What Tests Check for Narrow or Clogged Coronary Arteries?

CCTA and invasive coronary angiography show coronary anatomy, while stress, perfusion, ECG, echo and blood tests assess blood-flow effects or heart injury. Here is what each test can—and cannot—tell you.

The most direct tests for narrowed or “clogged” coronary arteries are coronary CT angiography (CCTA) and invasive coronary angiography. Both can show the artery’s anatomy. Stress tests answer a different question: whether exercise or medication causes signs that the heart muscle is not receiving enough blood. Doctors choose among them according to your symptoms, emergency risk, medical history and the specific uncertainty they need to resolve.

This article concerns the arteries supplying the heart (the coronary arteries). Tests for blockages in the neck, legs, kidneys or other vessels use different imaging and are not interchangeable with a coronary work-up.

Which tests show a coronary artery narrowing directly?

Tests that display the coronary artery itself are anatomical tests. They can show plaque, the location of a narrowing and, in many cases, how severe it appears. The two principal options are CCTA and catheter-based coronary angiography.

Coronary CT angiography (CCTA)

CCTA combines computed-tomography X-ray images with an injected contrast agent. The resulting scan can show the coronary arteries, plaque and areas of narrowing without placing a catheter inside the heart. It is noninvasive in the sense that it uses an intravenous injection rather than an arterial catheter, but it is still a medical procedure with preparation and risks.

CCTA may be useful when symptoms suggest coronary disease and earlier tests have not supplied enough information, or when a clinician needs a detailed, noninvasive view of coronary anatomy. It does not replace invasive angiography in every situation. Image quality can be affected by factors such as heart-rate control, heavy calcification or irregular rhythms, so the ordering team decides whether it is suitable.

Because CCTA uses X-rays, it involves radiation exposure. The contrast agent can require special consideration in people with kidney problems or a previous serious contrast reaction. Tell the clinical team if you could be pregnant, have kidney disease, or have ever reacted to contrast. The team may review medications, check kidney function or choose another test.

Invasive coronary angiography (cardiac catheterization)

During an invasive coronary angiogram, a clinician places a thin catheter into an artery, commonly at the wrist or groin, and guides it toward the heart. Contrast is injected while X-ray images are taken. The images show where a coronary artery is narrowed and how extensive the narrowing appears.

Unlike CCTA, this procedure can move directly from diagnosis to treatment when that is appropriate. The team may perform balloon angioplasty or place a stent during the same catheterization. A visible narrowing does not automatically mean a stent is needed; treatment depends on symptoms, the artery involved, blood-flow significance, other test results and the person’s overall risk.

Catheterization is performed in a hospital catheterization laboratory. It is invasive, so the team discusses bleeding, blood-vessel injury, contrast reactions, kidney effects and other procedure-specific risks before proceeding. The route, sedation, observation period and discharge instructions vary by hospital and by whether treatment is performed.

Test What it answers How it is performed Radiation, contrast or tracer Can treatment follow immediately?
CCTA What do the coronary arteries and plaque look like? CT scan with an intravenous contrast injection X-rays and usually contrast No; a separate catheter procedure is generally required
Invasive coronary angiography Where is the narrowing and how does the artery look on real-time X-ray? Catheter from an artery in the wrist or groin X-rays and contrast Yes, angioplasty or stenting may be performed when indicated

What tests look for the effect of reduced blood flow?

Functional tests do not usually create a direct picture of the artery’s inner channel. Instead, they observe the heart during exertion or medication-induced stress and look for evidence that blood supply cannot keep up with demand.

Exercise stress test

For a standard exercise stress test, you walk on a treadmill or pedal a stationary bicycle while clinicians monitor your heart rhythm, blood pressure and symptoms. The workload increases in stages. The team watches for chest discomfort, shortness of breath, rhythm changes or other patterns that suggest the heart is under strain.

If you cannot exercise adequately, medication can mimic some effects of exertion. An abnormal or inconclusive result may lead to an imaging stress test, CCTA, angiography or another evaluation. A normal result lowers the likelihood of some forms of flow-limiting disease but does not rule out every cause of chest pain.

Nuclear myocardial perfusion imaging: SPECT or PET

These tests use a small amount of radioactive tracer and a specialized camera to compare blood flow to the heart muscle at rest and during stress. Areas that receive less blood during stress than at rest can indicate reduced perfusion. A fixed defect may reflect prior damage, such as scar from an earlier heart attack.

SPECT and PET are both perfusion studies, but availability, image quality, radiation dose and clinical use differ by center and patient. The ordering clinician selects the method and stress protocol. The tracer is not a substitute for an anatomical angiogram: perfusion imaging shows the effect on heart muscle rather than mapping every plaque deposit.

Stress echocardiography

An echocardiogram uses sound waves to show heart structure, pumping function and wall motion. Images taken during stress can reveal a region that contracts less when the heart needs more oxygen, which may indicate inadequate blood supply. Echocardiography does not directly display the coronary artery lumen, and a wall-motion abnormality can have causes other than a current blockage.

Stress cardiac MRI and other advanced evaluation

Cardiac magnetic-resonance stress imaging can assess heart structure, function and perfusion without ionizing radiation. It is available only in appropriately equipped centers and may not be suitable for everyone with implanted devices or other MRI limitations. In people whose symptoms persist despite non-obstructive findings, clinicians may consider PET, stress cardiac MRI or invasive coronary physiology to investigate problems such as microvascular dysfunction or coronary spasm.

What supporting tests are used?

Electrocardiogram (ECG or EKG)

An ECG records the heart’s electrical activity through skin electrodes. It can identify patterns associated with an acute heart attack, prior injury, rhythm disorders or other cardiac problems. It does not photograph a narrowed coronary artery. A normal resting ECG therefore cannot by itself exclude coronary disease.

Echocardiogram at rest

A resting echocardiogram evaluates chamber size, valves, pumping strength and wall motion. A weakly moving region can point to prior damage or inadequate oxygen delivery, but the study is not a direct image of coronary narrowing.

Blood tests, including troponin

When an acute heart attack is suspected, serial blood tests for troponin and other markers help detect injury to heart muscle. They do not show the physical blockage. Emergency teams interpret them alongside symptoms, repeated ECGs and appropriate imaging or catheterization.

How doctors decide which test to order

There is no single universal sequence. The choice depends on the clinical question, the urgency and whether the result would change treatment.

  • Possible emergency: Severe or ongoing chest pressure, shortness of breath, sweating, fainting, or pain spreading to the arm, jaw or back requires emergency assessment. ECG and troponin testing are often early steps; urgent angiography may be needed when findings indicate an acute coronary syndrome.
  • Stable symptoms with a need to see anatomy: CCTA may provide a noninvasive view of plaque and narrowing when the patient is an appropriate candidate.
  • Stable symptoms with a need to measure blood-flow limitation: Exercise testing, stress echocardiography or nuclear perfusion imaging can show whether exertion produces evidence of inadequate supply.
  • High concern or a likely need for treatment: Invasive angiography may be chosen because it gives detailed images and can allow angioplasty or stenting during the same session.
  • Inconclusive or conflicting results: The clinician may select another modality rather than treating one test in isolation.

Age, kidney function, ability to exercise, heart rhythm, prior procedures, contrast reactions, pregnancy possibility and local expertise all affect the decision. A clinician also weighs the probability of disease before testing; the same result can mean something different in a person with low versus high pretest risk.

What to expect before and during each test

Before CCTA

Tell the imaging team about pregnancy, kidney disease, allergies or previous reactions to contrast and all medicines you take. The center may ask you to avoid food or caffeine for a specified period, check your heart rate and give medicine to slow the rate so the images are clearer. Follow that center’s written instructions rather than assuming every facility uses the same preparation.

During a stress test

Wear clothing and shoes suitable for exercise unless the center gives different instructions. Electrodes and a blood-pressure cuff remain in place while you exercise or receive a stress medicine. Report chest pressure, unusual breathlessness, dizziness or pain immediately; the staff can stop the test and treat symptoms. Medication instructions differ, so ask which drugs to take or hold.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Before and during invasive angiography

The team reviews medicines, allergies, kidney function and bleeding risk, places an intravenous line and explains sedation and access-site care. After local anesthetic, the catheter is advanced through the wrist or groin. You may feel a brief warm flush when contrast is injected. If a significant lesion is found and treatment is appropriate, angioplasty or stenting may follow. Afterward, pressure or a closure device is used at the access site, and you are monitored for bleeding and other complications.

What a “normal” or “blocked” result does—and does not—mean

A test result must be interpreted with symptoms and the rest of the evaluation. CCTA or angiography can show plaque that is present but not severely obstructive. Conversely, a stress test can indicate impaired blood flow without identifying one discrete blockage. Doctors may recommend medication, lifestyle measures, follow-up or additional testing rather than an intervention.

Angina can occur even when angiography or CCTA does not show a major obstructive lesion. An American Heart Association Professional Heart Daily summary updated November 10, 2025, notes that up to 50% of patients with angina in the discussed population have non-obstructive coronary artery disease on angiography or CCTA. That figure applies to the population and context described by the summary, not to every person with chest pain. Microvascular dysfunction and vasospasm are examples of problems that may require specialized testing and management.

When to seek urgent help

Call emergency services for new, severe or persistent chest pressure; pain that spreads to the arm, shoulder, back, neck or jaw; shortness of breath; fainting; cold sweating; sudden weakness; or symptoms that feel like a heart attack. Do not drive yourself or wait for an outpatient stress test. Emergency clinicians can perform an ECG and blood tests promptly and decide whether urgent imaging or catheterization is necessary.

Questions to ask the clinician

  • Is the goal to see the artery, measure blood flow, assess heart damage, or investigate another cause?
  • Why is this test preferable to CCTA, a stress study or invasive angiography in my situation?
  • Will I receive contrast, a radioactive tracer, medication to increase or slow my heart rate, or radiation?
  • Do my kidney function, pregnancy possibility, allergies, implanted devices or medicines change the plan?
  • If the result is abnormal or unclear, what test or treatment would come next?
