1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsA useful website usability audit follows real user journeys from intent to outcome, measures where people slow down or fail, and turns each observation into a fix with an owner. This checklist combines analytics, observed task sessions, expert inspection, and accessibility testing so you can find friction that a page-by-page review or automated scanner alone will miss.
What a website usability audit should prove
An audit is an evidence-gathering exercise, not a claim that every page or every accessibility barrier has been checked. The unit of analysis is a user trying to accomplish something: find a policy, compare options, submit an application, make a payment, or get support. A landing page can look polished while a later form, confirmation screen, or error state quietly causes abandonment.

Set the audit up to answer four questions:
- Can the intended user reach the correct outcome?
- How long does the journey take, and where do detours or errors occur?
- Does the user know what to do next and whether the task succeeded?
- Can people using keyboards, assistive technology, or different content settings complete the same journey?
Use analytics to identify common tasks, but do not treat traffic alone as proof that a task is important. A low-volume journey can have high consequences, while a popular page may be only a brief stop in a longer service.

Write the boundary
Record the audience, site or service being assessed, devices and browsers in scope, languages, environments, and the audit date. State whether you are reviewing a public website, an authenticated service, a transaction, or a combination. This prevents findings from silently expanding until the team can no longer tell what was actually tested.

Select representative journeys
Choose high-use and high-consequence tasks, then map every stage from the user’s trigger to the intended outcome. Include journeys that cross several pages, forms, content types, or service stages. A useful sample normally contains the home page, major templates, important content types, forms, search or navigation paths, and transaction steps. The Government Digital Service describes a similar sample-based approach—popular pages, a range of templates, and an end-to-end service where possible—and cautions that it is not full site coverage (Accessibility monitoring: how we test, updated September 30, 2024).
Document what the sample cannot establish
List excluded areas and explain why they were excluded. A sample can reveal patterns and high-impact problems, but it cannot demonstrate that every URL, component state, device, language, or assistive technology combination works. Keep that limitation in the audit report so stakeholders do not read “no issue found in the sample” as “the site is error-free.”
Core website usability audit checklist
- ☐ Audience, device, language, service boundary, and audit date are written down.
- ☐ High-use and high-consequence tasks are selected, including complete journeys.
- ☐ The page sample covers key templates, content types, forms, and transaction steps.
- ☐ Every task has a believable scenario, a neutral prompt, and an observable success condition.
- ☐ Sessions capture completion, elapsed time, abandonment, mistaken completion, errors, detours, ease, and confidence.
- ☐ Navigation, labels, information scent, status feedback, consistency, error recovery, memory load, and unnecessary steps are inspected.
- ☐ Automated accessibility checks are followed by keyboard, manual, and assistive-technology checks where possible.
- ☐ Findings record evidence, location, affected users, task impact, severity rationale, owner, and next action.
- ☐ WCAG conformance failures are separated from significant usability friction that does not fail WCAG.
- ☐ Participant, sampling, and device limitations are disclosed.
- ☐ Prioritized fixes are retested with the same tasks and compared over time.
Write neutral tasks with a measurable finish
Use a realistic situation
Give participants a believable reason to visit and a goal, without naming the navigation label or button they should choose. “You have moved and need to update the address on your permit” is more revealing than “Click the permits menu, choose address changes, and submit the form.” The first tests whether the information architecture communicates the right path.

Specify the outcome that counts: locating the correct policy, selecting the right service, submitting valid information, or receiving a confirmation that can be acted on. Define what constitutes failure and what counts as a mistaken success—for example, the participant believes an application was submitted although a required step was skipped.
Keep conditions comparable and respectful
Use the same wording and success criteria when comparing future rounds. Do not add irrelevant login, account-creation, or data-entry obstacles merely to make a task harder. Obtain informed consent, explain what will be recorded, and protect personal information. Participants should be able to withdraw without penalty.
Observe performance instead of relying on opinions
Capture behavioral measures
For each task, record whether the intended outcome was reached, elapsed time, abandonment, detours, and errors. Note moments when a person pauses, rereads, backtracks, opens an unexpected menu, or asks for help. Record mistaken completion separately from outright failure; it can be more dangerous when users leave with incorrect confidence.

Capture perceived ease and confidence
After each task, ask how easy it felt and how confident the participant is that the result is correct. Preserve useful comments in the participant’s own words, but do not turn a small qualitative sample into a population-wide percentage or claim statistical representativeness without an appropriate study design.

The GOV.UK Service Manual summarizes the relationship between metrics and observation this way: “You can usually see how well a transaction is working for users by using performance metrics combined with user research methods like usability testing.” Read the full guidance in Usability benchmarking a website or whole service.
Use analytics as a lead, not a verdict
Look for high exit rates, repeated validation errors, long waits between steps, search refinements, and unusual backtracking to nominate journeys for observation. Analytics can show where behavior changes, but it cannot by itself explain whether a label is unclear, a control is inaccessible, or the user completed the wrong task.
Inspect the interface for predictable friction
Navigation and information scent
Check whether headings, links, menus, and calls to action describe the destination in the words users expect. Look for near-duplicate labels, category names that require insider knowledge, dead ends, and pages that hide the next step. Test the journey from several entry points rather than assuming everyone starts on the home page.
Feedback and status
Every action should provide an understandable response: loading, saved, submitted, rejected, or still in progress. Examine asynchronous updates, multi-step forms, upload progress, confirmation pages, and session timeout messages. A visual change that is not announced to a keyboard or screen-reader user can create both a usability and accessibility problem.
Memory load and consistency
Identify places where people must remember a code, instruction, or earlier choice instead of being shown the information when needed. Compare control names, placement, date formats, required-field indicators, and confirmation language across templates. Inconsistent behavior forces users to relearn the interface at each step.

Error prevention and recovery
Check whether the design prevents predictable mistakes, explains what went wrong in plain language, preserves valid entries, and points to a specific correction. Test invalid, incomplete, duplicate, and interrupted submissions. Count unnecessary steps and handoffs, but do not remove a step that provides important verification or consent without understanding its purpose.
A heuristic review can organize this inspection. The Nielsen Norman Group Heuristic Evaluation Workbook structures inspection around Jakob Nielsen’s ten usability heuristics. Treat expert findings as hypotheses to validate with users, not proof that users encountered the issue.
Layer accessibility checks into every journey
Start with automated checks
Run an automated scan on representative pages and states to find detectable issues such as missing names, contrast failures, or invalid markup. Examples of commonly cited resources include Axe, WAVE, ARC Toolkit, SiteImprove, browser developer tools, and Microsoft Accessibility Insights. Tool availability and licensing change, and no paid product is required for an audit. Automation is a first pass, not a conformance certificate.

Complete the same journey without a mouse. Verify that every interactive control can receive focus, that focus order follows the task, that the visible focus indicator is clear, and that dialogs, menus, date pickers, uploads, and error summaries can be opened and closed. Ensure the keyboard user can reach the primary action and recover from an error without being trapped.
Check content and form semantics manually
Review link text, headings, page titles, meaningful image alternatives, text and control contrast, form labels, instructions, required states, and error associations. Confirm that status messages and dynamic updates are exposed to assistive technology. The GOV.UK guidance on testing for accessibility explicitly recommends combining automated and manual testing because automated checks miss issues.
Include assistive technology and disabled users where possible
Where resources allow, repeat representative tasks with screen readers, magnification, voice input, or other technologies used by your audience, and invite people who rely on them. Their experience can expose barriers that neither a scanner nor a sighted keyboard test reveals. Record the technology, browser, operating system, and settings so another tester can reproduce the result.

Use WCAG evaluation guidance carefully
W3C’s Evaluating Web Accessibility Overview explains an evaluation approach, while its Easy Checks provide preliminary checks for an initial review. WCAG-EM can help structure a conformance evaluation. Verify the WCAG version and your jurisdiction’s legal duties before publishing a compliance statement; an internal usability audit is not a legal determination.
Choose methods by the evidence they can provide
| Method | Evidence source | Typical scope | Can reveal | Accessibility support | Time and expertise | Tracking and retest |
|---|---|---|---|---|---|---|
| Analytics review | Existing behavioral data | Whole site or service funnel | Drop-offs, exits, repeated actions, slow steps | Usually indirect; device segmentation can identify leads | Low to moderate setup; interpretation required | Strong for trend comparison when instrumentation is stable |
| Expert heuristic inspection | Trained evaluator judgment | Selected pages, templates, or journeys | Labels, consistency, feedback, memory load, error recovery | Depends on evaluator; does not replace assistive-technology testing | Moderate; usability expertise improves coverage | Good when findings include locations and severity rationale |
| Automated accessibility scan | Rules-based tool output | Scanned URLs and states | Detectable markup, naming, and some contrast or structure failures | Accessibility-specific but incomplete | Fast; technical review is needed to interpret results | Good for regression checks on the same pages |
| Observed user session | Participant behavior and comments | End-to-end tasks | Task breakdowns, detours, mistaken success, ease, confidence | Strongest when participants include people using assistive technology | Higher coordination and analysis effort | Strong if prompts, success rules, and conditions stay consistent |
| Manual keyboard and assistive-technology testing | Hands-on task execution | Representative interactive states | Focus, semantics, announcements, traps, and practical barriers | Direct accessibility evidence | Moderate to high; requires suitable skills and technology | Good when environment details are recorded |
Combine methods rather than ranking one as universally best. A scanner cannot show that users misunderstood a confirmation, and a successful sighted session cannot establish keyboard access. The appropriate mix depends on the journey, risk, audience, and available expertise.
Log every finding so another person can act
Use one record per issue. Include all of the following:
- Identifier: a stable number or key.
- Location: URL, template, component, and exact journey step or state.
- Expected outcome: what a reasonable user should be able to do.
- Observed evidence: behavior, transcript excerpt, screenshot reference, metric, or test condition.
- Affected users: audience segment, device, browser, language, or assistive technology.
- Impact: effect on completion, time, abandonment, error rate, or confidence.
- Accessibility reference: applicable WCAG criterion or a note that the issue is usability friction rather than a conformance failure.
- Severity and rationale: explain why the issue is a blocker, high, medium, or low priority in this service.
- Owner and next action: the team responsible and the smallest useful step toward resolution.
- Validation status: whether the finding is confirmed, needs more observation, or is ready for retest.
Keep significant usability errors separate from WCAG failures. A confusing eligibility question may cause abandonment without violating a success criterion; it still deserves attention. Conversely, a technical conformance failure may not have appeared in the observed sessions but should remain visible to the accessibility owner.
Prioritize friction with an explicit rationale
Start with issues that prevent the intended outcome, create harmful mistaken success, block an accessibility technology, or affect a high-consequence service. Next consider problems that add substantial time, repeated errors, or loss of confidence across an important journey. Lower-priority cosmetic inconsistencies can wait when they do not alter comprehension or task performance.
Do not hide uncertainty behind a numerical score. Explain the evidence, the users affected, the consequence, and what additional validation would change the decision. When two findings compete for the same engineering capacity, prefer the one with broader reach or greater harm, not merely the one that is easiest to fix.
Best Value
Retest fixes and compare rounds
Repeat the same conditions
After a fix, rerun the original task prompts and success criteria, using comparable devices, browsers, and assistive technologies. Compare completion, elapsed time, abandonment, mistaken completion, perceived ease, and confidence. Document any change in participant mix or environment before interpreting a difference.
Restart testing when the interface changes
A small visual change can alter focus order, labels, validation, or a later step. The DWP Accessibility Manual’s basic accessibility checks advises restarting checks when the interface changes. Reopen the full affected journey, not just the edited component.
Make improvement periodic
Repeat benchmarking at intervals and after major releases. GOV.UK recommends periodic repetition to see whether a service is becoming easier to use. Preserve the task wording, measurement definitions, and finding history so trends remain interpretable.
Publish the audit with honest limits
State the dates, sample, participant characteristics, devices, browsers, languages, assistive technologies, automated tools, and manual checks used. Identify untested templates, authenticated areas, third-party components, and content states. Explain that findings indicate observed risk within the defined scope; they are not a guarantee of complete usability or accessibility coverage.
Close with a prioritized action list, named owners, target dates, and a retest plan. Include links to the evidence behind each issue and preserve privacy by removing unnecessary personal data from recordings and notes. This turns a one-time inspection into a repeatable improvement cycle rather than a certificate that expires with the next release.
