Before acting on a report, ask what the system measured, how the result was compared with other people, whether the image analysis was reviewed, and whether the finding changes your care.
What did the AI measure on my MRI?
Automated MRI analysis can estimate several different body compartments. A 2026 study in Radiology used a deep-learning framework to measure subcutaneous adipose tissue (fat beneath the skin), visceral adipose tissue (fat around internal organs), skeletal muscle, skeletal-muscle fat fraction, and intramuscular adipose tissue. Which measures appear on a report depends on the model and the anatomical region scanned; a region-specific measurement is not necessarily a whole-body estimate.

Ask your clinician:
- Which specific tissues or compartments are reported, and which part of my body was analyzed?
- Is each figure an absolute amount, a percentile, or a z-score?
- Was the segmentation technically adequate, and did a radiologist or clinician review the AI output?
These distinctions matter because measures with similar-sounding names are not interchangeable. For example, visceral fat and intermuscular fat describe different locations, while muscle quantity and muscle fat fraction describe different characteristics.

The 2026 Radiology study calculated measures for 66,608 participants from the UK Biobank and German National Cohort, using data collected between April 2014 and May 2022. Its reference curves were adjusted for age, sex, and height. That makes the study useful for describing patterns in those cohorts; it does not establish that a value predicts a particular outcome for an individual patient.

Ask which reference population and adjustments were used for your report, and whether that reference is appropriate for your age, sex, height, health status, and reason for having the MRI. The study authors and the Radiological Society of North America identify validation in clinical populations and disease-specific reference values as next steps. A score outside a reference range therefore needs context rather than an automatic interpretation as disease.
Does an association in a study mean I have a diagnosis?
No. The 2026 study found associations between certain body-composition categories and later outcomes in its participant population. Compared with the middle z-score category, the high visceral adipose tissue category had a hazard ratio of up to 2.26 for incident diabetes; the high intramuscular adipose tissue category had a hazard ratio of 1.54 for major adverse cardiovascular events; and the low skeletal muscle category had a hazard ratio of 1.44 for all-cause mortality. These are adjusted study estimates, not an individual’s absolute risk or a diagnosis.

A separate prospective analysis of 11,348 German National Cohort participants without known pre-existing conditions found that higher paraspinal intermuscular adipose tissue was associated with higher odds of several cardiometabolic risk factors. That association does not show that the measurement causes those conditions. As RSNA quoted senior author Jakob Weiss, MD, PhD, “But BMI does not reliably reflect a person’s actual body composition.” That observation is about BMI’s limitations; it does not make an MRI-derived measure a standalone diagnosis.

How reliable is this particular AI result?
Accuracy figures belong to the model and data on which they were evaluated. For example, a 2020 adipose-tissue segmentation study reported a Dice overlap of 0.94, sensitivity of 96.6%, specificity of 95.1%, precision of 92.1%, and accuracy of 98.4% on its study data. Those figures are not a universal accuracy guarantee for every scanner, scan region, patient, or AI system.
Ask which system analyzed your scan, whether it was validated for this type of scan and clinical use, and whether a clinician checked that the segmentation followed the anatomy correctly. An automated output can be affected by image quality, the scan protocol, the anatomical region, and how the system defines a compartment.

Can I compare this result with another scan or method?
Only if the measurement methods and regions are sufficiently comparable. A 2018 comparison of quantitative MRI and DXA reported correlations of 0.99 for whole-body adipose tissue and 0.97 for lean tissue, but agreement for visceral adipose tissue was lower, with a coefficient of variation greater than 20%. Correlation does not mean two methods give interchangeable values.

Before comparing results, ask whether both analyses used the same metric definition, anatomical region, scan protocol, segmentation method, and reference population. A whole-body estimate should not be treated as directly comparable to a single-slice or region-specific measurement. Also ask whether another analysis would affect a clinical decision; the evidence cited here does not establish that repeating MRI at a particular interval is beneficial.

Questions to take to your appointment
- What exactly did the AI measure, and from which MRI region?
- Is the reported number an absolute value, percentile, or z-score, and what reference population and adjustments were used?
- Was the AI output reviewed by a radiologist or clinician, and was the segmentation technically adequate?
- Is this a diagnosis for me, or a risk marker that needs to be interpreted alongside other information?
- How does the result fit with my history, examination, symptoms, and other tests?
- Is any follow-up needed, and would it change my care?
- If I have another scan, can its result be meaningfully compared with this one?
If you are concerned about a specific result, ask the clinician who can review both the scan and its report. Do not use a population-level association alone to decide on treatment or repeat imaging.
