These 10 free HTML appointment form templates cover clinics, salons, consultations, garages, spas, tutoring, restaurants, veterinary care and callback requests. They provide a front-end design—not a guaranteed reservation system: to show genuinely available times, prevent double-bookings and confirm appointments reliably, connect the form to a live scheduling service or your own backend.
What the 10 templates cover
Colorlib’s 2026 roundup groups its templates by the kind of appointment or request a business handles. The designs are described as plain HTML, CSS and vanilla JavaScript, without a required jQuery dependency or plugin. Colorlib says they can be used on their own or within Bootstrap 4, 5 or 6 projects. These are the publisher’s descriptions; its demo behavior has not been independently verified.

Can customers pick a date and time?
The Colorlib roundup says its demos generate time slots for a selected date, reject closed days, update prices and totals, and show an on-page confirmation after submission. Those are demo behaviors, not proof that a live appointment has been reserved. A browser form cannot know whether a displayed slot was just taken by another customer unless it checks a connected scheduling system.

For appointment requests confirmed by a person
A form can collect the customer’s preferred date and time, then send the request to staff for manual confirmation. Make the status clear in the form and confirmation message—for example, “Request received; your appointment is not confirmed until we contact you.” Form Koi describes its template as suitable for businesses that confirm appointments manually and says to replace the placeholder endpoint before publishing. For live booking
Connect the page to a calendar or booking backend that reads current availability and records the reservation when a customer submits. The system should re-check the slot at submission time, because availability can change while the form is open. Send confirmation only after the backend accepts the booking; an on-page message generated by JavaScript alone is not evidence of a saved appointment.

How do you receive appointment requests?
HTML fields collect input in the visitor’s browser. To receive a request, the form needs a destination: a server endpoint, form-handling service or scheduling product. Decide where requests are stored, which staff members are notified, and whether the customer receives an acknowledgment or a confirmed booking notice.

Use your own server endpoint, an appropriate form service, or a scheduling system that supports the workflow.
- Set the form action or integration. For a copyable form, replace the example or placeholder endpoint with the real endpoint provided by your service. FormBackend’s guide describes copying a template and replacing its placeholder action with the form endpoint; it also describes email notifications, auto-replies and integrations. - Configure notifications and replies. Confirm who receives submissions and what customers are told. An automated receipt should not say that an appointment is confirmed unless the system has actually reserved a slot.
- Test the complete journey. Submit a request using the published page and verify that it reaches the intended destination, that notifications arrive, and that the customer-facing response is accurate.

Is this appointment booking form template free?
The Colorlib roundup presents its templates as free downloads. The separate Colorlib booking roundup states a CC BY 3.0 condition for its broader collection: retain an attribution link when using the covered material for personal or commercial projects. Because the applicable terms can depend on the particular download, read that template’s license before using it on a business site, modifying it or redistributing it.
Can the form collect a deposit or payment?
A free HTML layout does not, by itself, process a payment. FormBackend says its described template collects requests rather than payments. Foldr Forms describes payment integration through Stripe for deposits or full copays as a capability of its hosted service, alongside real-time availability sync. That is a hosted workflow, not a feature of a standalone HTML file; check the provider’s current terms and pricing before choosing it. See Foldr Forms’ appointment form.

Which approach should you choose?
- Choose a downloadable HTML template if you want to adapt the page design and can connect it to an endpoint or booking backend yourself.
- Choose a request form with manual confirmation if staff will check availability and contact each customer before the appointment is final.
- Choose a hosted scheduling page or embed if you want the booking provider to handle the scheduling workflow. Google Calendar Help documents ways to embed an appointment booking page or add a booking button to a website, and lists providers including Shopify, Squarespace, Wix and WordPress. Eligibility and interface details can change; consult Google Calendar Help on booking pages for current steps.
Before publishing, make sure the fields match the service, the destination is active, and the page distinguishes a request from a confirmed reservation. Avoid collecting sensitive information that the business does not need for scheduling.
