The next generation of weight-loss medicines is no longer all in the pipeline: the U.S. Food and Drug Administration approved Foundayo (orforglipron) on April 1, 2026. Other much-discussed candidates—including retatrutide, survodutide and cagrilintide–semaglutide—have late-stage trial results, but those results do not make them approved treatments. The key differences are convenience, drug targets, trial populations and evidence—not a single proven “best” medicine.
Which next-generation weight-loss drugs are approved?
Foundayo (orforglipron): approved in the United States
FDA approved Foundayo on April 1, 2026, for adults with obesity, or adults with overweight and at least one weight-related comorbidity. It is used alongside a reduced-calorie diet and increased physical activity to reduce excess body weight and maintain weight reduction long term. FDA describes orforglipron as a GLP-1 receptor partial agonist pill.
FDA’s announcement quoted Acting Center for Drug Evaluation and Research Director Tracy Beth Høeg, M.D., Ph.D.: “Individuals who are overweight or obese now have an additional option for weight loss — a GLP-1 receptor partial agonist pill which does not need to be taken on an empty stomach.” The approval is specific to Foundayo and its indicated use; it does not approve every GLP-1 medicine, formulation or product marketed for weight loss.
Retatrutide: still investigational
Eli Lilly’s information says retatrutide is not FDA-approved. On July 23, 2026, the company said it planned to submit a biologics license application to FDA in the first quarter of 2027, following additional positive phase 3 results. That is the company’s stated plan, not an FDA decision or guaranteed submission date.

Survodutide and CagriSema: published trials, not established approvals
The cited phase 3 publication describes survodutide as an investigational glucagon receptor–GLP-1 receptor dual agonist. Phase 3a studies of cagrilintide–semaglutide, often called CagriSema, have also reported results. Those publications establish trial findings, not current approval status. The evidence available here does not establish CagriSema’s current filing status.
What is changing in the next wave?
- Convenience: Foundayo is a pill, and FDA says it does not need to be taken on an empty stomach. This is a meaningful difference in how the medicine is taken, not proof that it works better than injectable options.
- More than one target: Survodutide acts on glucagon and GLP-1 receptors. CagriSema combines cagrilintide and semaglutide. These approaches broaden the range of mechanisms being studied, but a mechanism alone does not establish clinical superiority.
- Different evidence questions: Late-stage studies examine different populations and outcomes. For example, the cited retatrutide and survodutide studies enrolled adults with obesity without diabetes, while the CagriSema program includes studies both with and without type 2 diabetes.
- Evidence beyond the scale: The results summarized below report body-weight change. They should not be read as establishing comparative effects on other health outcomes.
What have the late-stage trials found?
The figures below are results from separate placebo-controlled studies, not a head-to-head comparison. Each percentage is a study-level estimate or mean, not a promise of an individual’s result.

A separate 68-week phase 3a CagriSema study, REDEFINE 2, enrolled participants with overweight or obesity and type 2 diabetes and found significantly lower body weight with the combination than with placebo. The cited source information does not provide its complete result table, so a precise percentage is not stated here.
Why can’t these percentages identify a winner?
There is no direct comparison among the three trial programs in the findings above. Their populations, baseline characteristics, treatment periods, doses, analysis methods, placebo responses and discontinuations can differ. Even the meaning of a reported percentage depends on the study’s analysis: SYNCHRONIZE-1, for example, reports its week-76 result under a treatment-regimen estimand. Comparing the headline percentages as though every participant followed the same protocol would turn separate trial results into a misleading ranking.

A fair comparison needs the full study design and safety data, as well as the population for whom a medicine is intended. The results are useful evidence about what happened in each trial; they do not predict an individual outcome or show which medicine will be the best choice for a particular patient.

Approval is jurisdiction-specific: Foundayo’s approval described here is FDA approval in the United States. Retatrutide remains investigational according to Lilly’s information, and trial results for survodutide or CagriSema do not establish that either is available as an approved treatment. Do not treat a planned regulatory submission as permission to prescribe or obtain a medicine.

FDA has warned about unapproved GLP-1 products marketed for weight loss, including complaints about compromised cooling during shipment of compounded products. Products advertised as “research use only” are not evidence-based substitutes for trial medication or an approved prescription. Questions about treatment suitability and currently authorized options belong with a qualified clinician.
