The Nuka-Cade prize terminal lists Daddy-O as a level 4 prize costing 2,500 tickets. The prize selection rotates every couple of days, so this is a changing option rather than a guaranteed item available at all times.
What Daddy-O does
According to the Fallout Wiki item entry, Daddy-O grants +3 Intelligence and +3 Perception while applying -2 Charisma for eight minutes. The entry gives its form ID as 00156D0B. A Fallout 4 loading-screen hint describes it as popular with beatniks and intellectuals before the Great War, and says it raises Intelligence and Perception while lowering Charisma for a limited time.
Addiction risk and the disputed penalty
The Daddy-O item entry and the separate addiction reference both report a 35% addiction chance per dose. They do not agree on the exact Fallout 4 penalty: the item page lists -1 Intelligence and -1 Perception, while the addiction page says addiction is indefinite and causes -1 Intelligence until cured. Because those references conflict, the precise debuff should not be treated as settled. The shared 35% chance is the consistent figure in both sources.
