To recover that order, software must use the PDF’s tagged structure when it is reliable or analyze page geometry to identify columns and other layout regions. A visible gutter is a clue to a column boundary, not proof that every part of the page follows the same two-column pattern.
Why a PDF’s text order can differ from the page
In a PDF, words and other page elements are positioned for display. The sequence in which text is stored or emitted may not match the human reading order. A basic converter that sorts every text fragment by vertical position can therefore take a line from the left column, jump to the right column, and continue alternating across the page.

Correcting this requires more than recognizing the words. The converter must identify page regions, determine how those regions relate, and then reconstruct a sensible sequence within and between them. A heading recognized accurately can still end up after the paragraph it introduces if layout analysis gets the order wrong.

Use text positions and whitespace as evidence
One geometric method extracts text fragments with bounding boxes, measures where text appears across the page, and looks for a substantial vertical band of whitespace. That band may indicate a gutter. The software can then separate the page into regions and read each column from top to bottom rather than interleaving lines across the full page. This is a documented implementation strategy, not a rule every converter follows (pdf.js-based extractor documentation).

Segment mixed layouts before ordering them
Column detection alone is not enough when a page includes full-width elements. A title, heading, figure, or table may span both columns, while ordinary paragraphs below it do not. One approach is to split the page into horizontal bands or other regions, identify cross-column elements, order content inside each region, and place the spanning elements at the correct point in the sequence.
Whitespace-based recursive segmentation, often described as XY-Cut, is one way to split a page into regions. OpenDataLoader describes separating full-width elements such as titles and headers before segmentation and restoring them to their vertical positions afterward (OpenDataLoader’s reading-order and XY-Cut documentation). Google Research’s Ray Smith describes a different method: infer column layout from formatting tab stops, then apply that layout to impose structure and reading order (“Hybrid Page Layout Analysis via Tab-Stop Detection”).

Use a document’s tagged structure when appropriate
Some PDFs include tags that describe content structure and order. If the tags are accurate, a converter or accessibility tool may use them rather than infer every relationship from geometry. But tags can be incomplete or incorrectly grouped, so their presence does not guarantee a good conversion.

A gutter is evidence about a region of a page; it does not establish a single layout for the whole page or document. A first page may have a full-width title and abstract above two columns. A table or figure may cross a column boundary. A reference section may use different spacing, and a narrow gutter or skewed scan can make geometric clues less clear.

- Full-width titles and headings: These should appear before the column text they introduce, not be assigned to one column based only on position.
- Figures and tables: A block may interrupt a column, span both columns, or have a caption positioned separately.
- Mixed page designs: A paper may change layout between its opening page, body, appendices, and references.
- Ambiguous geometry: Narrow gutters, uneven column starts, skew, and noisy page content can mislead rules based on whitespace.
For these reasons, “sort the whole page top to bottom, then left to right” is not a dependable general solution. Layout regions must be identified before the converter decides how to order their contents.
Born-digital PDFs and scanned papers need different handling
A born-digital PDF contains text characters and usually provides positional and font information that can help a converter infer regions. A scanned PDF instead contains page images; it needs optical character recognition (OCR) to turn visible words into text. Some documents mix image pages and digital text, so a tool may need to choose the appropriate route page by page (all2md 1.14.0 PDF documentation).

OCR and reading-order reconstruction are separate tasks. OCR may recognize the words correctly while still assigning them to the wrong sequence. A scan’s skew or noisy text can also make both recognition and geometric layout analysis harder.

- Compare more than one page type. Check the opening page, a typical body page, a page with a figure or table, and the references. A converter that handles a plain body page may still fail on a full-width heading or a different reference layout.
- Check sequence, not just recognized words. Confirm headings come before their content, paragraphs do not alternate between unrelated sentences, and reference entries remain intact.
- Look for layout controls. If available, try a column-count setting or manual region/order correction. A fixed two-column override may help a consistently designed page, but can make mixed-layout pages worse.
- For a tagged PDF, inspect its reading order. Adobe Acrobat Pro’s Reading Order tool lets users split a highlighted region containing two columns into parts that can be reordered, or move an item in the Order panel or on the page. Adobe says this changes reading order without changing the visible appearance. This is a manual tagged-PDF and accessibility workflow, not a universal one-click repair for every text extractor (Adobe Acrobat Pro: Reading Order tool for PDFs).
- Choose another conversion route if needed. For a scanned page, use a tool that performs OCR; for a digital page, check that the tool uses layout-aware extraction rather than simple line sorting. If a document’s page types vary, verify each type instead of relying on one global setting.
What to compare when choosing a converter
Geometric or rule-based methods infer layout from text boxes, whitespace, tab stops, or recursive cuts. They can be lightweight and explainable, but their thresholds may fail on irregular pages. Learned or semantic layout analysis can classify region types and infer reading order, but introduces model and dependency considerations and still requires review. The available documentation does not establish a fair, neutral accuracy winner across these approaches.

One extractor’s own documentation reports processing a 75-page test PDF in 4.31 seconds at concurrency 1 and 1.87 seconds at concurrency 8, using headless Chromium. These are that implementation’s measurements for its test file, not a general speed or accuracy comparison (pdf.js-based extractor documentation).
