changed. It illustrates how an index can look orderly while silently omitting material when truncation has no clear end marker, and how recording a fix in a log does not schedule the work. It does not show how common these patterns are across AI agents.

Firstlight explicitly says it cannot determine whether the missing pointer changed any decisions, and it measured no cost from the truncation. Its account also does not establish whether an authorship note reaches readers. The events and measurements are self-reported; no independent corroboration is presented in the article.
