A CDX2 result is a clue about how tumor cells are differentiating, not a stand-alone answer about where a cancer began. Ask your pathologist how the stain fits the tumor’s appearance, the other immunohistochemistry (IHC) results, the quality of the sample, and your clinical picture—and what uncertainty remains.
What a CDX2 test can—and cannot—tell you
CDX2, short for caudal-type homeobox 2, is a transcription factor associated with intestinal epithelial differentiation. In pathology, an antibody stain can show whether tumor cells express CDX2. The result helps describe the tumor’s phenotype—the characteristics it shows under examination—and may support consideration of an intestinal-type origin.

A positive stain does not, by itself, establish that a tumor started in the colon or anywhere else in the gastrointestinal tract. Tumors outside the gastrointestinal tract can also express CDX2 when they show intestinal differentiation. A negative stain does not independently exclude an intestinal or colorectal origin, either. Tumor heterogeneity, poor differentiation, and limits in a small specimen can complicate the pattern or the interpretation.
The useful question is therefore not simply “Is CDX2 positive?” It is “What does this result contribute to the interpretation of this tumor, alongside everything else you know?” Mayo Clinic Laboratories describes its CDX2 immunostain as a technical-component test and says interpretation should be made in the context of clinical history and other diagnostic tests by a qualified pathologist.

Questions to ask about the result itself
1. What does CDX2 mean in my report?
Ask the pathologist to describe the result in plain language: is it positive, negative, or focal, and is the staining in the tumor cells? If the report uses terms you do not recognize, ask what they mean in this specimen rather than trying to infer a diagnosis from one word.
“Positive” means the tumor cells show CDX2 immunoreactivity according to the assay’s interpretation criteria. It does not necessarily mean that every tumor cell stains, nor does the word alone tell you how strongly the stain influenced the final interpretation. Ask whether the pattern is strong or weak and diffuse or focal, and what significance that pattern has in this particular case.
2. Does the result support an origin, or establish one?
Ask which possible origins the finding supports and which alternatives remain under consideration. A stain can contribute evidence for a phenotype without identifying a primary site on its own. Ask the pathologist to distinguish what the tissue findings suggest from what they can establish, and to explain how much weight CDX2 carries among the findings.

3. What does a negative result mean here?
Ask whether a negative stain lowers suspicion for an intestinal or colorectal origin in this specific case, and whether any other evidence still supports that possibility. A negative CDX2 result alone does not rule out those origins. Tumors may be heterogeneous, poorly differentiated, or represented by a limited sample, each of which can make an IHC pattern less straightforward.
How the full stain panel changes the interpretation
CDX2 is generally more informative when considered with the tumor’s microscopic appearance and other markers than when read alone. In the European Society for Medical Oncology (ESMO) 2023 guideline, the classic colorectal immunophenotype is CK7-negative, CK20-positive, and CDX2-positive; CK20 and CDX2 are usually diffuse and strong in that pattern. ESMO reports that at least 80% of colorectal cancers show this classic phenotype. That figure describes a pattern observed among colorectal cancers; it is not the probability that an individual tumor began in the colon, and it is not proof of a primary site.
4. How do CDX2, CK7, CK20, and the other stains fit together?
Ask the pathologist to walk through the complete IHC pattern rather than discussing CDX2 in isolation. Which results are most influential? Does the combination fit the tumor’s morphology? Do any markers point in a different direction, and if so, how are those findings reconciled?
The classic colorectal pattern is a useful reference, not a shortcut to a diagnosis. The interpretation depends on the specimen, the appearance of the tumor, clinical history, and the alternatives being considered. The ESMO guideline describes CDX2 as part of an initial IHC panel in selected workups of adenocarcinoma in the liver, alongside CK7, CK20, and TTF1. Other markers may be chosen according to the differential diagnosis; that does not mean every patient needs every stain or that the same panel applies in every situation.

5. Does the staining pattern match the tumor’s appearance?
Ask whether the amount and distribution of staining matter to the interpretation. Is it diffuse or focal? Strong or weak? Does that pattern fit what the pathologist sees under the microscope? The point is not to decide what a particular pattern means yourself; it is to understand why the pathologist gives it the weight they do.
Questions about sample quality and confidence
6. Was there enough suitable tumor tissue?
Ask whether the specimen contained enough appropriate tumor for the stain and whether its size or condition limits confidence. Small or poorly differentiated specimens can make IHC less useful, and a sample may not capture all the variation within a tumor. If the result is focal or does not fit other findings, ask whether sampling could be part of the explanation.
7. Were the stain controls satisfactory?
Ask whether the technical controls for the stain were satisfactory and whether any tissue-handling or technical issue affects how confidently the result can be interpreted. A pathologist can explain whether the laboratory result is technically interpretable and how that differs from the broader question of what the result means diagnostically.

Sample adequacy and stain controls are related but distinct questions: one concerns whether there is suitable tumor to assess, while the other concerns whether the staining procedure worked as expected. Neither question should be used in isolation to infer the primary site.

How to compare possible origins and decide what comes next
8. What findings favor each possibility?
If the team is weighing more than one possible origin, ask how it compares the evidence across several areas: microscopic appearance, the full IHC pattern and its distribution, specimen and control adequacy, fit with clinical history and imaging, and the value of any additional testing. Ask which findings are most persuasive and which remain unresolved.
This is a request for an integrated explanation, not a request to rank possible origins from CDX2 alone. A pathologist can put the marker in context; the oncology team can explain how that interpretation fits the larger clinical picture.
9. Could another test or review change the working diagnosis or treatment plan?
Ask whether additional stains, molecular testing, imaging, or clinical evaluation could clarify the origin—and, importantly, whether the result would change the working diagnosis or management. The appropriate next step depends on the case. These options are discussion prompts, not a recommendation that every person pursue every test.
If the tissue findings and the clinical picture appear to point in different directions, ask whether the pathologist and oncologist have discussed the difference. You can also ask who will explain how the pathology interpretation fits your overall diagnosis and next steps.
Best Value
Make the conversation specific to your report
Bring or refer to the pathology report so the discussion can focus on the actual result and the other stains listed in it. You might open with: “Can you explain what CDX2 contributes to the diagnosis in my case, and what it does not tell us?” Then choose the questions most relevant to the report:
- Is the stain positive, negative, or focal in the tumor cells?
- How do CK7, CK20, CDX2, and the other reported markers fit together?
- Does the staining distribution match the tumor’s microscopic appearance?
- Was the sample and the staining technically adequate for interpretation?
- What alternatives remain, and what evidence supports or argues against each?
- Would further testing change the working diagnosis or treatment plan?
You do not need to ask every question or request every possible test. The useful aim is to understand what the stain supports, what it leaves uncertain, and how the result fits the rest of the evaluation.
A plain-language answer to “What does CDX2 positive mean?”
It means the tumor shows a marker associated with intestinal differentiation. That may support an intestinal-type phenotype, but CDX2 alone does not prove where the tumor began. The pathologist interprets it alongside the tumor’s appearance, other stains, sample quality, and clinical findings.
