Blockchain projects face threats at several layers: smart-contract code and permissions, consensus mechanisms, external data, and the wallets people use to sign transactions. Five representative attack classes are contract logic flaws, access-control failures, oracle manipulation, consensus attacks, and phishing or key theft. They are not a universal ranking of the most frequent attacks: contract vulnerabilities, network attacks, and wallet scams are measured and addressed in different ways.
The OWASP Foundation’s 2025 Smart Contract Top 10 reports analyzing 149 security incidents and more than $1.42 billion in documented losses across the decentralized-ecosystem datasets it cites. Those figures describe the OWASP analysis, not all blockchain attacks or a universal estimate of losses. Its analysis draws on SolidityScan’s Web3HackHub (2024), Peter Kacherginsky’s “Top 10 DeFi Attack Vectors – 2024,” and Immunefi’s Crypto Losses in 2024 Report.

1. How can a smart-contract logic flaw drain assets?
A contract can behave as coded and still be unsafe if its logic permits an unintended result. Reentrancy is one example: a contract makes an external call, transferring control to untrusted code before completing its own operation. If the original contract has not yet updated its state, the caller may call back into it and repeat an operation using stale state.
Ethereum.org’s Smart contract security guidance describes it this way: “A reentrancy attack occurs when a malicious contract calls back into a vulnerable contract before the original function invocation is complete.” The risk is not limited to a particular token or application type; it depends on how the contract handles external calls and state.
#1 Best Overall
- FIDO2 & Passkey Ready: Business-ready and FIDO2 L1 certified. This key is supported by major management suites and is ideal for both individual and enterprise deployment. Works seamlessly with Gmail, Facebook, GitHub, Dropbox, Coinbase, and more.
- Universal Connectivity (USB-A ): Features a built-in USB-A connector—simply unfold the key and plug it into your compatible PC or laptop for seamless authentication on the go.
- Dedicated Manager App: Use the Thetis Manager App for the initial hardware PIN setup. Setting the PIN on the device first ensures a smooth registration process. Once the PIN is configured, you can begin registering the key across your favorite FIDO2-compatible online services.
- Ultra-Durable & Portable: Featuring a rotating metal cover, this key is water, crush, and tamper-resistant. It fits easily on a keychain and requires no batteries or network connectivity.
- Check FIDO2 compatibility before purchase - Known limitations: ID Austria is not supported (requires FIDO2 Level 2). Windows Hello login only works with Windows Enterprise editions that support Entra ID, and NFC is NOT supported.
Protect the contract’s operation order
- Apply checks-effects-interactions: validate the request, update the contract’s state, and then make external calls.
- Keep contract logic as simple as practical and use established libraries where appropriate.
- Test expected behavior as well as properties that should always hold, including under unusual inputs and call sequences.
Code already deployed to a public blockchain is usually difficult to change, and assets taken through a contract flaw can be difficult to recover. Design and review decisions therefore matter before deployment, not just after a problem appears.
2. What goes wrong when access controls are missing or incorrect?
Public and external contract functions can be called by accounts and other contracts on the network. If a sensitive operation—such as minting or administration—does not enforce the intended authorization, an unauthorized caller may be able to use it. A permission check that exists but is incorrectly applied can create the same kind of exposure.

Limit sensitive permissions
- Identify which functions can change important contract behavior or assets, and require explicit authorization for them.
- Use an owner-based or role-based access-control design suited to the project’s needs, and verify that each sensitive function enforces it correctly.
- Give each role only the permissions it needs; avoid treating a function’s visibility or an assumption about who will call it as authorization.
Ethereum.org’s Smart contract security guidance covers owner-based and role-based approaches. The right design depends on the project’s administration model; simply adding a role does not establish that permissions are correctly scoped.

3. How can an oracle or price feed be manipulated?
Contracts that rely on external data inherit assumptions about where that data comes from and how it changes. In particular, relying on an on-chain spot price can expose application logic to price manipulation. The impact depends on how the contract uses that value, so oracle risk needs to be assessed as part of the application’s design rather than treated as a generic feed-integration task.

Review the data dependency in context
- Document the source of each external price or market-data input and the contract decisions that depend on it.
- Evaluate whether the source and its update behavior are appropriate for those decisions, including the assumptions the application makes about market conditions.
- Test how the application behaves when the input changes or does not match the assumptions its logic requires.
These checks do not make every oracle or market-data design safe by default; they make its dependencies and assumptions visible for review.
4. What can a consensus attack do to a blockchain project?
Consensus attacks target a network’s ability to agree on transaction history and ordering, rather than a particular application’s contract code. An attacker’s capabilities depend on the chain’s consensus mechanism and the resources that mechanism requires. A “51% attack” is therefore not a universal description of every blockchain attack.

Ethereum proof-of-stake thresholds are network-specific
Ethereum.org’s Ethereum proof-of-stake attack and defense guidance describes the following capabilities in the Ethereum proof-of-stake case. These stake thresholds should not be applied to other networks or conflated with proof-of-work hash power.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
| Stake threshold described | Capabilities associated with it in Ethereum’s guidance |
|---|---|
| 33% | Delay finality. |
| 34% | Delay finality and possibly cause double finality. |
| 51% | Delay finality, cause double finality, censor transactions, and control the future. |
| 66% | The capabilities above, plus control over the past. |
These are descriptions of protocol-specific capabilities, not a guarantee that an attack will produce a particular outcome. Ethereum’s guidance also discusses substantial economic costs, slashing, social coordination, and caveats. Project teams should use the documentation for the network they actually depend on when assessing consensus risk.

5. How do phishing and key theft put wallets at risk?
A recovery phrase or private key gives control of the associated wallet. If an attacker obtains it through phishing or another form of social engineering, they can act as that wallet’s controller. This is a risk to accounts and signing behavior; it does not repair or secure vulnerable contract code.
Protect keys and review what you sign
- Never disclose a recovery phrase or private key, and do not keep cloud screenshots of either.
- Use offline private-key storage where appropriate. A hardware wallet can store keys offline, but it cannot determine whether a smart contract is safe.
- Check recipient addresses and read transaction messages before signing.
- Avoid unlimited token spend approvals; grant only the approval the situation calls for.
These practices follow Ethereum.org’s Ethereum security and scam prevention guidance. They help protect wallet access and signing decisions, but they are distinct from reviewing application contracts or securing a chain’s consensus.
How should a project build assurance across these layers?
No single review method establishes that a blockchain project is secure. Use assurance techniques that fit the code and threat being assessed, and treat each as a different kind of evidence.

- Developer tests: Check intended behavior and important properties as the contract is built.
- Static and dynamic analysis: Use analysis tools during development to examine code and its behavior. These methods complement, rather than replace, tests and human review.
- Fuzzing: Explore behavior with random inputs to find cases ordinary tests may miss.
- Formal verification: Use it to prove properties only against a formal specification and model. A proof does not establish properties that were omitted or modeled incorrectly.
- Independent audits: Add external review of the contract. An audit is not a guarantee that every flaw will be found.
- Bug bounties: Offer a channel for external researchers to report issues responsibly. A bounty can add scrutiny, but it is not proof that no undiscovered flaw remains.
Maintain version control and arrange code review independently of the people who wrote the code. Ethereum.org’s Smart contract security, audit, and bug-bounty guidance supports using these measures as layers of assurance rather than substitutes for one another.
