Why Does Weed Cause Anxiety? What a Controlled Study Found About Cannabis and Paranoia

THC can cause anxiety and paranoia in some people. An Oxford-led experiment helps explain the link, but its selected participants and intravenous dose limit what the results can say about cannabis use generally.

THC can trigger short-term anxiety and paranoia in some people. A controlled experiment found that intravenous THC increased paranoia, anxiety and worry, and unsettling perceptual experiences among people who already reported paranoid thoughts. The study was led by researchers at Oxford and published in 2015; it is not a newly published study.

How can THC lead to anxiety and paranoia?

In the Oxford-led experiment, paranoia increased alongside negative feelings—including anxiety, worry, depression and negative thoughts about oneself—and unusual perceptual experiences. The researchers’ analysis found that these changes helped account for the increase in paranoia. Changes in working memory did not account for it.

That offers a plausible explanation for how THC can contribute to paranoia in the study: feeling worried or self-conscious and experiencing perceptions as unusual may make a situation seem threatening. It is an interpretation of this experiment, not a universal diagnostic rule or a complete account of every person’s reaction.

What did the brain study actually test?

Freeman and colleagues reported a randomized, placebo-controlled experiment involving 121 people with paranoid ideation. Participants received intravenous THC, placebo, or THC after a cognitive-awareness condition. Researchers assessed paranoia using a real social situation, an immersive virtual-reality task, self-report measures and interviews. The study found increased paranoia, negative affect and anomalous experiences after THC, as well as reduced working-memory capacity. The awareness condition had little impact. The paper appeared in Schizophrenia Bulletin in 2015.

Who took part—and who did not?

Participants were 21 to 50 years old, had used cannabis at least once, and had paranoid ideation. Oxford’s account says people with a history of mental illness and relevant health conditions were excluded. This selected sample is not representative of all cannabis users.

Why was THC injected?

Researchers used intravenous THC to reduce variation in participants’ blood THC exposure. Injection is not the same as smoking cannabis or consuming an edible, so the experiment does not establish that those routes produce the same effects, intensity or timing.

What do the study’s numbers mean?

Oxford’s 2014 summary gives these figures for the experiment. They describe its participants and conditions, not the general population of cannabis users.

Figure What it refers to How to interpret it
121 participants The selected sample in the randomized intravenous-THC experiment Participants already had paranoid ideation; this is not a representative sample of all cannabis users.
50% with THC; 30% with placebo Participants experiencing paranoid thoughts in the respective experiment groups, as reported by Oxford These are group proportions in this experiment, not estimates of how often cannabis causes paranoia among users.
About 90 minutes The period Oxford said THC had effects in the study participants This does not establish how long effects last with other doses, routes or individuals.

Oxford’s 16 July 2014 summary describes the dose as equivalent to a strong joint. Because the THC was injected, that comparison should not be taken as proof that an ordinary joint produces the same exposure or response.

Does this explain every anxious cannabis experience?

No. The experiment supports an acute effect under controlled conditions in a particular group, but it cannot predict how every person will respond to every cannabis product, dose or route. It also does not establish that a single episode of anxiety or paranoia means someone has a mental illness.

The CDC’s cannabis and mental-health guidance says cannabis can cause disorientation and sometimes unpleasant feelings of anxiety and paranoia. The CDC also discusses population-level associations between cannabis use and psychosis or other mental-health outcomes, noting a stronger association with schizophrenia for earlier and more frequent use. Possible short-term effects and longer-term associations are distinct claims; neither turns an individual episode into a diagnosis.

Why call it a “new” study?

The study matching this description is not new: Oxford summarized it in 2014, and the paper was published in 2015. Without identifying another study, it would be misleading to describe this experiment as newly published.
