What to Ask Before Joining a Genetic Research Study

Before joining a genetic research study, ask what will happen to your samples and data, who can use them, whether you can withdraw, and whether any findings will be returned.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Before you sign a consent form for a genetic or genomic study, ask exactly what the study is trying to learn, what will be collected, who can use it, how long it will be kept, whether you can withdraw, and what (if anything) you will learn about your health. A research study is not automatically a diagnostic test or a treatment program. The answers below help you compare the actual protocol and consent form rather than relying on general assurances about confidentiality.

Start with the study’s purpose and sponsor

Ask the study team to explain the project in ordinary language, not only in technical terms. The consent process should describe the research purpose, the science behind it, the goals, and the organization sponsoring or funding it. NIH’s Required Elements of the Consent Form says these details belong in informed consent.

Questions to ask

  • What specific question is this study designed to answer?
  • Who sponsors or funds it, and who is responsible for running it?
  • Why was I invited? Is eligibility based on my diagnosis, ancestry, family history, age, medication, or another factor?
  • Is this research only, or will I receive a clinical test, treatment, or medical advice?
  • What are the realistic benefits, if any, for me personally? Could the main benefit be helping future research rather than improving my own care?

Do not assume that a study offering sequencing will identify a cause of illness or recommend treatment. Ask the team to distinguish research procedures from ordinary clinical care and to explain whether any result will be placed in your medical record.

Find out exactly what participation involves

Consent should describe the samples, information, analyses, visits, contacts, and time commitment expected of you. Ask for a step-by-step schedule and keep a copy of the current consent form.

Samples and procedures

  • Will the study collect blood, saliva, cheek cells, tissue, urine, or another specimen?
  • Will a sample be taken specifically for this study, or will researchers use material left over from clinical care?
  • Does collection require a needle, biopsy, fasting, medication changes, imaging, or other preparation?
  • Will the study perform whole-genome sequencing, exome sequencing, a targeted panel, or another analysis?
  • Could the researchers run additional tests later, and would those tests require a new consent?

Information and records

  • Which health information, family history, lifestyle details, or demographic data will be collected?
  • Will the team access my electronic medical record or request records from other providers?
  • Will information from surveys, wearable devices, registries, or other databases be linked to my study data?
  • How many in-person visits, phone calls, online questionnaires, or follow-ups are expected?
  • How long does participation last, and could the study contact me after the main study ends?

The National Cancer Institute’s Understanding Informed Consent Forms explains that consent should cover procedures, responsibilities, duration, benefits, and other practical details. Ask what you must do between visits and what happens if you miss an appointment.

Ask about physical, emotional, social, and privacy risks

Genetic research can involve more than the discomfort of collecting a sample. A result may raise anxiety, affect family relationships, or create concerns about employment, insurance, stigma, or confidentiality. Even when names and other direct identifiers are removed, genomic information can sometimes be linked back to a person or family because it is inherently identifying.

Questions about collection and follow-up

  • What are the possible short-term and long-term risks of each procedure?
  • What happens if a sample collection causes pain, bruising, bleeding, infection, or another complication?
  • Who should I contact if I feel distressed by a result or by the study process?
  • Could participation reveal information about biological relatives who did not volunteer?
  • Are there risks of discrimination, stigma, or unwanted disclosure, and what protections are in place?

Ask the team to describe safeguards in concrete terms: coding or de-identification, encryption, physical storage, access logs, staff training, and the process for handling a data breach. “Confidential” is not the same as “impossible to identify.” Ask what the researchers can and cannot promise.

Understand who can see your data and samples

A consent form should state how genomic data and samples may be shared. NHGRI’s Special Considerations for Genomics Research discusses the need to explain proposed uses and sharing. For studies covered by NIH genomic-data policy, the NIH Genomic Data Sharing Policy describes expectations for broad data sharing and repository access.

Clarify access and identifiability

  • Will my data be coded, de-identified, or directly identifiable to the research team?
  • Which study staff, laboratories, collaborators, students, contractors, or outside investigators can access it?
  • Will data go to a controlled-access repository requiring an application, or to a repository with broader or unrestricted access?
  • Will samples leave the institution or the country?
  • Can commercial companies, government agencies, or other third parties receive access?
  • Will my name, contact details, or medical-record number travel with the genetic data?

Ask how access decisions are made, whether users must follow a data-use agreement, and whether the study will notify participants about a material change in sharing. The exact answer is protocol-specific; compare the wording of each consent form instead of treating all repositories as equivalent.

Ask how long samples and data will be kept and what future research is allowed

Genetic studies often retain specimens and datasets after the original question is answered. Find out whether the consent covers only the present project or permits broad future research, including research on conditions unrelated to the reason you were invited.

Questions for the consent discussion

  • For how many years, or until what event, will my sample and data be stored?
  • Will the sample be destroyed after the current analysis, stored indefinitely, or used until it is exhausted?
  • May researchers study diseases, traits, ancestry, or other questions not yet specified?
  • Will future researchers need to contact me for new permission, or does this consent authorize future use?
  • Can I choose among options such as storage, outside sharing, or future unspecified research?
  • Will researchers publish aggregate findings, and could a publication contain information that makes a small group recognizable?

Broad consent can be convenient for future studies, but it also means you may not be asked about every later project. If the form uses terms such as “future research,” “broad sharing,” or “open access,” ask the coordinator to define them in practical terms.

Learn the real limits of withdrawing

NIH participant guidance states: “Participants have the right to withdraw from a research study at any time.” That right generally lets you stop future visits, new collections, and further participation. It does not necessarily allow a study to retrieve material that has already been distributed, analyzed, published, or placed in a repository.

“The potential limitations and consequences of withdrawing samples and data from research should be discussed in the consent form and as part of the consent process.” — National Institutes of Health, Required Elements of the Consent Form

Ask for a specific withdrawal procedure

  1. Who must I contact to withdraw, and can I make the request by email, phone, or a signed form?
  2. Will stopping participation halt only future activities, or will the team also destroy unused samples and delete identifiable data?
  3. What happens to coded or de-identified data that the study can no longer link to me?
  4. What happens to data already shared with another laboratory, repository, or collaborator?
  5. Can information already used in analyses, presentations, or publications be removed?
  6. Will the team confirm in writing what it destroyed, retained, or could not retrieve?

Ask whether withdrawal must occur before a particular analysis or data-transfer date. A promise to “remove your information” is meaningful only when the study explains which information remains identifiable and which material has already left its control.

Know whether you will receive genetic results

Never assume that testing means you will receive a personal report. Some studies return individual findings; some return only overall results; some return nothing. Policies can also differ for primary findings discovered while answering the study question and secondary findings discovered incidentally.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Questions about return of results

  • Will I receive individual findings, secondary findings, aggregate results, or no results?
  • Which categories of findings qualify for return, and who decides?
  • When and how would I be contacted?
  • Can I choose which types of results I want to receive or decline?
  • Will a laboratory confirm a research finding with a clinically validated test before it is reported?
  • Will a genetic counselor or another qualified professional explain the result?
  • Will the study help with referrals, follow-up testing, or questions from my clinician?

Research findings may be uncertain, may not meet clinical testing standards, and may be reinterpreted as scientific knowledge changes. A study’s failure to contact you does not prove that you have no genetic risk. Ask whether a returned finding should be confirmed through clinical care before anyone makes a medical decision, and who pays for that confirmation.

Discuss implications for biological relatives

A finding can say something about parents, siblings, children, or other biological relatives who never joined the study. Ask whether the study expects you to share results with relatives, whether the team will contact relatives directly, and what support is available for a difficult family conversation.

Useful questions

  • Could this result indicate a risk that relatives may also have?
  • Will the study provide a plain-language letter or counseling to help me explain it?
  • Can relatives receive testing or counseling through the study, or must they use their own healthcare?
  • What happens if a relative does not want to know?
  • Does the consent address situations in which researchers believe a serious, preventable risk may affect a relative?

There is no universal rule for how families should share genetic information. Ask the study team to explain its policy before you consent to receiving results.

Check costs, payment, and effects on ordinary care

Ask for the financial terms in writing. The study may reimburse travel, pay for time, provide parking, or cover research-only procedures, but arrangements differ by protocol. Payment can also be prorated or dependent on completing particular visits.

  • Will I be paid, reimbursed, or offered neither?
  • Which travel, childcare, parking, lodging, testing, or follow-up costs are covered?
  • When and how will payment be made, and could it create tax-reporting obligations?
  • Will my health insurance be billed for anything?
  • If an injury occurs during a research procedure, who pays for treatment?
  • Can I decline without losing ordinary medical care, benefits, or my relationship with a clinician?

Consent guidance calls for explaining participant responsibilities, benefits, and relevant costs. A coordinator should be able to identify which services are research-only and which are part of routine care.

Compare two studies using the same checklist

If you have more than one invitation, compare the actual consent forms rather than the study titles or a general promise of privacy. Put answers in a table so differences that sound minor become visible.

Comparison area What to record
Samples and procedures Specimen type, invasiveness, number of collections, visits, preparation, and follow-up contacts
Data identity and access Coded or identifiable status, outside laboratories, repository type, access review, and possible commercial or government access
Future use Current project only or broad future research; new consent required or not; storage period
Withdrawal How to request it, what can be destroyed, and what may already have been shared or analyzed
Results Individual or secondary findings, optional receipt, clinical confirmation, counseling, and follow-up
Time and money Study length, visit schedule, travel burden, payment, reimbursement, and injury coverage

“More privacy” or “more benefits” is not a useful comparison unless the team defines the terms. Record the exact language and ask both studies the same follow-up questions.

Use a final consent checklist before signing

Take time to read the entire form, including appendices, data-sharing sections, optional sub-studies, and withdrawal instructions. You can ask questions, take the form home when allowed, and discuss it with a trusted person or your own clinician. A study team should give you a contact for questions about participation and a separate contact for reporting research-related harm or concerns.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  • I can state the study’s purpose and sponsor in my own words.
  • I know every sample, record, survey, device, visit, and analysis involved.
  • I understand physical, emotional, family, social, and privacy risks.
  • I know who can access the data, where it may be stored, and whether sharing is controlled.
  • I understand storage duration and future-use permissions.
  • I know exactly how withdrawal works and what cannot be retrieved afterward.
  • I know whether any result will be returned, whether it will be clinically confirmed, and what support follows.
  • I understand costs, payment, injury coverage, and that declining will not affect ordinary care.
  • I have the current consent form and a phone number or email for the study team.

These questions are based on U.S. NIH and NHGRI consent guidance and are general information, not legal or clinical advice. Rules and practices can differ by country, institution, study type, and repository. Ask the study team for protocol-specific answers before deciding.
