What’s actually slowing this PC down?

Include both: a brief, decision-focused reason in the request and a link to the specific trace or supporting record. The reason tells the approver what action is proposed and why; the link gives them a way to inspect the evidence without burying the request in raw logs.
What should an approval request contain?
Make the request understandable on its own, then provide a path to the fuller context. A practical format is:
As an Amazon Associate I earn from qualifying purchases.
- Action: What you want approved.
- Reason: One concise sentence explaining why the action is needed.
- Context: A link to the relevant trace, record, or evidence.
- Decision needed by: Add a date only when the workflow has a real deadline.
The deadline is workflow-dependent, not a universal approval requirement. Keep the action and reason in the request; use the link for detail that would make the request difficult to scan.

| Approach | What it does well | Where it falls short |
|---|---|---|
| Short reason only | Gives the reviewer an immediate explanation. | May not provide enough evidence to assess or later identify the decision. |
| Link only | Can point to extensive supporting detail. | Leaves the reviewer to discover what is being requested and why. |
| Reason plus evidence link | Combines a clear decision prompt with access to the supporting record. | Works only if the link opens for the intended reviewer and leads to the relevant material. |
Salesforce’s guidance on approval-page fields recommends limiting the page to information needed for the decision, within the context of its own approval configuration: Salesforce: Design the Approval Request Page for Classic Approval Processes. That supports keeping the request focused, not omitting necessary evidence.

What makes the supporting link useful?
Link to a specific record that can be tied back to the request, rather than a broad folder or an undifferentiated log. The reviewer should be able to identify the action, relevant evidence, and decision context without searching through unrelated material.
AWS guidance for human review in agentic AI workflows says reviewers need enough context to decide without wading through raw logs. It recommends making full decision context available through the review interface and storing it durably: AWS Well-Architected Agentic AI Lens: Human-in-the-loop for critical decisions. This advice is most directly aimed at those AI workflows, but the practical distinction is useful elsewhere too: summarize the key factors in the request and link to the fuller record.

Access matters as much as completeness. Confirm that the intended approver can open the link. AWS describes authenticated access through the review interface and temporary signed links for cases where reviewers cannot access that interface; those are implementation approaches for the documented scenario, not universal requirements.

How should the approval remain traceable afterward?
The evidence should show what was requested and what decision followed. University of Mary Washington Internal Audit guidance emphasizes connecting documentation to the activity under review. It notes that a message saying only “approved” may fail to identify what was approved; the request and relevant transaction details should remain connected to the approval: UMW Internal Audit: Audit Documentation Request Guidance.

For a useful decision record, retain the requester and subject of the decision alongside the reviewer’s identity, decision, timestamp, and relevant rationale or context. Vendor workflow documentation offers examples of how systems may record approval comments and decision history, but those examples illustrate implementations rather than establish a universal legal retention rule: ApproveThis: IT access requests with an audit-ready trail, AxonFlow: Human-in-the-Loop AI Approval Workflows, and Context: What does the audit story look like?.

Action: Grant Priya access to the staging environment for the deployment review.
Reason: She needs staging access to verify the release before the scheduled deployment.
Context: Link to the access request and the relevant change record.
