Implanted and noninvasive vagus nerve stimulation (VNS) are different device categories, not interchangeable versions of one treatment. An implanted system requires surgery, clinician programming, and follow-up; a noninvasive cervical device is a handheld unit used against the side of the neck. FDA records associate specific noninvasive device categories with migraine or cluster headache, while implanted systems have their own device-specific indications. The right comparison starts with the exact model and its FDA labeling—not the general phrase “vagus nerve stimulator.”

What “FDA-cleared” or “FDA-authorized” means for these devices
FDA’s classification records describe a non-implanted, battery-powered handheld device category for migraine and a separate noninvasive category for headache, including cluster headache. Both are Class II devices with a 510(k) submission type. The described unit has intensity control and stainless-steel skin contacts; conductive electrode gel is provided. These classifications are indication- and category-specific: they do not mean every consumer product called a “vagus nerve stimulator” has the same mechanism, authorization, evidence, or approved use. For implanted VNS, the cited FDA labeling specifies stimulation of the left vagus nerve in the neck. It says safety and effectiveness have not been established for stimulating the right vagus nerve or other nerves, muscles, or tissues. Implanting and prescribing clinicians should be appropriately trained. A different system, Vivistim, uses paired VNS as part of upper-limb rehabilitation after ischemic stroke; that indication should not be generalized to other VNS devices or conditions. Risks and follow-up to consider with an implant
Implanted VNS involves an operation as well as use of the device. FDA labeling warns that an infection may be difficult to treat and could require the system to be removed. Stimulation-related symptoms and problems with the lead or generator are also issues to discuss with the treating team. MRI and other-device considerations depend on the exact system and its labeling, so ask the clinician to check the model-specific instructions rather than assuming all implants share the same conditions.

The Vivistim FDA safety study reported implant-site pain in 22.6% of participants and general hoarseness in 7.5%. Its labeling also reported one surgery-related dysphonia/vocal-cord paresis event, which resolved in about five weeks, and described about one in 100 surgeries of this type as associated with hoarseness or dysphonia. These are study-specific results for the Vivistim population and procedure, not universal rates for every VNS system. A recall is specific to model and serial number
FDA’s Class II recall notice, posted January 31, 2025, covers SenTiva REF 1000 and SenTiva Duo REF 1000-D generators with serial numbers below 500,000. The notice says a mechanically activated component may stick and stop stimulation, potentially allowing symptoms to return toward baseline and eventually requiring generator-replacement surgery. FDA relays recommendations for clinician monitoring and on-demand system diagnostics. A brand name alone does not establish whether a particular generator is included; patients should ask their clinician to check the exact model and serial number against the notice. Questions to ask before choosing or using a device
- What is the exact manufacturer, model, and FDA-labeled indication for this device?
- Does the indication match the condition being treated, and what benefits and uncertainties should I discuss with the treating clinician?
- For an implant, what are the surgical risks, programming and follow-up plan, potential adverse effects, and MRI or other-device restrictions?
- For an external device, what are the directions, contraindications, compatible supplies, and instructions for use at home?
- If I have an implanted SenTiva generator, has my clinician checked its model and serial number against the FDA recall notice?
Why wellness claims need separate evidence
The FDA records cited here support specific device categories and indications, not broad claims that VNS devices treat anxiety, improve sleep, reduce inflammation, or provide general wellness benefits. A device’s marketing language is not a substitute for its precise FDA labeling or evidence for a particular condition. The same caution applies to accessory claims: FDA says conductive electrode gel is provided with the described compatible handheld device, but that does not establish that any generic gel is compatible or safe. Follow the exact device instructions.
