It usually cannot tell you with certainty whether you will develop a condition, when symptoms will begin, or how severe or fast-progressing it will be. The meaning of any result depends on the question asked, the test’s scope, and your medical and family history.
What genetic testing can answer
“Genetic testing” covers several different kinds of tests, and each is designed to answer a different question. A diagnostic test may look for a genetic explanation for symptoms or a suspected condition. Predictive or presymptomatic testing looks for changes associated with a condition that may develop later. Carrier testing asks whether someone carries a variant that could be passed to children. Pharmacogenomic testing examines whether genetic differences may affect response to particular medicines. Newborn screening and research testing have their own purposes and should not be treated as interchangeable with a clinical diagnostic test. The National Human Genome Research Institute (NHGRI) notes that genetic testing cannot tell you everything about inherited diseases: NHGRI’s Genetic Testing FAQ.

Genetic brain disorders are not one uniform group. MedlinePlus lists conditions such as leukodystrophies, phenylketonuria, Tay-Sachs disease, and Wilson disease as examples. Some conditions result from inherited changes, some from new genetic changes, and others involve both genetic and outside factors. Not every brain disorder is inherited, and not every condition has a single-gene test. See MedlinePlus Genetics: Genetic Brain Disorders.

Positive result
A positive result means the test found a genetic change of interest. Depending on the test and the variant, it may support a diagnosis, show that someone is a carrier, indicate increased risk, or point to a need for more testing. A positive predictive result is not automatically a forecast: it generally cannot specify whether or when symptoms will appear or predict severity and progression.
Negative result
A negative result means the test did not find a known relevant change among the regions or variants it examined. It does not necessarily rule out a condition. Some tests do not detect every disease-causing alteration, and a suspected disorder may have causes the test was not designed to find. The test’s scope and the person’s clinical and family history therefore matter when deciding whether further evaluation is appropriate.

Variant of uncertain significance (VUS)
A VUS is a genetic change for which the evidence is insufficient or conflicting about whether it contributes to disease. It is unresolved—not a confirmed diagnosis or established risk result. An uncertain finding cannot, by itself, confirm or rule out a diagnosis. In some cases, testing affected and unaffected relatives can provide additional evidence for interpreting the variant.

Why a DNA test cannot reliably predict Alzheimer’s or another brain disorder for everyone
A genetic report may indicate that a person has a higher or lower risk for a condition, but that is not the same as saying the person will or will not develop it. Consumer tests often assess selected variants rather than every genetic and non-genetic factor associated with a condition such as Alzheimer’s disease. A higher-risk result is not a definite prediction, and a lower-risk result does not mean the condition is impossible. The specific result’s meaning depends on the test and the clinical context.
Direct-to-consumer reports are not clinical diagnoses. Raw genotype data can also be difficult to interpret without professional help. MedlinePlus advises discussing health-related results with a healthcare professional before making major changes to health, diet, or fitness: MedlinePlus Genetics: Direct-to-Consumer Genetic Testing. How to judge whether a genetic test is useful
“Accurate” can mean more than one thing. Before relying on a test, separate three questions:
- Analytical validity: Does the laboratory method accurately detect the genetic change it claims to assess?
- Clinical validity: Is that change actually associated with the disease or risk being discussed?
- Clinical utility: Would knowing the result help with diagnosis, treatment, management, or prevention?
MedlinePlus notes that CLIA standards concern laboratory practice and are designed to support analytical validity; CLIA compliance alone does not establish a test’s clinical validity or clinical utility. A broader panel is not automatically more useful: the relevant question is whether the test fits the clinical purpose and its findings can meaningfully guide care. More detail is available at MedlinePlus Genetics: Genetic Testing—Validity and Utility.

Questions to ask before testing
A genetic counselor, medical geneticist, or other qualified healthcare professional can help weigh whether testing makes sense and interpret what the result could mean for you and your family. Because relatives share genetic material, a finding can have implications beyond the person tested. NHGRI describes the role of genetics professionals in its genetic testing guidance; MedlinePlus also discusses the benefits, risks, and limits of testing at MedlinePlus Genetics: Genetic Testing Considerations.
Quick Recap
Before ordering a test, ask:
- What condition and genes or variants does this test cover, and what can it miss?
- Is the test diagnostic, predictive, carrier, pharmacogenomic, or intended for another purpose?
- What would a positive, negative, or uncertain result mean for my particular situation?
- How are VUS findings handled, and could testing relatives help interpret one?
- What follow-up or professional interpretation will be available?
- Will I be able to download raw data, and what privacy protections apply to that file?
