When an AI model produces unsafe, unexplained or out-of-scope results, a bank should treat the event as a model-risk and operational-resilience incident. The immediate sequence is to declare the incident, contain further decisions, preserve evidence, measure who and what was affected, correct or replace the failing system, remedy affected outcomes, and reopen only with documented approval and heightened monitoring.
The response must fit the bank’s existing incident-management, business-continuity, cybersecurity, vendor-risk and disclosure processes. A U.S. supervisory guide, a voluntary framework and a UK prudential rule do not create one universal playbook or notification deadline; authority depends on the model, the harm, the bank’s jurisdiction and the facts.

Use this eight-stage response sequence
| Stage | Immediate objective | Accountable participants |
|---|---|---|
| 1. Detect and declare | Record the suspected failure, affected version and initial severity. | Incident lead, business owner, model-risk function |
| 2. Contain | Stop or constrain unsafe decisions and activate a safe fallback. | Business, operations, technology and authorized decision-maker |
| 3. Scope impact | Identify decisions, people, dates, systems and possible harms. | Model risk, data, compliance, legal, privacy and operations |
| 4. Mobilize | Establish cross-functional command and explicit decision rights. | Incident lead with the relevant control functions |
| 5. Investigate and treat | Find contributing causes and choose mitigation, replacement or shutdown. | Independent validation, model owner, technology and security |
| 6. Remedy and communicate | Review, reverse or correct affected outcomes and meet notice duties. | Legal, compliance, customer teams and communications |
| 7. Recover | Validate the fix or fallback and return to service under controls. | Business, risk, compliance and technology approvers |
| 8. Learn | Update monitoring, inventory, controls, training and continuity plans. | Model governance and operational-resilience owners |
1. Detect and declare the incident
Define what counts as a suspected failure
The bank’s policy should cover more than a crashed application. A reportable suspicion can include material performance drift, inconsistent or harmful decisions, contaminated training or production data, unauthorized use, a security attack, a vendor outage, or loss of a required safeguard. These categories are practical policy choices rather than a regulator-mandated taxonomy, so the bank should set thresholds that match the model’s materiality and use.
At detection, record the time, reporter, model and version, application, deployment location, affected period and the business process that relies on it. Assign an incident lead and a provisional severity while facts are still incomplete. Do not wait for a proven root cause before escalating: a credible risk of continuing harm is enough to start containment.

Preserve evidence immediately
Place relevant records under the bank’s normal retention and legal-hold rules. Preserve model files and versions, input and output logs, monitoring alerts, change tickets, configuration, access records, vendor notices and downstream decision records. Capture the state of automated workflows so investigators can establish whether decisions continued after detection. Keep an evidence log that distinguishes original records from later exports or analyses.
2. Contain unsafe outcomes
Compare actual behavior with the approved use
Ask whether outputs remain within the model’s intended purpose, approval conditions, monitoring limits and risk tolerance. Check whether an automated process is still making, communicating or executing decisions while the investigation runs. The authorized owner should decide whether to limit functionality, insert human review, route cases to a fallback, roll back to a validated version, or disable the model.
Choose an intervention proportionate to the danger
| Intervention | Appropriate when | Required controls |
|---|---|---|
| Limit functionality | The unsafe behavior is confined to a feature, segment or transaction type. | Block the affected path, test unaffected paths and monitor for spillover. |
| Add human review | Qualified staff can detect or correct errors before a decision takes effect. | Define reviewer authority, workload limits, documentation and escalation. |
| Use a validated fallback | A prior model or manual process can perform the essential task safely. | Confirm capacity, access, decision standards and reconciliation. |
| Roll back | A recent model, data, code or configuration change is a credible contributor. | Use a known validated version, test dependencies and preserve rollback evidence. |
| Disable or decommission | Harm is severe or fast-growing, diagnosis is uncertain, or no safe control exists. | Use the assigned shutdown authority, notify affected owners and activate continuity arrangements. |
Before choosing, weigh severity and reversibility of harm, model materiality, the rate at which risk is growing, confidence in the diagnosis, availability of a safe alternative, service and fairness effects, third-party dependencies, and legal or contractual notice duties. Do not continue operation merely because the cause is unknown. A viable non-AI process should be designed and tested before an emergency where practical.

3. Scope the impact, not just the technical defect
Build an affected-population view
Identify the products, business lines, decision types, customers or counterparties, geographies, time range, third-party components and downstream systems involved. Join model logs to decision and account records to establish which outputs influenced approvals, pricing, limits, fraud actions, collections, customer service or other outcomes. Record the earliest plausible exposure date, not only the date the alert fired.
Estimate the error rate and potential severity with validated comparisons, sample review and affected-population analysis. Separate confirmed harm from plausible but unconfirmed harm. Document assumptions, missing data, confidence ranges and unknowns so that a later estimate does not overwrite the uncertainty that existed at the decision point.

Assess every relevant harm dimension
- Financial and credit: losses, incorrect approvals or declines, pricing errors, capital effects and customer compensation.
- Conduct and consumer protection: disparate outcomes, fair-lending concerns, misleading communications, collections treatment and appeal rights.
- Financial crime: missed or excessive fraud, sanctions or anti-money-laundering alerts and disrupted investigations.
- Privacy and security: unauthorized disclosure, sensitive-data use, prompt or access-control abuse and adversarial manipulation.
- Operational and legal: service interruption, contractual breach, recordkeeping failure, litigation exposure and reputational damage.
Not every model implicates every category. The incident lead should state which dimensions were assessed, which were excluded and why.
4. Mobilize the right owners
Name one incident lead with authority to convene the response and make or escalate containment decisions. Bring in the business owner, model developer or owner, independent model validation or model-risk management, operations, data and technology, cybersecurity, vendor management, compliance, legal, privacy, customer support and communications as the facts require.
Use the bank’s established incident-command and escalation paths, but write down responsibilities and decision rights for this event. The Federal Reserve, OCC and FDIC guidance emphasizes governance and clear accountability; NIST’s AI Risk Management Framework likewise expects assigned responsibilities and response plans. For a vendor model, request the provider’s incident timeline, affected versions, known customer impact and proposed fix, then test the bank’s local behavior. Outsourcing does not transfer the bank’s responsibility to understand and monitor the product.
5. Investigate the cause and select treatment
Test the full chain
Examine input data and pipelines, upstream system changes, model or feature versions, configuration, data quality, drift, implementation code, user behavior, access controls and vendor dependencies. Determine whether the event is deterioration, an implementation defect, misuse, adversarial activity, novel behavior or an external event; several causes can coexist.
Reproduce the failure with controlled data where possible, compare outputs with the approved baseline and test whether the behavior affects particular products or groups. Independent validation and outcome analysis should be proportionate to the model’s risk and use before the bank relies on a correction, override or overlay.

Choose a documented risk treatment
For each credible cause, record whether the bank will mitigate, avoid, transfer or accept residual risk. Mitigation might include data repair, a code change, a feature removal, tighter thresholds, a human override or additional monitoring. Avoidance can mean retiring the use case. Transfer, such as contractual allocation to a vendor, does not remove the need for bank oversight. Acceptance requires an explicit decision, rationale, owner, time limit and monitoring plan; it is not the default while facts are missing.

Decide what affected people are owed
Map the affected population to the decision and applicable law or policy. Cases may require human review, reprocessing, reversal, correction of records, restoration of service, an appeal channel, compensation or another remedy. Prioritize irreversible or high-impact outcomes and people who could not reasonably detect the error themselves.
Route notices through the responsible teams
Legal and compliance should determine whether the facts trigger regulatory reporting, consumer notices, privacy or cybersecurity notifications, contractual notices, or public statements. Deadlines vary by incident type and jurisdiction; there is no universal AI-incident clock. Communications should state what happened, the period and decisions affected when known, available remedies, how to seek human review and how uncertainty will be handled. Give customers and other affected users a channel for feedback, appeal and override where appropriate.
7. Return to service under controlled approval
Set acceptance criteria before reopening
Define objective tests tied to the failure mode and intended use. They may include corrected-data checks, performance and stability thresholds, fairness or conduct analysis where relevant, security testing, reconciliation to downstream systems, successful fallback drills and confirmation that logging and access controls work. Record residual risk and any time-limited restrictions.
Obtain the approvals required by the bank’s governance: business ownership, model risk or independent validation, compliance, legal where applicable, technology and the designated incident authority. A fix that passes a technical test but lacks an accountable risk decision is not ready for production.

Stage the release and prepare rollback
Re-enable in stages when the use case permits. Start with a limited population or transaction volume, compare outcomes with the validated baseline and increase monitoring frequency. Set rollback triggers in advance, assign who can invoke them and confirm that the fallback has capacity. Continue tracking appeals, overrides, incidents and user feedback after reopening; return to the prior safe state if thresholds are breached.

Update monitoring metrics and thresholds, validation plans, the model inventory, vendor oversight, response playbooks, staff training and business-continuity arrangements. Test the revised controls rather than merely filing the report.
NIST’s AI RMF Playbook lists post-deployment monitoring elements such as user input, appeals and overrides, decommissioning, incident response, recovery and change management. Its guidance is voluntary and expressly says it is neither a checklist nor a set of steps to be followed in its entirety; a bank should adapt those elements to its own governance.
What the main authorities require—and do not require
United States: revised interagency model-risk guidance
On April 17, 2026, the Federal Reserve Board, OCC and FDIC issued revised interagency model-risk guidance covering model development and use, validation and monitoring, governance and controls, and vendor or third-party products. It is supervisory guidance tailored to a bank’s size, complexity and model-risk profile, not a prescriptive enforceable checklist. The agencies state that noncompliance with the guidance alone will not result in supervisory criticism. It is expected to be most relevant to organizations above $30 billion in total assets, while smaller institutions with significant model-risk exposure may also find it useful. The revised document superseded earlier interagency guidance, so the former SR 11-7 materials should not be presented as the current version.

The 2026 guidance says generative and agentic AI are outside its scope because they are novel and rapidly evolving. It nevertheless says the bank’s risk-management and governance practices should guide controls for systems not covered. The agencies announced plans for a future request for information on banks’ use of AI; its status can change independently of this checklist.

NIST: voluntary design principles
NIST AI RMF 1.0 is voluntary, non-sector-specific and not a banking regulation. Its Manage outcomes call for planned, documented responses to prioritized risks, procedures to respond to and recover from previously unknown risks, and assigned mechanisms to supersede, disengage or deactivate systems whose performance or outcomes are inconsistent with intended use. It also calls for considering viable non-AI alternatives. One outcome states: Procedures are followed to respond to and recover from a previously unknown risk when it is identified.
United Kingdom: PRA SS1/23
The PRA’s SS1/23 sets principles for model identification and classification, governance, development, implementation and use, independent validation and model-risk mitigants. The current version was published and became effective on April 23, 2026. Its scope is UK-incorporated banks, building societies and PRA-designated investment firms using internal models for specified regulatory-capital approaches. It does not automatically apply to every bank or to institutions outside that scope.
Basel operational resilience
Basel Committee operational-risk principles connect governance, information and communications technology, business continuity and disclosure with the wider risk-management and operational-resilience framework. They support integrating an AI failure into existing ICT incident management and timely information transmission. They do not establish one universal AI-incident notification deadline.

When should a bank turn off an AI model?
Turn it off, or move immediately to a controlled fallback, when continuing operation could create severe or rapidly growing harm; outputs are materially outside intended use; the bank cannot establish effective human or technical controls; the model or vendor cannot provide trustworthy records; or a security event compromises integrity or confidentiality. A less drastic intervention is defensible only when the affected path is bounded, a safe control is operating, and the responsible authority has documented why residual risk is acceptable. The decision should also account for service availability, fairness across affected groups, contractual duties and the reversibility of the resulting harm.
