Those are different problems and need different fixes. Teams can improve results by making an agent’s interpretation correctable, setting a clear limit on repeated repair attempts, handing unresolved cases to people with the conversation context intact, and measuring resolution and customer sentiment separately from speed and self-service.
What “failure to answer” means
An answer can fail even when it is fluent, fast, and related to the topic. The useful question is whether the agent understood the customer’s intent and could carry out the work the customer needed.

- Misunderstanding: The agent settles on the wrong interpretation and responds or acts on it. The system has taken the conversation in the wrong direction.
- Non-understanding: The agent cannot interpret, answer, or complete the request. It may recognize the general topic but lack the capability or information to proceed.
The distinction matters operationally. A customer who was misunderstood may be able to correct an explicit interpretation. A customer whose request is beyond the agent’s capability needs a route to someone or something that can resolve it—not another paraphrase of the same failure.
Why a support agent can fail
Communication is a system problem, not just a model problem
Microsoft Research’s December 2024 report, Challenges in Human-Agent Communication, identifies twelve communication challenges for autonomous agents. It groups them around agents conveying information to users, users conveying information to agents, and challenges that affect human-agent communication more broadly. The report’s point is wider than factual accuracy: mismatched expectations, unclear communication, and insufficient user control can all undermine an interaction. It observes, “Although such agents can communicate with users through natural language, their complexity and wide-ranging failure modes present novel challenges for human-AI interaction.”

That framing changes what teams should inspect. A wrong answer may reflect a bad interpretation, unclear choices, missing context, or a poorly designed recovery path. Improving generated text alone will not necessarily fix those conditions.
Informational and transactional requests break down differently
A study by Gabriëlla Martijn, Charlotte van Hooijdonk, Hans Hoeken, and Florian Kunneman examined 200 real conversations with a rule-based, task-oriented chatbot at a Dutch public transport company. In that deployment, informational requests were generally recognized and often handled by the chatbot. Transactional requests were often recognized but redirected to a human. Misunderstandings were more common for informational requests, while non-understandings were more common for transactional requests.
This is evidence from one chatbot type and one company, not a universal failure-rate split. It does show why teams should not combine all contacts into one test set or treat every failed conversation as the same defect. Answering a question about a service and completing a transaction place different demands on an agent.
Repair attempts can restore—or further constrain—the conversation
The same study found different patterns in how the chatbot tried to repair misalignment. Confirmation made the chatbot’s interpretation visible and invited the customer to clarify. Offering options sometimes helped, but could constrain the exchange when the customer’s request did not fit the available menu. Rephrasing often led to repeated misunderstandings in this dataset.
These are observed patterns, not rules that every system will reproduce. They support a practical design principle: expose the interpretation in a way the customer can correct, but do not assume that a longer menu or another restatement will resolve an underlying capability gap.
Prior bot failure can affect the human handoff
A randomized online-chat field experiment at a meal-delivery company found that AI suggestions generally improved interactions, but the effect depended on the customer’s situation. When customers had already experienced chatbot comprehension failures, AI-assisted human-agent replies negatively affected sentiment. Unusually rapid human-agent responses also led some customers to think they were still speaking with a chatbot.
The implication is not that AI suggestions should always be withheld after a bot failure. It is that the handoff is part of the service experience: a customer may arrive frustrated, and an apparently automated or context-free response can compound that frustration. The human agent needs the prior interaction and a clear signal that the customer has already tried the bot.

| What to compare | What it helps reveal | Evidence-based interpretation |
|---|---|---|
| Informational vs. transactional requests | Whether the agent can explain an issue versus complete an action | In the 200-conversation Dutch public-transport chatbot study, these request types showed different patterns of misunderstanding, non-understanding, and human redirection. |
| Misunderstanding vs. non-understanding | Whether the agent chose the wrong intent or could not proceed | Confirmation may help surface a wrong interpretation; an inability to complete work calls for capability review or escalation. |
| Fresh conversations vs. contacts after a bot failure | Whether prior frustration changes the effect of automated assistance | In the meal-delivery field experiment, AI-assisted replies after chatbot comprehension failures negatively affected sentiment. |
| Resolution, sentiment, efficiency, and self-service | Whether an operational gain is also a customer outcome | The meal-delivery experiment found variation by issue type; the Qualtrics survey and Nubank deployment report use different populations and measures, so their figures are not interchangeable. |
How to improve AI support-agent results
1. Make the agent’s interpretation inspectable
For ambiguous or consequential requests, have the agent briefly state what it thinks the customer wants before it proceeds. Give the customer an obvious way to correct that interpretation. This addresses misunderstanding directly: the customer can identify the wrong turn before it becomes an incorrect action or a longer chain of irrelevant answers.
Keep confirmation proportional to the risk and ambiguity. A confirmation step should clarify the intended task, not add friction to every straightforward exchange. The interaction study supports confirmation as a repair strategy in its setting; teams should evaluate how it works with their own customers and tasks.
2. Offer choices without making the menu a dead end
Use fixed options when they represent plausible customer intents and help narrow the next step. Preserve a free-form route for requests that do not fit those choices. A menu that excludes the customer’s actual need can turn an attempt at clarification into another mismatch.

Set a repair limit for a conversation. If the customer has corrected the agent and the mismatch remains, stop rephrasing the same interpretation and move to a human or another appropriate resolution path. This is a practical recommendation based on the Dutch chatbot study’s finding that rephrasing often reproduced misunderstanding; it is not a universally tested numeric threshold. The important operational choice is to prevent an unresolved loop from becoming the customer’s only option.
4. Design escalation as part of the service, not as an error screen
When an agent cannot resolve a request, transfer the case with enough context for the person taking over to understand both the customer’s goal and what has already failed. Include the relevant conversation history and indicate whether the customer has experienced a comprehension failure. A handoff that makes the customer repeat the issue can erase the value of escalation.
Also make it clear that a person has taken over. In the meal-delivery field experiment, unusually fast AI-assisted human replies led some customers to believe they were still interacting with a chatbot. Human support teams should balance promptness with a response that demonstrates understanding and makes the change in responsibility clear.
5. Evaluate by scenario, not just by average
Separate test and production reviews by at least request type, repeat-complaint status, and whether the customer has already encountered a bot failure. The meal-delivery experiment found that AI suggestions improved efficiency and sentiment for subscription cancellations, but were least effective for repeat complaints involving systemic issues beyond the AI’s capability. A strong result on routine cancellations would not establish that the same assistance works for a recurring problem that requires a broader remedy.

For each segment, record whether the issue was resolved, what happened to customer sentiment, how efficiently the interaction proceeded, and whether self-service succeeded. Keep those outcomes distinct: a shorter conversation or higher self-service rate does not by itself establish that the customer’s issue was resolved or that the experience improved.

6. Iterate with production evidence and human review
A 2026 paper describing Nubank support deployments connects structured context engineering, human-in-the-loop prompt iteration, evaluation, and validation in production. These elements form a useful improvement cycle: provide relevant context, inspect failures with people who understand the work, revise the system, evaluate it against real scenarios, and validate the change in deployment. The reported organizational results describe Nubank’s experience; they do not establish that the same framework or gains transfer unchanged to other companies.

Response time and self-service are useful operational measures, but they answer different questions from resolution and sentiment. Track a balanced set of outcomes and examine them by the scenarios that create different demands on the agent.
- Issue resolution: Was the customer’s underlying need addressed, including when a human took over?
- Customer sentiment: How did the customer feel about the interaction, especially after a prior failure or on a repeat complaint?
- Response efficiency: Did the interaction progress without unnecessary turns or a repeated repair loop?
- Self-service: Did the agent complete the request without a human, and was that completion actually successful?
Qualtrics’ 2026 customer-service article summarizes a survey of more than 7,000 consumers across seven countries and seven industries. Under its measurement framework, consumers rated agent understanding 37% lower when issues were unresolved, falling from 4.44 to 2.78; the reported drops for knowledge and friendliness were 34% and 20%, respectively. Those are survey-specific results, not universal benchmarks. They underscore why understanding and resolution deserve their own attention rather than being inferred from friendliness or response speed.
The 2026 Nubank paper reports a 37 percentage-point improvement in AI transactional NPS and a 29 percentage-point gain in self-service rate for a card-delivery deployment, compared with prior agent variants. Those figures belong to that deployment and comparison; they are not expected gains for other support teams. The result also illustrates why reporting multiple metrics matters: the self-service change and the NPS change are separate outcomes, not substitutes for one another.
A practical review checklist
- Label failures as misunderstanding, non-understanding, or another clearly defined issue instead of counting every unsuccessful contact alike.
- Test informational and transactional tasks separately.
- Check whether customers can correct an interpretation and escape menus that do not match their needs.
- Review repeated rephrasing and define when the agent should stop and escalate.
- Give human agents the previous conversation and identify cases that follow a bot comprehension failure.
- Segment results by repeat complaints and prior bot failure, not just by channel or overall volume.
- Report issue resolution and customer sentiment alongside speed and self-service, without treating movement in one measure as proof of movement in another.
Frequently Asked Questions
What is the difference between an AI support agent misunderstanding a request and not understanding it?
A misunderstanding means the agent proceeds on the wrong interpretation. Non-understanding means it cannot interpret or handle the request. The distinction points to different responses: make interpretations correctable in the first case, and address capability or escalation in the second.
Should an AI support agent always offer multiple-choice options?
No. Options can help when they reflect likely customer intents, but they can box a customer in when the real request is missing from the menu. Keep a free-form route available.
Does faster AI-assisted support always improve customer experience?
No. In a randomized online-chat field experiment at a meal-delivery company, unusually rapid AI-assisted human replies led some customers to think they were still speaking with a chatbot. Speed and sentiment should be assessed separately.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Clear out junk files and repair common Windows errors3Fix the driver behind crashes, sound loss and screen glitchesCan higher self-service prove that an AI support agent is resolving more issues?
No. Self-service and resolution are distinct measures. Track whether the customer’s issue was actually addressed as well as whether a human was involved.
