There is no single automatic outcome when a crewed spacecraft has a problem on the way to the International Space Station (ISS). The response depends on when the problem occurs, what systems still work, and which safe options remain. During launch or ascent, a serious emergency may trigger a launch abort that separates the crew capsule from its rocket. During rendezvous, the spacecraft may instead stop its approach or move into a safe, collision-free orbit while the crew and ground teams assess the problem. Depending on the circumstances, the crew might continue to the station, return to Earth, or—if already docked—stay aboard the ISS while another ride home is arranged.
What does a spacecraft problem mean for the crew?
A reported anomaly does not automatically mean that a spacecraft will abort, turn around, or make an emergency landing. “Problem” can describe anything from an issue that teams can monitor while continuing the flight to a failure that makes the planned approach or return unsafe. The response is based on the spacecraft’s actual condition and its remaining ability to separate, maneuver, navigate, communicate, hold a safe orbit, and return.

NASA defines a contingency as “any unusual or emergency situation during the spacecraft’s launch, ascent, return, or splashdown that requires NASA and its partners to take immediate action to protect the crew and safely recover them.” The definition spans several phases because the safe response changes as the vehicle moves from a rocket-powered ascent to an orbital approach, station stay, or return to Earth. NASA’s overview of crew launch and return operations describes those contingencies and recovery planning.

For a spacecraft headed to the ISS, the broad sequence is to detect and assess the anomaly, protect the crew from immediate hazards, and compare the options the vehicle can still safely perform. Those may include continuing the flight, moving away from a hazardous approach, entering a safe state to allow troubleshooting, or returning to Earth. The sequence describes the decision logic, not a universal checklist: actual actions depend on the spacecraft and the particular failure.

What happens if trouble starts during launch or ascent?
Launch and ascent are different from an approach to the ISS. The spacecraft is still attached to, or traveling with, its launch vehicle, so a serious problem may call for an abort that carries the crew capsule away from the rocket. The aim is to separate the crew from a vehicle or flight condition that threatens them and then recover the capsule. It is not the same maneuver as backing away from the station in orbit.
Dragon and Starliner use different launch-abort systems
NASA describes Crew Dragon’s integrated launch-escape system as using eight SuperDraco thrusters to carry the crew away from danger during launch or ascent. NASA’s Commercial Crew reference describes Starliner differently: it has four dedicated launch-abort engines. These are vehicle-specific systems; the numbers and descriptions do not mean that the two spacecraft have identical designs or identical operating envelopes. NASA’s Commercial Crew Program press kit provides program information on the vehicles.
A launch abort is not a general-purpose escape mechanism for every later problem. Once a spacecraft is in orbit and approaching the ISS, the decision is instead about whether it can safely continue, maneuver away, remain in a safe orbit, or return. The launch-abort system should not be confused with those on-orbit options.

Escaping the rocket is only part of an ascent contingency. NASA says more than 50 locations along the flight path are monitored to help ensure downrange conditions are clear in case of an abort. NASA also describes monitoring and prepositioned rescue resources as part of the response planning. The capsule’s recovery route and the availability of rescue teams matter because a successful separation still has to lead to a safe recovery.

The point is not that every ascent anomaly triggers the same maneuver. Teams must account for the phase of flight and the vehicle’s condition, including whether an abort is appropriate and what recovery conditions are available. A problem that does not threaten the crew or prevent safe continued flight may be handled differently from one that requires immediate separation.
What if the spacecraft has trouble approaching or docking with the ISS?
An approach problem is handled in orbit, not with the launch escape system. If the spacecraft’s position, motion, or control state makes continuing toward the station unsafe, a priority can be to create separation and avoid a collision. NASA’s technical analysis describes moving a vehicle into a nearby collision-free orbit and placing it in a safe-haven mode, when the spacecraft can do so. That gives the crew and ground teams time to diagnose and troubleshoot rather than pressing ahead with a questionable docking attempt.
A safe haven buys time; it does not fix the spacecraft
NASA’s technical paper explains that “Safe haven modes independently and reliably place the spacecraft in a safe state to allow the crew (and ground if a communications link is open) a reasonable amount time to diagnose and troubleshoot in-flight problems.” A safe state is a way to manage risk while assessing the situation; it is not a guarantee that the vehicle can later dock or return.
The exact safe-haven behavior depends on the spacecraft’s guidance, navigation, and control design and performance. A maneuver that is available on one vehicle cannot be assumed to exist in the same form on another. NASA’s paper, “Aborts during launch and ascent…”, discusses general abort and safe-haven logic. It does not establish one sequence that applies to every spacecraft or anomaly.

Ground teams and the crew evaluate what remains possible
Once a vehicle is away from an immediate rendezvous hazard, teams can evaluate the failure and the available choices. The questions include whether the spacecraft can maintain a safe orbit, whether its navigation and control systems are reliable enough for another approach, and whether it retains the capabilities needed for a safe return. Communications and propulsion matter too, but a fault in one component does not by itself dictate a universal outcome; what matters is the effect on the spacecraft’s overall safe capability.
If the issue can be understood and the vehicle remains capable, continuing the approach may be an option. If docking is not safe or possible, a return may be chosen instead. A nearby safe orbit may allow time to reach that decision, but the spacecraft must still have the means to remain there and carry out the eventual plan. NASA’s technical paper stresses that abort strategies are shaped by each spacecraft’s guidance, navigation, and control architecture and performance.
How do teams decide whether the crew should return to Earth?
The return decision is a comparison of safe, feasible options—not a rule that every failed docking attempt requires an immediate landing. Teams consider what failed, what still works, where the spacecraft is, and whether the vehicle can perform the maneuvers required for return. The options may change as the spacecraft’s condition becomes clearer.
- Continue the mission: This may be considered if the anomaly is understood and the vehicle remains capable of a safe approach and docking.
- Hold in a safe orbit: If the spacecraft can reach and maintain a collision-free safe state, teams may use that time to diagnose the problem and make a better-informed decision.
- Return the crew: If the vehicle can no longer safely approach, or return is the better option, teams assess whether it can conduct a safe descent and recovery.
- Use another transportation option: If the crew reaches the station but the vehicle is later judged unsuitable for carrying them home, a different spacecraft may be used when arrangements and mission constraints allow.
These are possible outcomes, not a fixed ladder that every crew follows. A second vehicle is not necessarily available immediately: a change of ride depends on factors such as available seats, training, mission arrangements, and the specific emergency. NASA’s Office of Inspector General has also cautioned that NASA does not keep ready-to-launch emergency evacuation vehicles; cost and certification constraints are among the reasons.

Choosing where a return vehicle will land
Returning safely involves more than deciding to leave orbit. For a Dragon that cannot dock or malfunctions on the way to the station, NASA and SpaceX select a return location based on factors including the spacecraft’s orbital position, weather, and proximity to rescue forces. Those conditions affect where a recovery can be carried out safely and practically. NASA’s crew launch and return operations reference describes this planning.
Recovery planning is tied to the vehicle and the situation. A site that is suitable at one point in the flight may not be the best option later, and teams must account for both the return path and the ability to recover the crew. NASA describes downrange monitoring and prepositioned rescue resources for ascent contingencies as well; these are part of preparation, not evidence that every spacecraft follows an identical route.

The 2024 Starliner Crew Flight Test is a useful example of an inbound rendezvous problem followed by a deliberate decision about return. According to NASA’s Aerospace Safety Advisory Panel, Starliner lost five thrusters during rendezvous. Troubleshooting recovered four, enabling the spacecraft to dock with the ISS. NASA then used the station as a safe haven while teams tested and analyzed the spacecraft’s return options.

NASA chose to return Starliner without the crew. The astronauts remained aboard the ISS during the extended stay and returned safely on Dragon in February 2025, according to the panel’s 2025 annual report. The important point is the sequence: an issue during approach did not force an immediate return; the spacecraft was able to dock after troubleshooting, and the ISS provided a place for the crew to remain while NASA assessed whether Starliner should carry them home.

This case does not establish a standard outcome for future thruster problems. The available responses depend on the affected vehicle, the nature of the anomaly, and what the systems can safely do. Nor does the example support a definitive technical root cause: NASA’s Commercial Crew press-kit account described root-cause work as continuing. For the incident and crew-return account, see the Aerospace Safety Advisory Panel’s 2025 Annual Report.
What if the spacecraft becomes unsafe after docking?
A vehicle already attached to the ISS presents a different contingency from one that cannot complete its inbound approach. The station may provide a safe place for the crew to stay while teams assess the compromised vehicle and arrange another return option, but it cannot solve every spacecraft problem or guarantee an immediate evacuation.
The Soyuz MS-22 coolant leak changed the return plan
NASA’s Office of Inspector General reported that Soyuz MS-22 suffered a coolant leak in December 2022 and was considered potentially unsafe for returning its crew. The partners arranged a replacement Soyuz. The affected vehicle returned uncrewed in March 2023, while the crew returned in September 2023 after a mission extended from about six months to more than a year. This illustrates contingency planning after a docked vehicle was compromised; it was not an inbound rendezvous failure.
The ISS is not backed by a ready-to-launch rescue capsule
The OIG report also describes limits on evacuation options. NASA did not have ready-to-launch emergency vehicles available to send to the ISS. The report discusses a proposed extra Dragon seat that could accommodate one additional crewmember under the circumstances analyzed; that should not be interpreted as a plan to evacuate the entire station crew. A different vehicle can be part of a solution, but availability, seats, certification, training, and mission arrangements constrain what can be done. The incident and evacuation discussion appear in NASA OIG’s IG-24-020 report.
What is the current U.S. commercial crew context?
Program status changes, so any certification or flight-count statement needs a date. In an update dated June 30, 2026, NASA’s Office of Inspector General said Crew Dragon had been certified in 2020 and had flown 12 crewed missions to and from the station. The same update said Starliner remained uncertified after three flight tests uncovered unexpected issues. This is a dated status statement, not a prediction about later certification or flight schedules. See NASA OIG’s page on NASA’s Management of Its Commercial Crew Program.
Those program facts provide context for the examples, but they do not change the central decision logic. Launch escape, an on-orbit safe haven, and return in another vehicle are distinct options with different prerequisites. Which one is available depends on the spacecraft and the problem at hand.
What should a reader take away?
When a crewed spacecraft has a problem on the way to the ISS, mission teams first have to understand the phase of flight and the vehicle’s remaining safe capabilities. A dangerous ascent can call for a launch abort and recovery. An unsafe rendezvous can call for backing away into a collision-free orbit and troubleshooting. If the spacecraft reaches the station but cannot safely bring its crew home, the ISS may serve as a temporary safe haven while a different return plan is made. Each is a possible response, not an automatic result of every anomaly.
