In The Witcher 3, toxicity builds as you use potions, meditation restocks known alchemy items when suitable strong alcohol is in your inventory, and a recipe can be missing from the crafting list either because you have not learned it or because you lack its ingredients. Check those conditions separately before searching for a vendor or loot location.
Why is my toxicity so high?
Toxicity is accumulated exposure from potions; the overdose threshold is the point at which penalties apply. The Witcher Wiki’s potions reference says vitality loss begins when toxicity reaches a critical level. The threshold can depend on your alchemy abilities, so a character’s usable limit is not necessarily the same as the baseline.

The Official Witcher Wiki’s Heightened Tolerance listing gives a 75% baseline and a progression of 80%, 85%, 90%, 95%, and 100% at ranks one through five. What to check
- Review current toxicity and avoid taking more potions when you are near the limit.
- Check whether Heightened Tolerance is learned and its rank before assuming the baseline applies.
- Remember that other alchemy skills can affect toxicity or potion effects; the listed toxicity percentage is not the same thing as the amount of toxicity a potion adds.
Do potions refill when I meditate?
Yes. The potions reference says doses are replenished by meditation and that hard alcohol in the inventory is automatically used for the refill. Prima Games’ alchemy guide likewise describes decoctions being replenished during meditation with strong alcohols that restock alchemical items.

Keep suitable strong alcohol available and meditate to refill alchemy items you already know. This restocking rule does not mean you must gather each potion’s crafting ingredients again every time a dose is used.
Why can’t I see or make an alchemy recipe?
First distinguish a formula you have not learned from one you know but cannot currently prepare. The formula reference says known formulas appear in the alchemy list. The preparation reference says formulas without the necessary ingredients are not available for selection.

- Find the exact name. Check the Journal/Alchemy list for the recipe you expect; this helps establish whether it is already known.
- Check whether you learned it. Some formulas are initially available, while others must be acquired during play.
- If known, check preparation requirements. Confirm you have the required ingredients and suitable base for that formula. Missing requirements can keep it from being selectable.
- Pin it if your interface supports the feature. The formula reference notes that pinning can show ingredient needs in the shop panel, a feature it associates with patch 1.07.
- If unknown, pursue acquisition. Formulae may be learned by reading, getting information from NPCs, finding or buying them, and in some cases through experimentation.
- Check for expansion-specific content. The game includes Hearts of Stone and Blood and Wine; an item tied to an expansion may not be part of the base-game formula set.
Where can I find a missing formula?
There is no single complete, current-version location list established for every formula. Start with the exact recipe name, then check likely herbalists and alchemists, relevant loot, and NPC or quest information. Vendor availability and formula locations can depend on the particular recipe and game content.
