App Sandbox does not make a Mac disk cleaner useless, but it does prevent a sandboxed cleaner from freely scanning the whole disk. By default, it can read and write inside its own container; access to other files depends on user selection, saved permissions, Full Disk Access, and other macOS protections.
What App Sandbox limits
macOS gives a sandboxed app unrestricted access to its own container, not automatic access to the entire home folder or startup disk. This boundary limits what a cleaner can inspect or change without additional authorization. Apple describes the sandbox as a way to restrict an app’s access to user data and system resources in Protecting user data with App Sandbox.

That restriction affects the cleaner’s reach, not every operation it performs. It can still analyze and remove eligible files in locations it is allowed to access. A cleaner may therefore work on its container or on folders the user has authorized, while reporting other locations as unavailable.
How a cleaner can access files outside its container
User-selected files and folders
When a user selects a file or folder through an appropriate system interface, macOS can grant the app access to that resource. Selecting a folder can extend access to its nested contents. The app still may not be able to use a particular item if another macOS permission or protection blocks it.

Access that persists between launches
For continuing access to a selected resource, an app can create and store a security-scoped bookmark. On a later launch, it must resolve that bookmark and start access to the resulting URL before using the resource; it should stop scoped access when finished. Merely selecting a folder once does not mean an app can assume permanent access. Apple explains this flow in Accessing files from the macOS App Sandbox.
Full Disk Access
Full Disk Access is a separate permission that the user grants in System Settings > Privacy & Security. An app cannot obtain it automatically through an entitlement or through code. Apple states: “Your app can’t automatically gain full disk access through an entitlement or with code: the person using your app must choose to grant access in System Settings > Privacy & Security.” A cleaner should handle a denial rather than assume the user will approve the request. Does a disk cleaner need Full Disk Access?
Not necessarily. Whether it needs that permission depends on the locations and actions behind the feature. A cleaner that scans only accessible folders can still analyze or remove eligible files there. A feature that promises to inspect or clean a wider range of protected locations may require Full Disk Access or access to a folder selected by the user, as well as app support for that access.

POSIX permissions, access control lists (ACLs), System Integrity Protection, and data protection can also deny access. If a scan omits a location or an attempted cleanup fails, the cause may be one of these protections rather than the sandbox alone.

Some cleaner features may conflict with sandbox rules
Apple documents activities that are incompatible with App Sandbox, including using the Authorization Services API, accessing or changing preferences belonging to other apps, and terminating other running apps. These restrictions can affect particular cleaner features, but they do not mean that every cleanup or disk-analysis operation is prohibited. A feature’s exact behavior depends on what it does and what access it has.

A feature list alone cannot establish whether a cleaner is sandboxed, what entitlements it has, or which files it can reach at runtime. For example, DiskCleaner’s documentation lists cleanup, app-uninstall, disk-analysis, system-monitor, optimization, project-purge, and installer-cleanup tools; that vendor description does not establish the app’s sandbox configuration or independently verify its access. - Which locations can it scan before you grant additional access?
- Can you select folders, and does the app explain how that access is retained?
- Does a particular feature ask for Full Disk Access, and what does it do if you decline?
- Does the scan clearly report inaccessible locations or files?
