Weight-Bearing vs. Resistance Exercise for Bone Health: What’s the Difference?

Weight-bearing exercise loads bones while you are upright; resistance exercise challenges muscles against a force. Learn why bone-health guidance favors combining both with appropriate balance work.

Weight-bearing exercise loads your skeleton while you are upright and working against gravity. Resistance exercise makes your muscles work against an opposing force, such as your body weight, an elastic band, free weights or a machine. A single movement can provide both kinds of loading.

Neither category is universally “better.” Guidance from the Bone Health & Osteoporosis Foundation (BHOF), the U.S. National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS), and the Royal Osteoporosis Society supports combining weight-bearing, resistance and—when appropriate—balance exercise, with the choice and progression adapted to your fracture history and health.

What weight-bearing exercise means

Weight-bearing exercise is movement performed while your feet or another part of your body supports you against gravity. Your bones carry some of the load instead of being fully supported by water, a saddle or a machine. The stimulus can come from simply being upright, from moving your body through space, or from impact as your feet repeatedly contact the ground.

The BHOF and NIAMS list walking, climbing stairs, dancing, jogging and racket sports as weight-bearing examples. These activities do not all load the skeleton to the same degree: a stroll and running have different impact, and a person’s speed, surface, footwear and medical history change the demands. “Weight-bearing” describes the basic loading situation, not a guarantee that an activity is suitable for everyone.

Why upright loading matters

Bone is exposed to a mechanical load when you stand and move against gravity. Regular loading is one part of a program intended to help maintain bone health. The practical question is whether the movement is safe and sufficiently challenging for the individual, not whether it has the weight-bearing label alone.

What resistance exercise means

Resistance exercise makes a muscle produce force against an opposing load. The resistance may be your own body weight, an elastic band, a dumbbell or barbell, or a resistance machine. Pressing, pulling, lifting, lowering and holding are all ways to ask muscles to work against that force.

Resistance training can be done at home or in a gym. Equipment is optional: a body-weight movement is still resistance exercise, while bands, free weights and machines let you change the challenge. The key feature is muscular effort against resistance, rather than whether the exercise is performed standing.

How it can overlap with weight-bearing

Categories describe the main loading stimulus, so they can overlap. A body-weight squat performed while standing loads muscles against resistance and places the skeleton upright against gravity. Stair climbing also has an upright, weight-bearing component and requires the leg muscles to overcome resistance. By contrast, a seated machine exercise may provide substantial resistance work while giving the body more support and little or no impact.

Side-by-side comparison

Question Weight-bearing exercise Resistance exercise
Primary stimulus Upright loading against gravity, sometimes with impact Muscles working against an opposing force
Examples Walking, stairs, dancing, jogging and racket sports Body-weight movements, elastic bands, free weights and machines
Body support The feet or another body part supports you while you move May be standing, seated or lying down, depending on the exercise
Impact Varies by activity; walking and jogging are not equivalent loads Impact is not required; the challenge comes from resistance
Equipment Often possible with no equipment Body weight is enough, with bands, weights or machines as options
Balance contribution Upright activities can challenge balance, but the effect depends on the movement Can support strength needed for everyday tasks; balance work may need to be added separately
Main safety question Is the impact and upright load appropriate for your bones and balance? Is the resistance, position and technique appropriate for your strength and fracture history?

The comparison is about how the load is applied, not a ranking. An exercise can be useful for general fitness without being a meaningful impact exercise, and an exercise can strengthen muscles without putting the body through impact.

Which activities count as weight-bearing?

Walking and stairs

Walking keeps you upright and asks the legs and trunk to support the body. Stair climbing adds repeated rises and descents and can be more demanding than level walking. Use a handrail or another support if balance is uncertain, and ask a professional how to modify the activity if you have a fracture history.

Dancing and jogging

Dancing is weight-bearing because you remain upright while stepping and changing direction. Jogging adds greater repeated impact than ordinary walking. Whether that impact is appropriate depends on bone density, previous fractures, joint health, balance and conditioning; the label alone does not make jogging safe for every person.

Racket sports

Racket sports combine time on your feet with starts, stops and changes of direction. They therefore provide weight-bearing loading and may challenge balance and coordination. A healthcare or exercise professional can help determine whether the speed, twisting and impact fit your situation.

Activities that support fitness but not impact loading

Swimming and cycling can be excellent cardiovascular activities, yet the water or bicycle supports much of your body weight. They are not impact exercises in the way walking, stairs, dancing, jogging and racket sports are. They can remain part of a well-rounded routine, but they do not replace a weight-bearing component when bone loading is the goal.

Which activities count as resistance exercise?

Body-weight movements

When you move or hold your body against gravity, your muscles provide force against resistance. Examples include controlled standing or seated movements that use your body weight. The exercise can be adapted by changing the position or using support, but the appropriate variation depends on your abilities and clinical advice.

Elastic bands

Bands provide progressively greater tension as they are stretched. They are portable and can be used for upper- and lower-body exercises. Anchor placement, band tension and range of motion affect the load, so a qualified professional can help you choose a safe setup.

Free weights and machines

Dumbbells, barbells and machines let you select an external load and target particular muscle groups. Machines may provide more body support, while free weights require you to control the load through space. Neither format is automatically safer; the correct choice depends on technique, supervision, strength and fracture risk.

Why guidance favors a combination

NIAMS describes a combination of weight-bearing, resistance and balance training as the best approach for building and maintaining healthy bones and helping prevent falls and fractures. The reason is complementary loading: weight-bearing activity exposes the skeleton to upright or impact forces, resistance exercise challenges muscles and the bones connected to them, and balance practice addresses a major contributor to falls.

The recommendation is not a promise that every exercise produces the same bone response or that a combination eliminates fracture risk. It is a framework for covering different needs while tailoring intensity and movement selection. The Royal Osteoporosis Society also emphasizes exercise choices that are appropriate to the person’s condition rather than treating one category as a universal solution.

What the 2024 NOGG guideline says

The 2024 guideline from the National Osteoporosis Guideline Group reports that combined weight-bearing and resistance-strengthening programs are effective at reducing bone loss in the femoral neck and lumbar spine in postmenopausal women. The guideline search result does not provide a single effect size in the material available here, so it should not be turned into a percentage claim.

How often should you exercise?

Different organizations express frequency in different ways. Treat these as population-level guidance, not an individualized prescription.

Organization Weight-bearing or general activity Muscle strengthening
Bone Health & Osteoporosis Foundation 30 minutes of weight-bearing exercise on most days Two to three days each week
NIAMS general adult activity guidance At least 150 minutes of moderate-intensity activity or 75 minutes of vigorous activity each week At least twice each week

BHOF’s 30-minute suggestion and NIAMS’s 150-minute or 75-minute targets use different ways of describing activity, so they should not be added together as if they were one required formula. Your clinician may recommend less, more, or a different balance based on your diagnosis, medications, fitness and fracture risk.

A practical way to combine the categories

The following process helps you turn the definitions into a routine without assuming that a particular exercise is safe for everyone.

  1. Start with a health and fracture review. Note osteoporosis or low bone density, vertebral or other fractures, pain, balance problems, joint limitations and relevant medical conditions. Ask a healthcare professional or osteoporosis-aware physiotherapist or exercise professional which movements and impact levels are appropriate.
  2. Select a weight-bearing activity you can perform consistently. Walking, stairs, dancing, jogging and racket sports all fit the category, but they differ in impact and coordination demands. Choose the option that matches your current ability and the advice you received.
  3. Add resistance work for major movement patterns. Body weight, bands, free weights and machines are all legitimate tools. Use a supported or lighter variation when needed, and change the resistance only when your professional adviser considers the progression appropriate.
  4. Include balance practice when it is relevant. NIAMS includes balance training in the combination used to help prevent falls. A person with instability may need supervision and a stable support rather than practicing challenging positions alone.
  5. Distribute sessions across the week. Use the BHOF and NIAMS frequency ranges as planning references, not a reason to force a session through pain, poor balance or unsafe fatigue.
  6. Reassess as your condition changes. A new fracture, worsening pain, a new diagnosis or a change in treatment can alter which exercises and impact levels are appropriate. Review the plan instead of assuming that a previously tolerated movement remains suitable.

Illustrative weekly structure

An example—not a prescription—would place weight-bearing activity on several days, schedule resistance work on two or three nonconsecutive or otherwise manageable days, and add balance work where advised. The exact activity, duration, intensity and rest pattern should come from your health and exercise assessment. The point of the structure is to include all relevant components without treating one session or one exercise as sufficient on its own.

Safety when you have osteoporosis or a fracture history

Get individualized advice before changing impact or load

People with osteoporosis, low bone density, spinal fractures, multiple fractures or other physical limitations should ask a healthcare professional or qualified exercise professional about suitable movements and progression. The same activity can be reasonable for one person and inappropriate for another because fracture location, balance, muscle strength and other conditions differ.

Be cautious with forward bending

BHOF warns that forward bending from the waist can raise spinal-fracture risk in people with osteoporosis. Ask for instruction on safer alternatives and technique rather than assuming that a familiar stretch, abdominal exercise or lifting position is harmless. Your adviser can explain how to keep the spine and hips positioned for the movement you need to perform.

Separate exercise from osteoporosis treatment decisions

Exercise is important, but it does not replace osteoporosis medicine when medicine is indicated. Discuss medication, fracture risk and exercise together with the clinician managing your bone health. Do not stop or delay prescribed treatment because an exercise program has started.

Choosing between weight-bearing and resistance for a specific goal

If your priority is upright bone loading

Begin by discussing a weight-bearing option such as walking, stairs, dancing, jogging or a racket sport. The choice should reflect your fracture history, balance and tolerance for impact. Swimming or cycling can support cardiovascular fitness alongside it, but they do not supply the same impact loading.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

If your priority is muscular strength

Use resistance from body weight, bands, free weights or machines. Select the format that allows you to maintain safe alignment and consistent technique. A supported machine is not automatically superior to a band, and a heavier weight is not automatically better; the useful load is the one you can perform safely and progress under appropriate supervision.

If your priority is reducing fall risk

Do not rely on either category alone. NIAMS places balance training alongside weight-bearing and resistance work because falls can lead to fractures even when bone density is unchanged. Ask for balance exercises suited to your stability and environment.

Common misunderstandings

  • “Weight-bearing means lifting weights.” No. Walking and dancing are weight-bearing, while resistance training may use weights but can also use body weight or bands.
  • “Resistance exercise must be done in a gym.” No. Bands and body-weight movements can provide resistance at home; machines and free weights are optional tools.
  • “Swimming and cycling cover bone-loading exercise.” They support general fitness, but water and a bicycle support body weight, so they are not impact exercises.
  • “More impact is always better.” Impact suitability depends on the individual. Jogging or racket sports may be inappropriate for someone with particular fractures, balance limitations or other conditions.
  • “One category makes the other unnecessary.” Official guidance favors a combination of weight-bearing, resistance and appropriate balance work rather than a universal winner.
  • “Exercise can replace prescribed osteoporosis medicine.” Exercise and medication address different parts of fracture prevention. Medication remains important when a clinician says it is indicated.

Equipment, supervision and access

You can begin understanding the categories without buying equipment. Walking and stairs require no dedicated resistance device, and body-weight movements are a resistance option. Bands, free weights and machines are ways to vary the load, not prerequisites for bone-health exercise.

Professional instruction is particularly valuable after a vertebral fracture, multiple fractures, a diagnosis of osteoporosis, or a long period of inactivity. An osteoporosis-aware physiotherapist or exercise professional can screen movements, adapt positions, and help you progress without turning general guidance into an unsafe personal prescription.
