The spinning beach ball is macOS’s wait cursor: it means an app is temporarily not responding normally or is waiting. It is a symptom, not a diagnosis, so the cursor alone cannot tell you what caused the delay.
What the spinning beach ball tells you
When the cursor appears over an app, that app may be busy, stalled, or unable to respond at that moment. Apple describes the related condition as an app or process being “not responding.” In Activity Monitor, such processes are marked “(Not Responding).” Apple’s Activity Monitor guide

The cursor does not identify a particular fault. Possible factors include the app, a file it is handling, compatibility, a peripheral, a plug-in, or a broader hardware issue. A useful first distinction is whether one app or several are affected, and whether the episode is brief or keeps recurring.
What to do when it appears
Try closing and reopening the app
If the app becomes responsive, save your work and continue. If it remains stuck, Apple recommends closing and reopening it. Apple’s app troubleshooting steps

Force Quit only if needed
Open the Apple menu and choose Force Quit. Select the unresponsive app and choose Force Quit. Unsaved changes may be lost. Apple’s guidance for frozen apps
Check the process in Activity Monitor
Open Activity Monitor, find the process marked “(Not Responding),” select it, and click the Stop button. Choose Quit when you want the process to exit when safe; choose Force Quit to end it immediately. Force quitting may lose data or cause problems for apps or processes that depend on it. If the beach ball keeps returning
Repeated episodes call for broader troubleshooting rather than assuming the cursor points to one specific cause. Apple recommends restarting the Mac and checking for software updates. Also check whether the app is compatible with your macOS version, try a different file, and consider whether the issue began after connecting a peripheral or installing a plug-in. Activity Monitor can show CPU, memory, energy, disk, and network activity, which may help you observe what the Mac is doing. Those readings alone do not establish why a particular beach-ball episode happened. Activity Monitor User Guide
