Check the PATH and host of the process that actually failed, then verify its shell, launch settings, working directory, and command lookup behavior. The right fix depends on where the command runs.
First identify which process failed
“Installed” does not mean every process searches the same directories. A terminal may build its PATH by reading shell startup files; a desktop app may inherit a different environment. An editor’s integrated terminal, task, debugger, extension, remote session, container, or WSL distribution can each run with different settings or on a different host.

Start by recording the command name, the complete error message, the application and operating system, and whether the failing process runs locally, remotely, in a container, or in WSL. Visual Studio Code’s CLI documentation describes its “‘code’ is not recognized” error as: “Your OS cannot find the VS Code binary code on its path.” That wording points to command lookup, but other errors may mean the program started and then failed. Do not diagnose from an exit code alone. Microsoft’s VS Code troubleshooting guide notes, “An exit code alone is not a diagnosis”; the full message, shell, operating system, and launch context matter. VS Code terminal troubleshooting

Compare the environment that works with the one that fails
Check PATH in the working terminal and in the failing process’s own context. The executable’s containing directory must be available to the process doing the lookup. If the app exposes separate settings for a task, debugger, extension, or automation, inspect those too; the app’s general terminal configuration may not govern them.
In VS Code, check the selected terminal profile and its executable path and arguments, the working directory, environment overrides, inheritance settings, and automation profile. If an integrated terminal works but a task does not, inspect the task’s options.cwd, options.shell, and environment. For a remote window, investigate the host where the command actually runs rather than only the local machine. VS Code terminal troubleshooting
Apply the narrowest correction supported by what you find: add or restore the missing executable directory in the failing process’s environment, fix an incorrect shell or working directory, or remove an override that replaces PATH. Avoid changing global PATH blindly when the problem is confined to one app or launch configuration.

Check the selected shell and its startup files
A shell can be configured with an incorrect executable, arguments, or startup behavior. Test the selected shell directly with the same executable and arguments the app uses. If it works without startup customization, inspect the relevant profile files for commands that fail, exit early, or overwrite PATH.
VS Code’s troubleshooting guide gives these examples for starting shells without their usual profile customization:
bash --noprofile --norczsh -fpwsh -NoProfilecmd.exe /d
If the command works in a clean shell but not the configured one, focus on that shell’s startup files and app profile rather than reinstalling the tool. VS Code terminal troubleshooting

Consider platform-specific lookup differences
VS Code’s code command
VS Code’s CLI FAQ says Windows and Linux installations should put code on PATH. On macOS, you may need to run Shell Command: Install ‘code’ command in PATH from the Command Palette. VS Code may also launch an interactive login shell to retrieve a shell environment and align PATH with a user-created terminal. Visual Studio Code CLI documentation
macOS apps launched from Finder
A GUI app may not receive the environment you see in Terminal. PyInstaller documents that a Finder-launched macOS app bundle can have PATH set to /usr/bin:/bin:/usr/sbin:/sbin. In that documented context, common terminal locations such as /usr/local/bin and /opt/homebrew/bin may be absent. The app may need a suitable environment configuration or the executable’s full path. Windows App Paths
Windows supports an App Paths registry mechanism for registering an executable’s fully qualified path and prepending a path for processes launched using ShellExecuteEx. Microsoft presents it as an application-specific alternative to adding entries to the global system PATH. It is not a guarantee that a bare command will resolve in every shell; shell lookup and application launching can follow different rules. Microsoft: Registering Applications

Windows executables launched from WSL
When a Linux process in WSL invokes a Windows executable by name, Windows locations need to be available in the WSL environment. Check Linux PATH with echo $PATH. Microsoft notes that shell setup can remove Windows paths if it replaces PATH without preserving the existing value. Microsoft: Working across Windows and Linux file systems
If the failure is in the shell itself, inspect its command cache
Bash and zsh can cache a command’s previously resolved location. This is most relevant when the command fails in that shell, especially after installing it or changing environments—not when only a separate GUI app cannot find it. In bash or zsh, use type command_name to see what the shell will run. Conda documents clearing a stale location with hash -r in bash or rehash in zsh; reactivating the environment can also refresh the cache. Conda troubleshooting

A running application has the environment it received when it started; changing a shell profile or system setting does not by itself show that the already-running app has received the update. Relaunch the relevant app or use its documented environment-refresh behavior, then inspect PATH from the same failing task, terminal, or process context again. Match the fix to where the command fails

The practical rule is to trace the failing process, not just the installation: establish its host and PATH, then check the shell and launch configuration. That separates a missing search path from a wrong shell, startup-file problem, or stale command location.
