Do not ask the agent to repair its own mistake or treat a safe-looking final reply as proof that no data left the system. Then determine what information the agent accessed, which tools it used, where data went, and whether notification or recovery steps apply.
First response: stop further activity
Act in this order when an agent has revealed information, sent a message, changed a file, or behaved in a way you cannot explain. Speed matters, but preserve evidence before deleting or resetting anything.

- Use an external kill path. Pause the agent, scheduled job, workflow, integration, API route, or connected tool from its administrative console, cloud control plane, endpoint-management system, or network control. If the product has a documented emergency-disable procedure, use that. Do not tell the agent to “undo” its action: a refusal in the final response does not reverse a tool call that already occurred. The U.S. Department of Energy’s AI security guidance and OWASP prompt-injection guidance both emphasize an independent control that can prevent additional actions.
- Revoke access that may be exposed. Disable or rotate API keys, OAuth tokens, service-account credentials, passwords, session cookies, webhooks, and integration permissions. Invalidate active sessions where the identity system allows it. If a token was embedded in a prompt, tool argument, log, or generated output, treat it as compromised even if you do not see misuse. Stolen credentials remain useful until changed, a point highlighted by the Federal Trade Commission (FTC).
- Isolate the affected connection. Temporarily disconnect the agent from mail, cloud storage, databases, source repositories, payment systems, or messaging platforms. For a business, security staff may isolate the host, workload, tenant, or network segment while keeping a controlled copy available for investigation.
- Preserve evidence. Save the complete conversation, system and developer instructions, retrieved documents, attachments, tool names, arguments, returned data, destination addresses, approval records, timestamps, user and service identities, audit logs, and the exact agent, model, connector, and tool versions. Restrict this material to the response team. Do not paste secrets into a normal support ticket or public forum, and do not destroy forensic evidence while trying to clean up.
- Escalate through a verified route. Employees should use the organization’s security incident channel and notify the privacy or legal lead named in its incident plan. Contact the agent or integration provider through a verified support or security route when its service, logs, or access controls are involved. Individuals should secure the affected account and use the provider’s official support channel, not a contact address supplied by the suspicious output.
Classify what happened before choosing a remedy
“The AI leaked my data” can describe very different events. Classification determines whether the priority is a credential reset, a takedown request, account recovery, digital forensics, or formal notification.
| Incident pattern | What to establish | Immediate emphasis |
|---|---|---|
| Visible output exposure | Who saw the response, whether it was stored or shared, and whether it included secrets or personal information | Restrict the conversation, remove copies where possible, and rotate any disclosed credentials |
| Tool-mediated transfer | Which tool ran, its arguments, destination, response, and authorization scope | Disable the tool or connector, revoke its permission, and preserve provider-side logs |
| Account or session compromise | Which identity was used, active sessions, sign-in history, and changes made under that identity | Invalidate sessions, reset credentials, enforce stronger authentication, and review account changes |
| Cross-user or tenant exposure | Which other user, tenant, or service could access the data and for how long | Contain the access boundary, preserve access logs, and involve the provider and privacy lead |
| Malicious content manipulation | Whether an email, file, web page, or retrieved record contained instructions that redirected the agent | Quarantine the content, stop retrieval, and investigate for indirect prompt injection |
Also record whether access is still possible, whether the information is already outside your environment, the sensitivity and volume of the data, and the countries or contractual environments involved. Those facts shape the response more reliably than the agent’s explanation.

How an agent can mishandle information
Indirect prompt injection (agent hijacking)
An agent combines instructions with content it reads. A hostile instruction hidden in an email, document, ticket, web page, or database field can influence the agent even though the user never typed it. NIST describes this as agent hijacking, a form of indirect prompt injection that can cause unintended actions. In a 2025 Center for AI Standards and Innovation evaluation using held-out Workspace tasks and attacks developed for an upgraded Claude 3.5 Sonnet, the strongest baseline attack succeeded 11% of the time and the strongest new attack 81% of the time. These are controlled evaluation results, not a probability that a real-world incident will occur.
Excessive agency and broad permissions
An agent may have permission to read a mailbox and also to send mail, or to read cloud files and also delete or share them. OWASP’s excessive-agency example shows how a malicious incoming message could manipulate a mailbox-summarization agent into forwarding sensitive inbox contents when the integration also has send permission. Remove capabilities the workflow does not need, reduce OAuth scopes, separate read and write identities, and require a person to review high-impact actions such as sending, publishing, deleting, or paying.
Memory, retrieval, and tool-chain failures
Poisoned memory, unsafe retrieval results, data exfiltration through outputs, and high-impact actions without oversight are recognized OWASP risks. In Model Context Protocol (MCP)-enabled setups, relevant warning areas include secret exposure, permission-scope creep, tool poisoning, command injection, contextual prompt injection, weak authorization, and insufficient audit telemetry. MCP is not required for an incident; use this list as an investigation checklist only when MCP servers or tools are part of the architecture.

Preserve a usable evidence record
Investigators need to reconstruct the agent’s state and every external effect. Create a restricted incident folder or case record and capture:
- Full prompts and responses, including system, developer, user, retrieved, and tool-generated content.
- Source files, emails, web pages, records, and attachments supplied to the agent, with hashes or immutable copies when your process supports them.
- Every tool call, argument, result, destination, authorization decision, human approval, and error.
- Identity-provider, application, cloud, database, network, connector, and vendor audit logs covering a window before and after the event.
- Timestamps with time zone, user and service-account identifiers, workflow or job IDs, model and agent versions, connector versions, and configuration changes.
- Copies or screenshots of exposed pages, messages, or files, while avoiding further distribution of the sensitive content.
Record who collected each item and when. Ask providers to preserve server-side logs if their retention window is short. Keep the evidence access-controlled because it may contain the same secrets or personal information involved in the incident.
Determine what the agent accessed and where it went
Containment is not complete until you know the data path. Build a timeline from logs and provider records, then answer these questions:

- [Dont Hesite to Order] 24 hours customer services and professional technology team are standing by
- What repositories, messages, databases, files, memories, or secrets could the agent read at the time?
- What did it actually retrieve, transform, quote, summarize, or store?
- Did it invoke a tool, plugin, connector, browser, code runner, or external API?
- What exact destinations received data, including email addresses, URLs, storage locations, or other tenants?
- Was the output visible only to the requesting user, or to another user, administrator, public page, search cache, or downstream service?
- Could a third party still access the information through a live link, token, shared object, cached copy, or retained provider log?
- What changes did the agent make, and have those changes been reversed safely?
Check the agent’s final response against tool and access logs. A harmless-looking answer does not prove that a transfer, deletion, or permission change did not occur. The FTC recommends reviewing logs, examining service-provider access, determining the information and people affected, and removing improperly posted information while considering cached copies.

Organization response: coordinate, investigate, and verify
Follow the organization’s incident-response plan rather than letting an individual operator improvise. A practical team may include information security, IT, digital forensics, privacy, legal counsel, operations, communications, and management. The FTC recommends mobilizing such a coordinated response quickly, securing systems, updating credentials, preserving evidence, reviewing provider access, and testing that fixes worked.
Contain without destroying the trail
Use a controlled suspension or read-only mode where possible. Preserve a forensic copy before rebuilding a workload or deleting a connector. Keep a record of every containment action, its time, and its owner. If the agent is embedded in a production process, identify safe manual or failover procedures so business continuity does not require re-enabling unsafe automation.

Validate remediation
After changing keys, scopes, filters, or policies, test with non-sensitive data. Confirm that old tokens fail, sessions are invalidated, removed permissions are no longer usable, and audit events appear. Recheck scheduled jobs, webhooks, cached links, shared folders, and downstream queues; an overlooked secondary path can recreate the exposure.
Decide whether independent forensics is warranted
An organization may consider an independent forensic investigator when the source or scope is unclear, evidence is distributed across providers, regulated information is involved, or legal counsel needs a defensible technical record. This is a risk-based decision, not a requirement for every mistaken response.

Do not announce a universal deadline or assume every agent error is a legally reportable personal-data breach. Duties depend on jurisdiction, the type of information, the organization’s role, contracts, and whether unauthorized access or disclosure is established. The FTC notes that all U.S. states and certain territories have breach-notification laws, with additional rules for some health-record incidents. A privacy or legal lead should assess current law wherever affected people or systems are located.
Best Value
When communication is required or prudent, state:
- What is confirmed and what remains under investigation.
- The dates or period involved, without exposing further sensitive details.
- The categories of information that may have been involved.
- What the organization has done to stop access and prevent recurrence.
- Specific actions affected people can take and where verified updates will appear.
Avoid misleading reassurance, unexplained silence, or publishing technical details that create another attack path. Keep a single verified update channel and coordinate public statements with security and legal teams.
Individual recovery after an exposure
Match follow-up to the information involved rather than applying the same remedy to every event.
- Passwords and authentication data: Change the affected password from a trusted device, do not reuse it elsewhere, revoke sessions, and enable multifactor authentication. Reset any other account where the same secret was reused.
- API keys, tokens, or recovery codes: Revoke and reissue them through the service’s official console. Remove copies from prompts, scripts, notebooks, and shared documents.
- Payment information: Contact the relevant bank, card issuer, or payment service using the number on an official statement or card. Ask what replacement or transaction-monitoring steps fit the account.
- Government identifiers or identity data: Follow the recovery guidance appropriate to your country. For U.S. consumers, the FTC directs people to IdentityTheft.gov for steps tailored to the information exposed; contacting credit bureaus may be appropriate in some cases.
- Private messages, files, or photographs: Remove public links, revoke shares, ask the host to take down improperly posted material, and look for cached or downloaded copies. Inform people whose information appears in the material when appropriate.
Credit freezes, fraud alerts, monitoring services, or identity-restoration support are not automatic requirements and cannot guarantee that misuse will not occur. If an organization offers monitoring or recovery assistance, verify exactly what it covers, for how long, and how to enroll through an independently verified channel.
Recommended Free Tools
Prevent a repeat incident
- Give each agent the minimum read, write, send, and delete permissions required for its task.
- Use separate identities for retrieval and high-impact actions; make sensitive actions require human approval.
- Segment confidential data and apply field-level filtering before content reaches a model.
- Treat retrieved content as untrusted instructions. Keep external text separate from control instructions and test against indirect prompt injection.
- Set destination allow-lists, rate limits, data-loss controls, and egress monitoring for tools and connectors.
- Log prompts, retrievals, tool calls, approvals, outputs, and permission changes with retention appropriate to the risk.
- Maintain an external emergency stop that operators can use when the agent is unresponsive or manipulated.
- Review OAuth scopes, MCP server permissions, plugin manifests, webhooks, and service-account memberships on a schedule.
- Run controlled exercises that test secret exposure, cross-user access, mass export, unauthorized sending, and recovery procedures.
- Document who owns the workflow, who can suspend it, who investigates, and who makes privacy-notification decisions.
The central lesson is operational: an agent’s permissions define the potential blast radius. Limiting those permissions, retaining reliable telemetry, and keeping a human-controlled shutdown path turn an ambiguous AI error into an incident that can be contained and investigated.
