Seek medical advice promptly if diarrhea comes with blood or pus in the stool, severe abdominal or rectal pain, repeated vomiting, dehydration, or a change in alertness. For adults, US National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) guidance also advises contacting a doctor for diarrhea lasting more than two days, a high fever, or six or more loose stools in a day. Children and people at higher risk may need care sooner.
When should an adult seek medical care?
Do not wait for a duration threshold if a serious warning sign appears. NIDDK says adults should contact a doctor right away for:

- Confusion or another change in mental state.
- Frequent vomiting, or being unable to drink enough fluids or oral rehydration solution.
- Severe pain in the abdomen or rectum.
- Black, tarry stool, red blood in the stool, or pus in the stool.
- Signs of dehydration, such as very little urine, dizziness, or unusual tiredness.
For adults without those urgent signs, NIDDK also lists diarrhea lasting more than two days, a high fever, or six or more loose stools in one day as reasons to talk with a doctor. These are care thresholds, not a recommendation to wait if symptoms are worsening. NIDDK’s US guidance is at Symptoms & Causes of Diarrhea.

People who should be more cautious
Older adults, pregnant people, people taking antibiotics, and people with weakened immunity are more likely to develop problems with viral gastroenteritis, according to NIDDK. Keep in contact with a clinician if you fall into one of these groups, particularly if you are struggling to keep fluids down or are not improving with oral rehydration.

When does diarrhea in a child need a doctor?
Children can become dehydrated quickly, and the adult thresholds do not apply to them. NIDDK advises prompt contact with a doctor for a child with diarrhea lasting more than a day, frequent loose stools, frequent vomiting, severe pain, dehydration signs, or black, bloody, or pus-containing stool. It also identifies any fever in an infant and a high fever in an older child as reasons to seek advice.
Contact a doctor promptly if an infant refuses food or drink for more than a few hours. Seek advice sooner for a child under 12 months, a child born prematurely, or a child with another medical condition. If the child cannot drink enough or does not improve with oral rehydration, contact a clinician rather than relying on home care alone. NIDDK’s child and gastroenteritis guidance is available at Symptoms & Causes of Diarrhea and Symptoms & Causes of Viral Gastroenteritis.

How to recognize dehydration
Diarrhea can cause dangerous fluid loss. Watch for a combination of symptoms and for whether they are getting worse:
- Thirst or a dry mouth.
- Urinating less than usual, or dark urine.
- Tiredness, dizziness, or persistent dizziness when standing.
- Sunken eyes, or unusually fast breathing or heart rate.
In babies and young children, possible signs include fewer wet diapers or no wet diaper for three hours or more, no tears when crying, a sunken soft spot on the head, and unusual drowsiness. NIDDK and the NHS list these as signs that need attention; anyone with dehydration symptoms should seek medical advice promptly. The NHS’s Dehydration guidance is UK-specific, while NIDDK’s patient guidance applies to the United States.

When is it an emergency?
Get emergency help if dehydration is accompanied by confusion, difficulty waking, difficulty breathing or rapid breathing, cold skin, or blue, grey, pale, or blotchy skin. The NHS says these can be signs of shock. In the UK, call 999 or go to A&E; elsewhere, use your local emergency number or emergency department. Do not delay emergency care to try rehydration at home.
For less severe but concerning dehydration signs, the NHS advises urgent contact with a GP or NHS 111 in the UK. These routes are not universal: outside the UK, use your local urgent-care service, clinician, or medical advice line. Severe sudden abdominal pain, difficulty breathing, or not responding normally also calls for urgent assessment.

What to do while monitoring symptoms or arranging care
Replace fluids and electrolytes
If symptoms are mild and there are no warning signs, drink fluids regularly. Water and fluids containing electrolytes can help replace what is lost; an oral rehydration solution is one option. If nausea makes drinking difficult, take small, frequent sips. Oral rehydration sachets should be mixed as directed. The NHS advises asking a pharmacist which solution is appropriate.

Continue feeding infants safely
Continue breastfeeding or bottle-feeding. Do not dilute or weaken infant formula. Keep offering fluids and seek medical advice if a baby or child cannot drink enough, is vomiting repeatedly, or shows dehydration signs.
Be careful with medicines
Do not give a child under 12 medicine to stop diarrhea, says the NHS. NIDDK says clinicians typically advise against over-the-counter antidiarrheal medicines for infants and children, and for people with bloody stool or fever. If you are already taking an over-the-counter medicine and diarrhea worsens or lasts more than two days, contact a doctor. Ask a clinician or pharmacist what is appropriate for your circumstances.
Eat as appetite returns
When appetite comes back, people can usually return to their normal diet, according to NIDDK. The NHS says diarrhea often stops within five to seven days and vomiting within one or two days; these are typical duration descriptions, not guarantees or reasons to wait through warning signs. NHS self-care advice is at Diarrhoea and vomiting.
