When an AI agent can act without human approval, a mistaken interpretation or malicious instruction can become a real change in connected systems. The damage depends less on how confidently the agent responds than on what tools and permissions it can use—and whether an independent control checks consequential actions before they execute.
How an agent can turn an instruction into an action
An AI agent does more than generate text: it may read emails, files, or web pages, then call tools such as search, messaging, cloud storage, or administrative systems to pursue a goal. If it treats hostile content as an instruction, it can use those tools to carry out the attacker’s intent while appearing to continue the user’s task.

NIST describes a central weakness in this kind of agent hijacking: trusted developer instructions may not be reliably separated from untrusted external data. A malicious instruction can be hidden in an ordinary-looking email, document, or website the agent is asked to process. The chain is straightforward: the agent reads the content, mistakes it for valid direction, and applies the authority its connected tools provide.
What can go wrong
Following an attacker’s instructions
An injected instruction could redirect the agent to download and run untrusted code, send cloud files to an unknown recipient, or send phishing messages. NIST’s evaluation included scenarios of this kind in a simulated environment. They demonstrate possible consequences under test conditions; they are not counts of confirmed incidents in deployed products.

Using more authority than the task needs
Permissions set the blast radius. A tool intended to summarize email does not need permission to send or delete messages. A broadly privileged service identity may also expose data beyond the individual user’s scope. OWASP treats excessive permissions and excessive autonomy as separate risks: the agent may make an unauthorized tool call, or it may be allowed to act too freely even when its tools are legitimate. Its guidance recommends narrow tools and user-scoped permissions (OWASP, LLM06:2025 Excessive Agency).
Making a destructive or public change
Deletion, payments, security or permission changes, production deployments, and public posts can be difficult to reverse or costly to correct. If a misunderstanding or hijack triggers one of these actions, the change may take effect before anyone notices. The concern is not limited to malicious prompts: an agent can also misread a request or act on incomplete context.
Exposing data or sending harmful messages
An agent that can both read and send may be manipulated into forwarding sensitive content or sending misleading messages at scale. OWASP describes an example in which a malicious incoming email tricks an email agent into searching the inbox and forwarding sensitive information. If sending is not needed, remove that capability; where it is needed, limit access to the user’s own data and review messages before they go out (OWASP, LLM06:2025 Excessive Agency).

Compounding errors, retries, and costs
One flawed action can trigger later actions across connected services or agents. OWASP identifies cascading failures in multi-agent systems and denial-of-wallet attacks caused by unbounded loops. Repeated attempts can also matter for hijacking: in one NIST evaluation across five injection tasks, average attack success rose from 57% after one attempt to 80% when each attack was tried 25 times. Those percentages describe that specific test setup, not the general failure rate of agents in use.
What NIST’s attack results do—and do not—show
NIST CAISI reported a held-out evaluation of Workspace tasks in which the strongest attack success rate rose from 11% for the strongest baseline attack to 81% for the strongest new attack developed for the upgraded model. These are controlled red-team results for particular models, tasks, environments, and attack methods. They show that evaluation outcomes can vary substantially with the attack method; they do not estimate how often real-world agents are compromised.
The sources cited here do not establish a representative rate of real-world incidents caused by agents acting without approval, nor do they establish a named real-world no-approval incident. The NIST figures should not be read as the percentage of deployed agents that fail.

Why an approval button is not enough
A human checkpoint can catch a consequential action, but a generic “Approve” prompt is weak protection if the person cannot see what will happen or if the authorization can be reused or altered. OWASP recommends controls beyond a simple approval prompt for destructive, financial, administrative, or externally visible actions: approval should be tied to the actor, tool, target, parameters, time, and expiry, and the downstream system should enforce authorization too (OWASP AI Agent Security Cheat Sheet).

Approvals also lose value if users are interrupted for every routine step. NIST’s comments summary records concern that frequent requests can produce consent fatigue. Reserve human review for actions whose consequences justify the interruption, and make each request specific enough to support a real decision (NIST NCCoE summary of comments).

Give the agent only the authority it needs
- Separate read and write access; do not grant sending, deletion, payment, or administrative tools to a task that does not require them.
- Scope credentials to the user and the resources needed for the task, rather than connecting the agent through a broadly privileged identity.
- Prefer narrow, purpose-built tools over generic tools that can perform many unrelated actions.
Match oversight to impact and reversibility
Read-only, low-impact actions within the user’s scope may be suitable for automation. Require stronger review for deletion, payments, permission changes, external messages, production changes, and actions whose purpose or authorization is unclear. Impact, reversibility, data sensitivity, external visibility, permission scope, and the ability to verify execution are useful practical factors—not a universal risk-scoring standard.

Make the proposed action concrete
Show the person the actual tool, target, and normalized parameters: for example, which file will be shared, with whom, or which deployment will be changed. Bind approval to that exact action, make it short-lived, and prevent replay. An approval for one recipient or transaction should not silently authorize a different one.
Check authorization outside the model
A separate policy service or the destination system should validate identity, scope, authorization, and approval at execution time. Do not rely on the agent’s own judgment to certify that its action is permitted. Fail closed if approval, risk classification, or audit validation is unavailable, and use idempotency where possible to reduce the harm from duplicate execution. OWASP’s live AI Agent Security Cheat Sheet details these execution controls.
Keep records and test against attackers
Log tool calls and consequential actions so an operator can reconstruct what happened. Rate-limit harmful operations and test with adversarial inputs and repeated attempts, rather than checking only whether the agent handles a single benign prompt. NIST’s evaluation results illustrate why multiple attempts can reveal risks missed by a one-shot test (NIST CAISI, published January 17, 2025; updated December 19, 2025).
