When can you go back to work after the flu?
For most workers, both parts of CDC’s return rule must be true for at least 24 hours: your symptoms are getting better overall, and you have not had a fever while off fever-reducing medication. One condition alone is not enough. In its workplace guidance, CDC defines a fever as a temperature of 100°F (37.8°C) or higher. See CDC’s Flu: What To Do If You Get Sick.

So, “24 hours after the fever breaks” is incomplete: symptoms must also be improving, and medicine should not be masking a fever. If you still feel too unwell to work, continue to rest at home.
What if you have flu but no fever?
Flu does not always cause a fever. If you have suspected or confirmed flu without a fever, CDC’s flu-specific advice is to stay home from work for at least five days after your symptoms began. The usual fever-free return test does not replace this no-fever instruction.

What precautions should you take when you return?
Returning to normal activities does not mean transmission risk is gone. CDC says people with flu are most contagious during the first three days of illness, although some people, including young children and people with weakened immune systems, may be contagious longer. These are general patterns, not a way to determine whether a particular person is still infectious.
For the five days after resuming normal activities, take added precautions around other people, especially anyone at higher risk of severe respiratory illness. CDC’s examples include:

- Improving airflow or choosing cleaner air.
- Practicing good hygiene.
- Wearing a well-fitting mask.
- Keeping physical distance.
- Testing when around other people.
These measures are additional precautions; they do not replace the stay-home criteria. CDC’s respiratory-virus guidance describes the five-day period after returning.

Does the rule differ for healthcare and patient-care workers?
Yes. Healthcare settings have separate infection-control rules because workers may expose patients, including people at higher risk of serious illness. CDC’s seasonal influenza guidance says healthcare personnel with fever and respiratory symptoms should not report to work. If those symptoms begin at work, they should stop patient-care activities, put on a facemask, and notify the appropriate supervisor or infection-control or occupational-health contact before leaving. Exclusion continues until at least 24 hours after fever ends without fever-reducing medication; ongoing respiratory symptoms may need occupational-health assessment, particularly for staff with patient contact. CDC also has broader, separate work-restriction recommendations for healthcare personnel with viral respiratory infections. Follow your facility’s policy and occupational-health instructions rather than applying the public’s general return rule to patient-care work. CDC says treatment benefit is greatest when it starts within two days of illness onset, although treatment started later may still help some people who are at higher risk or very ill.
