Vision-language models (VLMs) misread images for several different reasons, not one generic “hallucination” problem. A model may invent an object, assign the wrong attribute, misunderstand which objects are related, misread text, or reach a conclusion that is not supported by what the image shows. The fix depends on the failure mode.
The most reliable approach is to narrow the question, request observable evidence, inspect difficult regions at higher quality, use specialist recognition or OCR when appropriate, and require the system to express uncertainty instead of guessing. Prompting and decoding methods can reduce particular errors, but no cited technique has been shown to solve every kind of visual mistake.

What “misreading an image” actually includes
When a VLM gives a wrong answer, first classify the error. Different mistakes arise at different stages of image understanding and require different checks.

The model names something that is not present, misses an object that is present, or assigns the wrong property. Examples include calling a bicycle a motorcycle, identifying a baseball as a baseball bat, or describing a red item as blue. These errors are especially common when objects have similar shapes, when the object is small, or when lighting and image quality hide discriminating details.

Relationship errors
The individual objects may be recognized, but the model gets their relationship wrong. It might say that a person is holding an item when the item is beside the person, claim that one vehicle is behind another when it is in front, or confuse which object a description refers to. R-Bench (2024) studies this category with image-level questions about whether a relationship exists and instance-level questions about local visual comprehension. Its analysis discusses relationship–relationship, subject–relationship, and relationship–object co-occurrence patterns, as well as long-tail relationship distributions in visual instruction tuning.
OCR and document errors
A model can produce text that looks plausible but is not in the image. Blur, occlusion, low contrast, compression, unusual fonts, skew, and tiny characters make this worse. OCR hallucination is not merely a spelling problem: the system may confidently complete a number, name, date, or address that it cannot actually read.
Grounding and reasoning errors
Sometimes the visual elements are recognized, yet the answer still requires a conclusion the image does not support. A model may identify several objects correctly but fail at counting, comparing, tracking a spatial condition, or applying the user’s question to the right region. The ICLR 2025 Visual Description Grounded Decoding (VDGD) paper describes this as a gap in contextualizing visual information with the prompt. Recognition-focused improvements do not automatically solve reasoning-intensive questions.
Why VLMs make these mistakes
Fine-grained visual confusion
The 2025 ACL Visual Evidence Prompting paper links hallucinations to weak fine-grained visual comprehension, particularly when scenes or objects are visually or semantically similar. General-purpose models compress an image into representations that may not preserve the small cue that distinguishes two categories. A narrow prompt can expose the problem: “Is this a bicycle or a motorcycle? Name the visible features that decide.” If the answer rests on a detail too small to resolve, the model may substitute the more familiar category.
Visual Evidence Prompting addresses this by feeding outputs from a specialist visual model to a generalist VLM as additional evidence. The specialist is not automatically correct; its output is another observation that must be checked against the pixels and the task.

Language priors and missing context
VLMs are trained with large amounts of language, so likely wording can pull an answer away from the image. If a scene resembles a familiar caption, the model may complete the expected story even when a key object or relationship is absent. R-Bench reports that evaluated systems can overlook visual content, rely too heavily on language-model common sense, and struggle with spatial relationships that depend on context.
Co-occurrence patterns can amplify this effect. Objects that frequently appear together in training data become an attractive guess, while unusual combinations or rare relationships receive less reliable treatment. A prompt that asks for a complete scene description can therefore produce more unsupported details than a prompt that asks only for verifiable observations.

Blur, occlusion and low contrast remove evidence rather than merely making the model “less confident.” The 2025 NeurIPS KIE-HVQA work examines degraded documents such as identity cards, invoices and prescriptions, using simulated degradations and pixel-level annotations for OCR reliability. Its central concern is that a system may fail to recognize when a precise answer is not feasible and then invent a plausible one.
A reliable workflow treats unreadable content as an abstention case. “The last two digits are not legible” is safer than a fabricated number, especially for prescriptions, financial documents, identity records and other high-consequence material.
Reasoning can fail after perception succeeds
Correctly detecting visual elements is only the first step. The model must select the relevant evidence, preserve spatial context, follow the question’s scope and derive an answer without adding assumptions. VDGD’s authors report that techniques aimed mainly at visual recognition can fall short on reasoning-heavy prompts. This finding comes from that paper’s setup; it is not proof that every current system fails through exactly the same mechanism.

A practical workflow for reducing image-reading errors
Use the following sequence when an answer must be grounded in a particular image.
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →- Define the decision. State exactly what must be determined: one object category, a count, a relationship, a transcription, or a comparison. Avoid asking for a broad caption when you need one precise fact.
- Ask for observations before conclusions. Request visible features, locations and text exactly as seen. For example: “List the markings on the left label. If any character is unclear, use an underscore and identify its position.” This separates evidence collection from interpretation.
- Constrain the answer format. Require a short list, a table of object and location, or a transcription with an uncertainty note. A fixed format makes unsupported additions easier to spot.
- Provide the relevant crop. For small text, distant objects or crowded scenes, crop the area of interest and preserve enough surrounding context to identify relationships. Do not assume that enlarging a blurred crop restores information that was never captured.
- Use task-specific recognition when stakes justify it. A specialist detector, OCR engine or document parser can supply additional evidence. Compare its output with the image; treat neither system as ground truth.
- Ask for an uncertainty decision. Include an explicit option such as “answer only if the evidence is clear; otherwise say ‘uncertain’ and explain which region is ambiguous.” This is particularly important for degraded documents and fine-grained categories.
- Verify critical claims independently. For identity numbers, medication instructions, payments, legal text, safety labels or measurements, use a clearer image, a second capture, a human reader or an authoritative record. A fluent answer is not verification.
- Record the image conditions. Note resolution, blur, occlusion, contrast, crop and whether the model had access to the original file. These factors determine whether a failure indicates a model limitation or insufficient input evidence.
Mitigation methods and what each one targets
The following techniques address different parts of the generation process. The cited papers did not evaluate them head-to-head, so they should not be treated as a single ranked league table.
| Method | Mechanism | Most relevant use | Costs and questions |
|---|---|---|---|
| Visual Evidence Prompting (ACL, 2025) | Feeds outputs from small or specialist visual models to a generalist VLM as explicit evidence. | Fine-grained distinctions and visually similar objects or scenes. | Requires a suitable specialist model. Check whether its evidence is reliable for the target domain and whether it improves the specific confusion. |
| Visual Description Grounded Decoding (VDGD, ICLR, 2025) | Generates an image description and uses it during decoding to ground the response. | Questions that require contextualizing visual information and reasoning over it. | Training-free in the paper’s formulation, but adds inference work. Check effects on recognition and OCR tasks as well as reasoning. |
| Visual Contrastive Decoding (VCD, CVPR, 2024) | Changes decoding to reduce object-hallucination tendencies without additional training or external tools. | Object-presence and object-description errors. | Measure latency and possible changes in other task metrics; improvement on one hallucination pattern does not establish better OCR or spatial reasoning. |
| Assembly of Global and Local Attention (AGLA, CVPR, 2025) | Uses image–prompt matching to emphasize prompt-relevant local features and suppress distractions. | Small objects and cluttered scenes where relevant evidence is localized. | Requires attention or image-processing changes. Test whether relevance highlighting helps the target image distribution. |
| Uncertainty-aware OCR behavior (KIE-HVQA, NeurIPS, 2025) | Trains and evaluates uncertainty awareness and refusal behavior for degraded documents. | Blurred, occluded or low-contrast text where abstention is safer than completion. | Calibration matters: test whether refusals occur on genuinely ambiguous cases and whether clean-document performance remains useful. |
Choosing an approach by failure mode
When similar objects are confused
Start with a crop and an observation-first prompt that asks for distinguishing features. A specialist visual model or Visual Evidence Prompting is a logical next experiment when the error is category-level and fine-grained. Evaluate on examples that include near-neighbors, not only easy instances.
When the model invents objects
Ask for an object list with locations and require “not visible” when evidence is absent. VCD is designed around object hallucination and can be evaluated against a presence/absence test. Check whether fewer invented objects come at the cost of missed real objects.
When relationships are wrong
Ask about one relationship at a time and include directional language: “Is the red sign to the left of the doorway, yes or no?” For crowded scenes, provide a crop that retains both objects and their connecting context. Use a relationship-focused test set rather than an object-recognition score.

When text is unreliable
Improve capture quality first: focus, lighting, resolution, perspective and contrast are often more valuable than a longer prompt. Ask for character-by-character transcription with uncertainty markers, and allow refusal. A second OCR system can flag disagreements but cannot recover characters that are absent from the pixels.
When the answer requires reasoning
Separate detection from reasoning: obtain a structured list of visible items and positions, then ask the model to compute or compare only those items. VDGD is relevant to this class because it uses an explicit description during decoding, but its reported gains are benchmark-specific.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How to evaluate whether errors are improving
Use an evaluation that matches the failure you are trying to reduce. A single overall score cannot establish reliable image understanding.
| Failure to measure | Useful test design | What to report |
|---|---|---|
| Object or attribute mistakes | Presence/absence pairs, near-neighbor categories, and attribute questions with balanced examples. | False positives, false negatives and attribute accuracy, separated by object size and image quality. |
| Relationship mistakes | Image-level relationship existence questions and instance-level local-comprehension questions, as in R-Bench. | Relationship accuracy by relation type, subject and object category, and scene density. |
| OCR errors | Clean and degraded documents with character-level or pixel-level reliability labels. | Exact transcription, character error rate, unsupported-character rate and useful abstention rate. |
| Reasoning errors | Questions requiring counting, comparison, ordering or conditional conclusions from visible evidence. | Answer accuracy plus evidence-grounding checks; report failures separately from recognition failures. |
| Visual illusions and context sensitivity | Image-context reasoning and visual-illusion tasks such as those in HallusionBench. | Task-level accuracy and the types of unsupported explanations produced. |
Keep the model version, prompt, image resolution, preprocessing, decoding settings and dataset fixed when comparing a mitigation. Report image quality and prompt type alongside the score. A method that helps clean photographs may not help scanned forms, and a method that reduces object hallucination may leave OCR unchanged.

How to interpret the published figures
KIE-HVQA result
The KIE-HVQA authors report approximately a 28% absolute improvement in hallucination-free accuracy over GPT-4o on KIE-HVQA. This is the result of their 7B Qwen2.5-VL-based model on that benchmark and degradation setup. It is not a general improvement estimate for arbitrary OCR systems, documents or commercial products.
Best Value
VDGD result
The VDGD paper authors report improvements ranging from 2% to 33% across eight benchmarks. The range belongs to their evaluated model, prompts and benchmark definitions. It should not be converted into an expected gain for a production workload without reproducing an equivalent evaluation.
HallusionBench and R-Bench
HallusionBench is presented as a diagnostic suite for image-context reasoning and visual illusions; its authors report that tested advanced VLMs found the tasks challenging, but the retrieved material does not establish a single figure suitable for quoting here. R-Bench is useful because it separates relationship existence from local visual comprehension rather than collapsing both into one score.
Prompt patterns that encourage grounded answers
Observable inventory
“List only objects clearly visible in the image. For each, give its approximate location and one visible feature. Do not infer purpose, identity or action. If an item is unclear, label it uncertain.”
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errorsRelationship check
“Answer whether the person is holding the blue object. Use yes, no or uncertain. Cite the visual cue: contact, distance or occlusion. Do not infer the action from the scene’s typical context.”
Constrained OCR
“Transcribe the label exactly as visible, preserving capitalization and punctuation. Mark unreadable characters as _. Do not complete a word from context. Report which characters are affected by blur or occlusion.”
Reasoning from an evidence table
“First produce a table with item, color, position and count. Then answer: which listed item is farthest left? Use only the table and state ‘uncertain’ if two positions cannot be distinguished.”
Common failure patterns and recovery steps
- Confidently invented detail: Re-run with an observation-only prompt, a smaller scope and an abstention option.
- Correct objects, wrong relation: Crop both objects together, ask a binary directional or contact question, and test each relation separately.
- Repeated OCR substitutions: Capture a sharper image, correct perspective and contrast, then compare two OCR passes. Do not merge disagreeing outputs by intuition.
- Answer changes with wording: Treat the result as prompt-sensitive. Use a fixed prompt template and evaluate multiple paraphrases before trusting the task.
- Long explanation with little evidence: Require a short evidence list first and reject conclusions that cannot be traced to a listed observation.
- Refusal on a readable image: Check crop size, resolution and preprocessing. Uncertainty behavior is useful only when it is calibrated rather than indiscriminate.
What cannot be concluded from current results
There is no reliable global percentage for how often VLMs misread images. Error rates vary with model, image domain, resolution, prompt, language, decoding settings and the definition of “wrong.” Benchmark scores for object recognition, relationship understanding, OCR and reasoning are not interchangeable measures of overall image understanding.
The cited methods also differ in requirements. Some use an extra visual model, some add a description-generation step, some alter decoding, and some train uncertainty behavior. A technique that is training-free may still increase inference time; a specialist model may improve evidence quality while adding deployment complexity. Select and test a method against the failure mode, image conditions and risk level that matter in your application.
Quick Recap
A concise decision checklist
- What exactly is wrong: object, attribute, relationship, text or reasoning?
- Is the decisive visual evidence large enough, sharp enough and unobstructed?
- Can the question be narrowed to observable facts?
- Would a crop or specialist OCR/detector add useful evidence?
- Does the workflow permit “uncertain” or refusal?
- Is the chosen benchmark measuring the same failure mode?
- Have you reported model setup, image quality, prompt and publication year with every number?
- Does a human or authoritative source verify high-consequence answers?
