Why AI Agents Fail at Multi-Step Tasks—and How to Improve Reliability

Multi-step agent failures can start with unclear intent, flawed plans, tool errors, or misread results. Learn how to find the first consequential failure and measure reliability beyond one score.

AI agents often fail at multi-step tasks because success depends on a chain: understanding the request, planning, choosing and using tools, interpreting results, and completing every constraint. A mistake at any stage can derail later steps. Improving reliability means finding where the chain breaks and testing consistency, robustness, confidence, and failure severity—not relying on one model upgrade or a single success score.

Why multi-step tasks expose more failure points

A task that requires several actions is not simply a harder version of a one-step question. The agent must carry intent and constraints through a sequence of decisions, often using tools and updating its understanding as new information arrives. It can make a plausible choice at one step and still fail the overall task because a later action does not follow from the tool output or violates an earlier requirement.

Microsoft Research’s AgentRx taxonomy separates several points where a run can break. That distinction matters: an apparent reasoning failure may instead come from an unclear request, an invalid tool call, a policy block, or a system fault.

Intent and planning failures

  • Intent-plan misalignment: the plan does not serve what the user actually asked for.
  • Under-specified intent: the request leaves an important choice unresolved, but the agent proceeds as if it were clear.
  • Plan-adherence failure: the agent forms a reasonable plan but deviates from it without a sound reason.

Tool and state failures

  • Invalid invocation: a tool is called with the wrong arguments or in a way that violates its schema.
  • Output misinterpretation: the agent receives a result but reads it incorrectly or fails to carry its implications into the next decision.
  • Invented information: the agent introduces facts that were not supplied by the user or established by the tools.

Capability, policy, and infrastructure failures

  • Unsupported request: the agent attempts something beyond its available tools or capabilities.
  • Guardrail block: a safety or policy constraint prevents an action.
  • System failure: infrastructure or another component fails independently of the agent’s reasoning.

These categories suggest different fixes. Better task specifications can reduce ambiguity; schema checks can catch malformed tool calls; clearer traces can reveal a mistaken interpretation; and infrastructure faults need operational remedies rather than a new prompt.

How one mistake cascades into task failure

An early wrong assumption can shape the plan, the selected tools, and the interpretation of later results. The agent may then produce a fluent final response that conceals the point where the run went off course. The paper Where LLM Agents Fail and How They can Learn From Failures describes this as a cascading failure: a root-cause error propagates through later decisions until the task fails.

Long, probabilistic trajectories make these failures harder to diagnose. The same input can lead to different outputs, and in multi-agent systems one component can pass an error to another. A final answer alone usually does not show whether a side effect actually occurred or which decision first made success unlikely. The useful diagnostic target is the earliest consequential breach in the trajectory, not merely the last visible symptom.

What benchmark results can—and cannot—tell you

Benchmark scores describe performance under a particular task set and evaluation method; they are not a general failure rate for all agents. TravelPlanner, a travel-planning benchmark published in 2024, contains 1,225 curated planning intents and reference plans in a sandbox with nearly four million data records. Its authors report a 0.6% success rate for GPT-4 on that benchmark. They identify difficulty staying on task, selecting appropriate tools, and tracking multiple constraints. That result illustrates the challenge of that benchmark’s travel-planning tasks; it does not estimate how often AI agents generally fail.

GAIA is another benchmark for real-world questions involving multi-step reasoning, tool use, web browsing, and file manipulation. Its task levels range from shorter chains to multi-tool reasoning and long-horizon plans. The Princeton HAL dashboard reports measures including exact-match accuracy, consistency across repeated runs, confidence calibration, and robustness to formatting perturbations. Because the dashboard is dynamic, any quoted score or ranking needs a dated snapshot and should remain tied to the listed system and evaluation.

Measure reliability beyond a single success score

Towards a Science of AI Agent Reliability frames reliability across several dimensions. Its findings are specific to the evaluated models, benchmarks, and perturbations, but support a practical lesson: raw accuracy can conceal operational weaknesses. Across the evaluated agentic models and two benchmarks, the authors report that capability gains produced only small reliability improvements.

Dimension What to ask Useful evaluation
End-to-end success Does the agent complete the defined task and satisfy all required constraints? Score a representative task set using an explicit completion rule.
Consistency Does the same system succeed repeatedly on the same task? Repeat runs and compare outcomes, not just average scores.
Robustness Does performance hold when wording, formatting, tools, or environment conditions change? Test equivalent prompt formulations and relevant tool or interface perturbations.
Calibration Does confidence distinguish likely successes from likely failures? Compare stated confidence with observed outcomes across runs.
Safety and severity When a run fails, how harmful or irreversible is the result? Track failure types and consequences, especially for high-impact actions.
Diagnostic visibility Can an evaluator determine what happened and where the run first failed? Retain a trace of plans, tool calls, returned values, and state changes.

A practical workflow for improving reliability

  1. Define success and boundaries. State what counts as completion, which constraints must hold, what actions are prohibited, and when missing information requires clarification or a stop. If the task has several requirements, make them checkable rather than leaving completion to a vague final judgment.
  2. Record the execution trace. Preserve the plan, each tool call and its returned value, and relevant state changes. Do not treat a natural-language summary as evidence that an external action—such as a change or submission—actually happened.
  3. Validate actions and results as they occur. Check tool arguments against the tool schema before invocation. After a result arrives, verify that it is interpreted correctly and that relevant task constraints still hold. Checking only at the end makes it harder to contain a bad assumption.
  4. Find and classify the first consequential failure. Review the trace from the beginning and identify the earliest step that made the outcome incorrect or unsafe. Classify it as an intent or planning problem, tool invocation error, output misinterpretation, unsupported capability, guardrail block, or system fault; then address that cause rather than patching only the final response.
  5. Repeat and perturb evaluations. Run tasks more than once and test relevant prompt or environment variations. Report the task set, conditions, and scoring method alongside results so another reader can tell what the numbers mean.

What failure diagnosis can add

AgentRx offers an example of trajectory-level diagnosis. It normalizes different kinds of logs, derives executable constraints from tool schemas and policies, checks those constraints step by step, records evidence-backed violations, and uses a grounded judge to identify the critical failure step. In a benchmark of 115 manually annotated failed trajectories spanning τ-bench, Flash, and Magentic-One, Microsoft Research reports a 23.6 percentage-point improvement in failure-localization accuracy and a 22.9 percentage-point improvement in root-cause attribution over prompting baselines. Those are diagnostic gains in the stated experiment, not evidence of a universal increase in task completion.

Better diagnosis makes it possible to target the right intervention: clarify an ambiguous task, correct a tool interface, improve output validation, add a stop condition, or repair a system fault. Whether any particular intervention improves reliability still needs to be measured on the tasks and conditions that matter for its intended use.
