A free scan shows the junk files, broken settings and background clutter dragging Windows down - then fixes them in one click.Free scan · Windows 10 & 11A vulnerability scan produces a list of candidate problems. It does not say who owns each problem, how urgent it is for your organization, what will be done about it, or whether the fix worked. Those gaps are where most vulnerability work stalls, and the handoff from scanner output to accountable action is what closes them.
What a scanner actually tells you
CISA’s 2023 Mitigation Guide: Healthcare and Public Health (HPH) Sector describes vulnerability scanners as tools that run credentialed or uncredentialed scans of network-accessible systems and look for known vulnerabilities when they are properly configured. The same guide places scanning as one stage in a longer sequence: scanning, assessing and prioritizing, acting, verifying, and improving. The scanner feeds that sequence. It is not the sequence.

A scan result therefore leaves several questions open:
Quick wins for a faster PC:
Clear out junk files and repair common Windows errorsFree Scan →Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →- Business context. A finding on a patient-facing system and a finding on a lab workstation may carry the same technical label but very different consequences.
- Priority. The scanner reports that a weakness exists. It does not rank that weakness against everything else in the queue.
- Completion. A later scan, or another form of detection, is the only evidence that the weakness is gone. Closing a ticket is not that evidence.
Building the handoff, step by step
A handoff should give the receiving team enough context to act without re-investigating the finding. The following sequence covers the minimum:
- Identify the affected asset and its business function. Name the system and the function it supports, not only the hostname or IP address.
- Attach the vulnerability details. Include the identifier, affected component, and the scanner’s technical description so the engineer does not have to re-derive it.
- Add exploitation information. State whether the vulnerability is known to be actively exploited and where that information came from.
- Write the priority rationale. Explain in one or two sentences why the item sits where it does in the queue, drawing on the inputs described below.
- Name the accountable role. Identify the system owner or administrator who will carry the work, and the security role that reviewed the treatment.
- Record the treatment decision. State whether the item is being remediated, mitigated, or accepted, and who approved that choice.
- Define closure evidence. Specify what must be observed before the item can close, such as a rescan result or a repository query showing no remaining instances.
CISA’s Cyber Resilience Review Supplemental Resource Guide: Vulnerability Management, which addresses vulnerability management as an organizational capability rather than a scanning task, calls for agreed time frames, defined roles and responsibilities, consistent processes, and an approved tools list. Those agreements are what make a handoff repeatable. Without them, each finding becomes a negotiation.
For exploited vulnerabilities that require a response, CISA’s Cybersecurity Incident and Vulnerability Response Playbooks include a checklist that treats response as work to be tracked to completion across discovery, evaluation and prioritization, and remediation. That supports an explicit owner-and-status record. It does not prescribe a particular ticketing product, and any tool that holds owner, status and evidence fields will serve the same purpose.
Priority: keep the reasoning, not just the number
CISA advises mapping assets to business-critical functions and prioritizing vulnerabilities that are actively exploited. It identifies several inputs that answer different questions, and they should not be treated as interchangeable scores:
Recommended Free Tools
| Input | What it measures | What it does not tell you |
|---|---|---|
| CVSS | Technical severity of the vulnerability | Whether anyone is exploiting it, or what the affected system does for the organization |
| EPSS | Likelihood that the vulnerability will be exploited | Whether the flaw is already being used in attacks, or the consequence if it is |
| CISA KEV (Known Exploited Vulnerabilities) | Whether the vulnerability is known to be actively exploited | How severe the flaw is in technical terms, or the local business impact |
| SSVC (decision tree) | Exploitation status, technical impact, mission prevalence, and public well-being impact combined into a decision outcome | Whether the organization has the capacity to act on that outcome, which the handoff must record |
A finding can be technically severe and unlikely to be exploited, or moderate in severity and already in active use. The handoff should show which of these dimensions drove the decision. A line such as “CVSS high, KEV listed, patient-facing system, no compensating control” gives the next person something to check. A bare score gives them nothing to challenge.

Choosing the disposition
CISA says the security team, system owners, and system administrators should come together to determine the appropriate treatment. Three dispositions are distinguished, and they are not synonyms for ticket statuses:
| Disposition | What it does | Typical use, per CISA | What the record must show |
|---|---|---|---|
| Remediation | Fully fixes or patches the vulnerability | The preferred end state where a fix is available and can be applied | The fix chosen, the change owner, and the verification method |
| Mitigation | Reduces the likelihood or impact without removing the flaw | Often a temporary measure while a fix is unavailable | The control applied, the residual exposure, and the condition that will trigger a fix |
| Acceptance | Takes no action to fix or reduce the risk | CISA describes it as potentially justified when risk is low, or when fixing costs or risks exceed the likely consequences | The rationale, the approver, and who holds the accepted risk |
Acceptance is the disposition most often recorded without a decision behind it. Treat it as a formal choice with a named approver and a reason written down, not as the default for items that nobody has time to fix.
Recording decisions so they survive turnover
CISA’s resilience guidance recommends a repository of prioritized vulnerabilities and their dispositions that maintains a historical record. The point is continuity. When the engineer who accepted a risk changes teams, the next reviewer should be able to see what was decided, why, by whom, and when it should be revisited. A repository that holds only current status cannot answer those questions.
Verifying the outcome
CISA recommends another scan after remediation is considered complete, to confirm that the vulnerability was effectively remediated or mitigated. The resilience guide adds two steps: monitor corrective actions, and query the repository or rerun detection. Additional instances found afterward may show that the root cause was never addressed.

That last signal is easy to miss. Consider a worked example, which is illustrative rather than drawn from CISA: a vulnerable library is patched on one host and the ticket is closed. A rescan of that host comes back clean, but the same library reappears on three other hosts built from the same image. The fix worked locally, while the root cause, the image baseline, was never changed. The handoff should therefore close only after the rescan covers the affected population, and any recurrence should reopen the item against the cause rather than the host.
Verification also applies to mitigations. A mitigation should carry a condition that prompts a revisit, such as a fix becoming available, and that condition should be checked on the same schedule as the scans.
How far CISA’s guidance reaches
The materials described above are CISA guidance, not a universal legal mandate. The healthcare-sector mitigation guide recommends scanning software, devices, and systems at least monthly. That cadence is a recommendation for that sector, and it should not be read as a deadline that applies to every organization. The resilience review materials and the response playbooks are reference resources for building the process. Neither names a commercial scanner, ticketing system, or service, and their inclusion here does not imply endorsement of any product.
