A CDX2 test usually uses immunohistochemistry (IHC) to check whether cells in a tumor-tissue sample express the CDX2 protein. A positive result supports intestinal differentiation and can help pathologists characterize a tumor; a negative or reduced result does not rule out colorectal cancer. CDX2 is one part of the pathology picture, not a stand-alone test for tumor origin, stage, prognosis, Lynch syndrome, or treatment choice. It also does not replace direct mismatch-repair (MMR) or microsatellite-instability (MSI) testing.
What a CDX2 test measures
CDX2 is a transcription factor associated with intestinal differentiation. In pathology, a CDX2 test generally means IHC performed on tumor tissue, often from a biopsy or a surgical resection. The test looks for CDX2 protein expression in the tumor cells. Published studies have used formalin-fixed, paraffin-embedded primary colorectal tumor tissue.

The pathology report may describe the stain as positive or present, reduced, or negative or lost. The wording reflects what was seen in the tested sample and how the laboratory interpreted the stain. Laboratories may use different assays and scoring approaches; the evidence does not establish one threshold that applies universally. If a report gives a percentage, intensity, or other score, its meaning should be interpreted using that laboratory’s criteria. A result is not simply a universal numerical cutoff that can be compared across every report or laboratory.

Those roles are related but not interchangeable. A stain that helps identify a tumor’s differentiation pattern does not, by itself, establish what will happen to a particular patient. Nor does evidence of these roles mean every patient needs CDX2 testing; the evidence does not establish a universal CDX2 ordering guideline.

What CDX2-positive means
A positive stain means the tested tumor cells express CDX2 under the laboratory’s interpretation criteria. In the appropriate clinical and microscopic setting, that finding supports an intestinal phenotype and can contribute to identifying or characterizing a colorectal primary or a metastasis.
Pathologists interpret CDX2 alongside the tumor’s appearance under the microscope, the patient’s clinical history, and other findings. A broader immunohistochemical panel may include SATB2 and cytokeratins such as CK7 and CK20. The combination can be more informative than any one marker alone. A positive CDX2 result therefore supports a conclusion; it is not, by itself, proof of the complete diagnosis or of the tumor’s site of origin in every case.
CDX2 positivity does not determine the cancer’s stage or show whether it has spread. It also does not identify Lynch syndrome, establish MSI-high status, or independently select a treatment. Those questions require other clinical, pathology, and molecular information. In practical terms, the stain answers whether CDX2 expression was seen in the tested tumor cells; it does not answer every question about what the cancer is or how it should be managed.
What CDX2-negative or reduced means
A negative or lost result means that the sampled tumor did not show the expected CDX2 expression according to the laboratory’s criteria. Reduced expression means the stain was present but diminished as interpreted by the laboratory. These results do not rule out colorectal cancer: most colorectal cancers express CDX2, but a minority do not, and expression varies among tumor types.

Loss of CDX2 has been associated in some cohorts with right-sided tumors, deficient MMR (dMMR), and BRAF mutation. These are associations, not diagnostic rules. In the 449-patient cohort discussed below, the CDX2-negative subgroup was small, so those patterns cannot establish the molecular status or tumor location for an individual patient. A negative result should not be used to infer that a tumor must be right-sided, dMMR, or BRAF-mutated.
A negative stain should be read in the context of the tumor morphology, other stains, and any relevant molecular testing. If the report’s wording is unclear, ask the pathologist or oncologist what “negative,” “lost,” or “reduced” means under the laboratory’s scoring method and how it fits with the other findings. In particular, ask whether the finding changed the pathologist’s interpretation or is one piece of a broader panel.
How often colorectal tumors express CDX2
Published rates vary by study, patient selection, tumor histology, antibody methods, and scoring. The estimates below describe the cited studies’ populations; they are not predictions for an individual sample.
| Finding | Reported result | Study context |
|---|---|---|
| CDX2 expression in primary colorectal cancer | 88.2% | 2022 meta-analysis by Hong Bae Choi and colleagues, pooling 38 articles. |
| CDX2 expression in metastatic colorectal cancer | 89.3% | 2022 meta-analysis by Hong Bae Choi and colleagues, pooling 38 articles. |
| Expression in adenocarcinoma versus medullary carcinoma | 88.6% versus 43.6% | Histology-specific estimates reported in the 2022 meta-analysis. |
| CDX2-negative among evaluable patients | 18 of 441, or 4.1% | Korphaisarn and colleagues’ 449-patient study, published online in 2021 and in a 2022 journal issue. |
The meta-analysis estimated expression in most primary and metastatic colorectal cancers, while finding lower expression in medullary carcinoma than in adenocarcinoma. The 449-patient cohort reported a different negative rate from some earlier cohorts. Differences in who was studied, histology, antibody methods, and scoring may contribute; no single percentage should be treated as universal. The pooled rate and an individual study’s rate are not contradictory guarantees about a particular person’s result; they summarize different groups of tumors studied under varying conditions.

CDX2 compared with MMR and MSI testing
CDX2, MMR immunohistochemistry, and MSI testing answer different questions. CDX2 IHC looks for a differentiation-associated protein. MMR IHC evaluates mismatch-repair proteins, while MSI testing assesses microsatellite instability. CDX2 loss has been associated with MMR deficiency in some studies, but it is not exclusive to dMMR and cannot establish MMR or MSI status.
| Test | What it evaluates | What it can contribute | Can it establish MMR deficiency or MSI status? |
|---|---|---|---|
| CDX2 IHC | Expression of the CDX2 protein in tumor cells. | Evidence supporting intestinal differentiation and tumor characterization when combined with other findings. | No. Direct MMR or MSI assessment is needed. |
| MMR IHC | Mismatch-repair protein expression. | Assessment of MMR enzyme status. | It directly evaluates MMR proteins; interpretation belongs with the pathology report and clinical context. |
| MSI testing | Microsatellite instability. | Assessment of MSI status; in stage II cases, results can help inform adjuvant chemotherapy decisions. | Yes, it directly assesses MSI rather than inferring it from CDX2. |
The College of American Pathologists’ colorectal protocol describes universal testing for MSI and/or MMR enzyme status by IHC as recommended by EGAPP guidelines. Its protocol states: “Since MSI-H cancers have a favorable prognosis, MSI testing for stage II cases can help in making decisions regarding adjuvant chemotherapy.” That statement concerns MSI testing, not a universal mandate to test CDX2 or a treatment decision based on CDX2 alone. The cited protocol version is CAP ColoRectal 4.4.0.1, identified as approximately 2025.
Because CDX2 is not an MMR or MSI assay, a CDX2 result cannot tell you whether a cancer is MSI-high or whether a person has Lynch syndrome. Lynch syndrome assessment requires the appropriate testing and clinical evaluation; CDX2 alone cannot answer that question. If those are the questions behind a report, look for direct MMR or MSI results and discuss whether further evaluation is appropriate with the treating team.

Loss of CDX2 has been associated with adverse features and poorer outcomes in some colorectal cancer cohorts. The 2022 meta-analysis found better overall and cancer-specific survival among patients whose tumors expressed CDX2, although its underlying studies varied. Such findings describe groups, not an individual patient’s outlook.

In one 449-patient study, patients with stage I–III disease and CDX2 loss had worse univariate five-year overall and disease-free survival. In multivariable analysis, loss remained associated with worse overall survival: hazard ratio 2.4, with a 95% confidence interval of 1.12–5.11. This is a cohort-level association, not a personal survival estimate or a prediction that a particular person will have a specific outcome.
A separate study of stage II–III disease found that MMR phenotype changed the relationship between CDX2 negativity and outcome. In that study, pMMR/CDX2-negative cases had worse cancer-specific survival than the compared groups, while the interpretation differed for dMMR/CDX2-negative tumors. The result illustrates why CDX2 should not be treated as an isolated prognostic label: stage and MMR status matter to interpreting the observed association.
These studies do not establish CDX2 as a stand-alone clinical prediction tool or as a treatment-selection test. A clinician interprets prognosis using the full case, including stage, pathology, molecular findings, and other relevant clinical information. Ask how CDX2 fits with those factors rather than treating the stain as a forecast by itself. The published estimates are useful for describing patterns across patient groups, but they cannot say what an individual patient’s outcome will be.
How to read the result in your pathology report
Start by identifying exactly what the report says: positive, reduced, or lost/negative, and whether it provides a score or percentage. Then read the result alongside the diagnosis, other immunostains, and any MMR or MSI results. A stain is a finding about the tested tissue, not a complete summary of the cancer.
Best Value
- Locate the CDX2 entry. Check whether the report calls expression positive, reduced, or negative/lost, and note any stated scoring details.
- Review the surrounding pathology findings. Consider the tumor diagnosis and morphology, and the other markers listed in the report, including any SATB2, CK7, or CK20 results.
- Check MMR/MSI separately. Look for direct MMR IHC or MSI results; do not infer them from CDX2.
- Ask how the result affects the case. Your oncologist or pathologist can explain whether CDX2 contributed to characterizing the tumor and how it relates to the full diagnosis and other test results.
Useful questions include: “What does positive or negative mean under this laboratory’s scoring approach?” “How did CDX2 fit with the tumor’s appearance and the other stains?” “Have MMR or MSI been tested directly?” and “Does this result change any decision in my case, or is it an additional pathology finding?” You can also ask whether the result is being used to help characterize the tumor, rather than to make a prediction or treatment decision on its own.
Frequently Asked Questions
What does CDX2 positive mean in colorectal cancer?
It means the tested tumor cells express the CDX2 protein under the laboratory’s interpretation criteria. In context, this supports intestinal differentiation and can help characterize a colorectal primary or metastasis. It does not independently establish the full diagnosis, stage, or treatment.
What does it mean if CDX2 is negative?
A negative or lost stain means the sampled tumor did not show expected CDX2 expression according to the laboratory’s criteria. It does not rule out colorectal cancer. Loss has been associated with some tumor features, including right-sided location, dMMR, and BRAF mutation, but those associations are not diagnostic rules for an individual.
Recommended Free Tools
Does CDX2 tell me if my cancer is MSI-high or if I have Lynch syndrome?
No. CDX2 IHC evaluates a differentiation-associated protein. MMR IHC and MSI testing assess different features and are needed to determine MMR or MSI status. CDX2 alone cannot identify Lynch syndrome.
Does CDX2 predict my prognosis?
CDX2 loss has been associated with poorer outcomes in some patient cohorts, but the relationship varies with factors such as stage and MMR phenotype. The result is not a personal survival estimate or a stand-alone prediction tool; your clinician interprets it with the full pathology and clinical picture.
Does a CDX2 result determine my treatment?
No stand-alone treatment-selection role is established for CDX2. A positive or negative stain does not independently select therapy. Direct MSI testing can help inform adjuvant chemotherapy decisions in stage II cases, but that is a role of MSI assessment, not CDX2.
The Bottom Line
CDX2 IHC shows whether tumor cells express a protein associated with intestinal differentiation. Positivity can support tumor characterization, while loss does not rule out colorectal cancer and may have population-level prognostic associations. Interpret the stain with the full pathology report and clinical context; use direct MMR or MSI testing for those results.
