Photos can make facial differences stand out because they capture one angle, one distance, one lighting setup and one instant. A close selfie, a slight head turn, uneven shadows or the unfamiliar left-right orientation of a photo can all change how your face looks. One picture is not a reliable test of facial structure.
Why can a photo make my face look uneven?
Distance and perspective change proportions
A photograph turns a three-dimensional face into a flat image. When the camera is very close, features nearer to it can look larger relative to features farther away. Wide-angle photography can add geometric distortion; DermNet’s clinical photography guidance gives an enlarged-looking nose as an example.

A 2023 study indexed by PubMed compared smartphone and full-frame photographs with a full-frame image taken from 60 inches as its reference. In that study’s setup, the tested 24, 35 and 50 mm focal lengths produced 12%–19% more vertical midface stretching than the reference. Those results describe the study’s cameras and conditions, not a universal distortion percentage for every phone or selfie. Read the study abstract on PubMed.

A small head turn or a camera held above or below eye level can make one side appear more prominent. A still image also freezes a moment: a slight difference in expression can make one cheek, eye or corner of the mouth look different from the other. Strong light from one side can cast a sharp shadow that exaggerates contours.

Clinical photography guidance emphasizes keeping position and expression consistent because small changes can alter appearance. The review of clinical photography for facial practice discusses standardized capture, neutral expression and controlled lighting. For skin photographs, the American Academy of Dermatology recommends natural light from a window.
Why do I look different in a photo than in the mirror?
A mirror shows a reversed left-right view; an ordinary photograph generally does not. Since faces are not perfectly symmetrical, reversing the orientation moves small differences to the opposite side. The result may feel unfamiliar because you are used to seeing your reflection—not because a photograph reveals a more defective or more truthful face.

A peer-reviewed article discusses how frontal facial images differ from their mirror images and why the two views are not superimposable. Read the article on mirror and frontal facial images. A separate study found a preference for mirror images among some patients with facial palsy, but that was a narrow clinical sample and should not be treated as evidence about everyday selfie users. How can I take a more consistent comparison photo?
If you want to compare images, keep the capture conditions as similar as you can. This reduces avoidable variation; it does not turn a photo into a diagnosis or a perfect representation of your face.
- Increase the camera distance. Avoid a very close, arm’s-length view; step back and crop afterward if needed. Distance is the key perspective factor, and no particular phone lens or focal length guarantees a true likeness. - Keep the camera near eye level and face it squarely. Use the same head position each time rather than comparing a straight-on photo with one taken from above or at an angle.
- Use a relaxed, neutral expression. A smile, squint or momentary movement can change how features appear in a still frame.
- Choose even, soft light. Window light is a practical option; avoid a strong one-sided source that casts a sharp shadow. The AAD’s photo guidance also recommends taking pictures from different perspectives when documenting a skin concern.
- Compare like with like. Use several images taken under similar conditions instead of drawing conclusions from one unusually flattering or unflattering frame.
When is facial asymmetry a medical concern?
Longstanding differences in appearance are not the same as a new change in facial movement. A single casual photograph cannot establish the cause of asymmetry. If a change is persistent, worsening, painful or accompanied by altered facial movement, seek a clinician’s assessment rather than relying on repeated selfie comparisons.

Sudden weakness or drooping on one side of the face needs urgent medical assessment. The National Institute of Neurological Disorders and Stroke lists sudden one-sided facial weakness or paralysis and a drooping eyebrow or mouth among Bell’s palsy symptoms.
