Yes. A multimodal AI system can interpret instruction-like content inside an image and let it influence its response. The risk is not that an image magically runs code: it is that the model may confuse untrusted visual content with instructions, and an application may give that model access to sensitive data or consequential tools.
How an image can become an instruction
When a vision-capable model processes an image, it may read text in the image—whether plainly visible or visually concealed—and treat that text as a direction. If the image is handled alongside an ordinary user request, instructions embedded in the image can compete with the request or with the application’s intended behavior. This is known as image-based prompt injection, also called multimodal prompt injection.

The image is the delivery route; the security problem is instruction confusion. Unlike a program exploiting a software flaw by executing code in the image file, this attack attempts to influence how the model interprets content. OWASP describes prompt injection as a risk for multimodal inputs and warns that malicious image content can alter model behavior when processed with benign text. OWASP’s LLM01:2025 guidance explains the broader risk.
- Content arrives: A user or application supplies an image the model is asked to inspect.
- The model interprets it: The model may recognize instruction-like text as part of the image and respond to it, even if the person handling the image does not notice that text.
- The application determines the stakes: The result may be limited to a changed answer, or it may matter more if the model can access private records, call tools, or produce content that another system renders or acts on.
Why connected tools change the consequences
An injected instruction can steer a model’s output, but the consequences depend on what the surrounding application permits. Consider the difference between a model that can only describe an image and an agent that can access internal data or initiate actions.

| System | What an injection may affect | What constrains the impact |
|---|---|---|
| Image-understanding model without tools | Its answer, caption, or analysis may be diverted from the user’s request. | The model’s limited data access and inability to take actions through connected tools. |
| Tool-enabled agent | Its answer, and potentially proposed actions involving data or connected services. | The permissions and data access granted by the application, and whether those actions are independently authorized. |
That distinction is why prompt wording alone is not a security boundary. OWASP’s LLM Prompt Injection Prevention Cheat Sheet advises keeping trusted instructions separate from untrusted data, while cautioning that labels and prompt wording do not enforce that separation. Authorization needs to be enforced where the application grants access or executes a tool call.
What the reported attack rates do—and do not—show
Experiments demonstrate that image-based injection can succeed under particular test conditions, but their rates are not estimates of how many deployed AI systems are vulnerable.

- A March 4, 2026 arXiv preprint by Neha Nagaraja, Lan Zhang, Zhilong Wang, Bo Zhang, and Pawan Patil evaluated image-based prompt injection on COCO images with GPT-4-turbo. It reported an attack success rate of up to 64% for the study’s most effective configuration under stealth constraints. That is a result from the authors’ setup, not a rate for deployed models generally. Read the preprint.
- An April 19, 2025 arXiv preprint by Le Wang and co-authors studied coordinated cross-modal manipulation of multimodal agents. It reported at least a 26.4 percentage-point increase in attack success across its evaluated tasks. The comparison is specific to those tasks, not a universal increase for AI systems. Read the preprint.
The sources cited here do not establish a representative population estimate for how often image-borne injection occurs in deployed AI systems. Experimental success rates establish that the attack is feasible in evaluated settings; they do not establish how widespread it is in production.

OWASP’s Q1 2026 exploit roundup describes GrafanaGhost, disclosed on April 7, 2026, as an indirect prompt-injection path involving Grafana AI features. In the report’s account, malicious external content could lead the AI companion to ignore guardrails and render an external image, with enterprise data sent as a URL parameter to an attacker-controlled server. The roundup says exploitation required substantial user interaction; it notes patch acknowledgment on April 8, 2026, and says no CVE had been publicly assigned at the time of the report. This is an example of how indirect prompt injection and external rendering can combine in a particular application. It does not establish that every image-enabled AI system has the same weakness.
Controls that reduce the risk
No single prompt instruction can guarantee that a model will ignore malicious image content. The stronger approach is defense in depth: treat external content as untrusted, limit what the application can do, and check consequential operations outside the model.

Handle images as untrusted input
Apply the same caution to images as to documents, links, and other externally supplied content. A benign-looking image should not be treated as trustworthy simply because its instructions are not obvious to a person. Keep trusted instructions and untrusted content distinct in the system design, without relying on labels or prompt phrasing to enforce that distinction.

Require approval for consequential actions
Require human approval before operations such as sending, deleting, purchasing, or changing records. Approval should apply to the specific proposed operation, rather than granting general permission for a sequence of actions that could change after the model processes additional content.
Control external requests and rendered output
If the application can render model-produced content or make external requests, restrict those paths. Validate URLs and output handling so that a manipulated response cannot freely trigger an external request or expose data through rendered content.
Test systematically, not with one example
Evaluate varied image inputs and repeat attempts. For results to be interpretable, record the model and version, defense configuration, test corpus, number of runs, and how success is defined. A single blocked example is not evidence that the system is robust across inputs or repeated attempts.
The practical security boundary
Image-based prompt injection is a real model-interpretation risk, but its impact is shaped by the application around the model. The central safeguard is to ensure that interpreting an image cannot, by itself, grant access to data or authority to take action: enforce permissions at tool boundaries, keep capabilities limited, and put human review and output controls where the consequences warrant them.
