An architecture decision record (ADR) should make visible the premises that materially affect a decision: what the team knows, what it believes, and what remains uncertain about requirements, constraints, dependencies, quality attributes, stakeholders, or future conditions. Explain how those premises shaped the selected option, what trade-offs follow, and what change would justify revisiting the choice. An “Assumptions” heading can help, but no cited template makes that heading universal.
What an ADR needs to establish
The essential job is to let a future reader understand why the decision made sense in its original context. AWS says an ADR should define the decision context, the decision, and its consequences for the project and deliverables. The GDS Way describes a common format as title, status, context, decision, and consequences; its context captures the facts behind the need for a decision.

The GOV.UK Architectural Decision Record Framework adds date, stakeholders consulted, and links to supporting documents. These are useful traceability fields, not a requirement that every team adopt one identical template. A dedicated “Assumptions” section is likewise optional: include the information wherever it makes the context and rationale clear.

Which assumptions are worth recording?
Record an assumption when a reader could mistake it for an established fact, or when the decision would be hard to understand without it. Focus on premises that could change the choice or its expected outcome, rather than listing every background belief.
- Requirements and quality attributes: what the system must do, and which qualities—such as availability, security, or performance—are driving the choice.
- Constraints: limits that shape feasible options, such as compatibility, policy, schedule, or operating conditions.
- Dependencies and environment: what the decision relies on elsewhere in the system or on an external service, platform, or operating environment.
- Stakeholders and responsibilities: whose needs or decisions the architecture depends on, where relevant.
- Future conditions: expected growth, change, or continued support that materially affects the option’s suitability.
Separate verified facts from estimates and unresolved beliefs. Identify the basis for a consequential assumption where possible—a requirement, stakeholder input, measurement, or linked analysis—so readers can judge how much weight to give it. This is a practical way to make context and rationale legible, not a prescribed field list shared by every ADR template.
Connect assumptions to the decision and its alternatives
Do not leave important assumptions as an isolated inventory. State how each significant premise favors or rules out an option. For example, if a choice depends on an external service meeting a particular availability need, say that this expectation informed the selection and link to the evidence or analysis supporting it.

For consequential decisions, briefly describe the serious alternatives and their pros and cons. The ADR should preserve enough reasoning to explain why the chosen option won, but it need not become a full design document: Martin Fowler recommends keeping an ADR brief and linking to deeper analysis where needed. AWS guidance also places the decision in its context and consequences; neither calls for reproducing every design detail in the record.

Make uncertainty and trade-offs useful to future readers
Show what the decision makes easier and what it makes harder. Include material risks, costs, or downstream effects, not only intended benefits. The GDS Way explicitly calls for positive and negative consequences, and the GOV.UK template includes consequences as a core field.

Where uncertainty could affect the decision, state its confidence level if that helps, what evidence could resolve it, and who can validate it. Name a concrete trigger for reconsideration, such as a requirement changing, a dependency no longer meeting expectations, or an assumed constraint being removed. Fowler recommends recording confidence and context changes that should prompt reevaluation; GOV.UK advises reviewing the decision as its context or consequences change.
Choose a proportionate format and governance path

The GOV.UK framework is expressly for the UK public sector and maps decision scope to team, programme, department, or cross-government levels and review bodies. Those governance arrangements should not be treated as universal rules for private-sector or other national contexts. Whatever format a team uses, keep the ADR concise, link supporting material, and agree how decisions are reviewed and shared.

Keep the decision record current without erasing its history
An accepted ADR is evidence of the decision made under the conditions recorded at the time. If the decision changes, AWS guidance recommends a new, linked ADR that supersedes the earlier one; GDS Way similarly says to mark a decision as superseded and link its replacement. This preserves the original reasoning instead of silently rewriting history. Teams can record implementation clarifications or newly discovered consequences through their agreed process.
