The main established drug treatment for erythropoietic protoporphyria (EPP) is afamelanotide, sold as SCENESSE. In the United States, it is a 16 mg implant placed under the skin by a trained healthcare professional every two months to increase pain-free light exposure in adults with a history of EPP phototoxic reactions. It can help, but it does not replace protection from sunlight and other triggering light. Treatment also depends on reducing exposure and planning for symptoms when a reaction occurs.
How EPP treatment is organized
EPP treatment has two distinct aims: preventing phototoxic reactions by limiting triggering light, and increasing tolerance to light where an approved therapy is appropriate. Measures used to ease pain during a reaction are symptom relief, not prevention. The available approaches differ in evidence, local authorization and access, so treatment decisions should be made with a clinician familiar with porphyria.

Afamelanotide (SCENESSE): the main established drug treatment
What it does and who it is for
Afamelanotide is a long-acting analogue of alpha-melanocyte-stimulating hormone. By activating the melanocortin-1 receptor, it increases production of eumelanin, the pigment that helps protect skin from light. A 2023 peer-reviewed consensus guideline recommends it to prevent phototoxic symptoms in EPP and reports increased pain-free sun exposure and improved quality of life in adults.
Recommended Free Tools
In the United States, the FDA indication is to increase pain-free light exposure in adults with a history of phototoxic reactions from EPP. SCENESSE received US marketing approval on October 8, 2019. The European Medicines Agency describes its use as prevention of phototoxicity in adults with EPP; its materials also discuss seasonal timing. Indications, access, administration arrangements and reimbursement vary by jurisdiction, so check the local product information and discuss suitability with a specialist.

How it is given and what the study found
Each SCENESSE implant contains 16 mg of afamelanotide. A healthcare professional proficient in the procedure and trained before administration inserts it under the skin every two months. In a six-month study of 93 patients summarized by the EMA, mean direct sunlight exposure without pain between 10 am and 6 pm was 116 hours with SCENESSE and 61 hours with placebo. These are study averages, not a promise of how much any individual will be able to tolerate. The EMA notes that the medicine was authorized under exceptional circumstances and that longer-term benefit and safety information was still being collected through a registry.
Safety and monitoring
The FDA label warns of hypersensitivity, including serious reactions, and notes that treatment may darken existing moles and freckles. It recommends regular full-body skin examinations. Patients should review their medical history, other medicines and monitoring plan with their clinician.

Light protection and practical changes
Light protection remains part of care whether or not someone uses afamelanotide. Consensus guidance recommends avoiding sunlight when possible and wearing opaque clothing. Because exposure can happen in everyday settings, practical changes can also matter:
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
- Consider car window tinting to reduce light exposure while travelling.
- Discuss accommodations and education with a school or workplace when needed.
- For sensitivity to indoor lighting, consider indoor lights that minimize blue light.
- For prolonged surgery, the guideline lists filters that limit transmission of light in the 340–470 nm range as an option.
These steps reduce exposure; they do not make all environments or lighting risk-free. A clinician can help tailor precautions to a person’s triggers and daily routine.

Does sunscreen help with EPP?
Sunscreen should not be treated as a substitute for opaque clothing or light avoidance. The consensus guideline says some patients report a small benefit from broad-spectrum or tinted products, including formulations with zinc oxide or titanium dioxide, which block some far-UVA wavelengths that can activate protoporphyrin. It says non-broad-spectrum or non-tinted sunscreens are not useful for preventing protoporphyria phototoxic symptoms.
Options when afamelanotide is unavailable or unsuitable
A consensus guideline says limited natural sunlight exposure, increased gradually, or carefully increased low-dose narrowband UVB phototherapy may be considered to encourage gradual production of the body’s own melanin. The effectiveness of these approaches is unclear and not well established; they should not be presented as equivalent to afamelanotide in evidence. Any light-hardening approach should be discussed with a clinician familiar with EPP.

What to do during a phototoxic episode
Cold compresses or cooling devices may be considered to ease symptoms during an episode. The consensus guideline reports no evidence of benefit for narcotic analgesics, oral or topical corticosteroids, antihistamines, acetaminophen or NSAIDs for EPP phototoxic symptoms, and identifies pain treatment as an unmet need. Ask a clinician about an individual symptom-management plan rather than assuming medicines used for other kinds of pain or inflammation will help.

Investigational treatment
The 2023 consensus guideline described dersimelagon, an oral melanocortin-1 receptor agonist, as under investigation for preventing protoporphyria phototoxicity. At that time, a Phase 2 study had been completed and a Phase 3 study was ongoing. That information does not establish the trial’s current status or whether the medicine has since been authorized; consult current local regulatory information or a porphyria specialist for an update.
