How The Medium’s dual realities work
Bloober Team’s third-person psychological horror game follows Marianne to an abandoned resort, where she can interact with the material world and its spirit-world reflection. The official game page describes the central idea as exploring both worlds together and solving puzzles across them: The Medium official game page. Some sequences let you move through both realities at once; other moments use transitions between them.

The spirit world is not just a second backdrop. PLAION’s support description identifies three practical uses for Marianne’s abilities: investigate places her physical self cannot reach with Out of Body, create a Spirit Shield, and unleash a Spirit Blast. See PLAION support: The Medium. Pay attention to what an obstacle asks you to do in each reality rather than treating every puzzle as a search for a single switch.
Tips for progressing without missing opportunities
- Inspect optional spaces before moving on. Collectibles may be off the main route, hidden in interaction screens, or require backtracking, according to TrueAchievements’ general hints. Look around thoroughly before triggering a clear transition to the next area.
- Use your abilities deliberately. Try Insight when an object or clue seems easy to overlook, and use Out of Body to investigate reachable spirit-world areas. These tools support exploration as well as puzzles.
- Choose hold or toggle controls to suit you. TrueAchievements recommends switching hold-based controls such as sprint and breath to toggle if that feels more comfortable. This is optional, not a game requirement.
- Consider Events Autocompletion if you want fewer repeated actions. The guide notes it can skip a short orientation action for one collectible type. It is a convenience setting, not a way to recover collectibles already missed.
These recommendations come from the guide’s general hints, not from a complete first-party route map. For exact item locations, use a collectible walkthrough that matches your platform and version rather than relying on a route from another edition.
#1 Best Overall
- The Medium is a third-person psychological horror game that features patented dual-reality gameplay and an original soundtrack co-composed by Arkadiusz Reikowski and Akira Yamaoka
- Use your psychic abilities to solve puzzles spanning both worlds, uncover deeply disturbing secrets, and survive encounters with the Maw - a monster Born from an unspeakable tragedy
- Delve into a mature and morally ambiguous story, where nothing is what it seems - with every new perspective you will change your perception on what happened at the Niwa resort
- The Medium's spirit world is a dark mirror reflection of our reality, a grim and unsettling place where our unpunished deeds, evil urges, and vile secrets manifest themselves and can take on a form
- Explore the physical world and the spirit world simultaneously, and use the interactions between them to solve dual-reality puzzles, unlock new paths, and awaken memories of past events
Which collectibles are missable?
TrueAchievements warns that collectible achievements are missable, that each collectible category must be completed within one playthrough, and that the game has no chapter select. It lists 11 postcards; some collectibles may be tucked into interaction screens or involve returning to an earlier part of an area. The safest approach is to keep a checklist by category and inspect thoroughly before proceeding.
The guide identifies collectible achievements as a whole as missable, rather than establishing a complete first-party list of every missable item and its route. Do not assume a later chapter replay will be available to fill a gap. TrueAchievements’ general hints and tips provide the cited completion warnings; use a platform-appropriate walkthrough for a step-by-step location list.

- BECOME A MEDIUM: Wield unique psychic abilities reserved for those with the gift. Travel between the realities or explore them both at the very same time. Use the Out of Body experience to investigate places where your real-world self can't go. Create energy shields and deliver powerful spirit blasts to survive the spirit world and its otherworldly dangers.
- SEE WHAT'S HIDDEN: Delve deep into a mature and morally ambiguous story, where nothing is what it seems and everything has another side. As a medium you see, hear and experience more than others, and with every new perspective you will change your perception on what happened at the Niwa resort.
- PLAY IN TWO WORLDS AT THE SAME TIME: Never-seen-before and officially patented gameplay that plays out across two worlds displayed at the same time. Explore the physical world and the spirit world simultaneously, and use the interactions between them to solve dual-reality puzzles, unlock new paths, and awaken memories of past events.
- ENTER A DARK REALITY INSPIRED BY BEKSIŃSKI: The Medium's spirit world is a dark mirror reflection of our reality, a grim and unsettling place where our unpunished deeds, evil urges, and vile secrets manifest themselves and can take on a form. This world has been invented and designed under the inspiration of Zdzisław Beksiński's paintings, Polish dystopian surrealist internationally recognized for his distinctive and strikingly ominous style.
- MUSIC BY TAMAOKA & REIKOWSKI: Immerse yourself in the disturbing and oppressive atmosphere of the game thanks to the original 'dual' soundtrack co-created by Akira Yamaoka and Arkadiusz Reikowski. Yamaoka-san is a legendary Japanese composer best known for his work on the Silent Hill series.Reikowski is a Hollywood Music in Media Awards nominee who worked on such acclaimed horror games as Blair Witch, Layers of Fear, and Observer.
How to plan Echoes and cumulative achievements
XboxAchievements reports 39 achievements worth 1,000 points for the Xbox achievement set. Its undated guide, accessed in 2026, says there are 32 Echoes in the game and 31 are sufficient for the “Calling Out to Me” achievement. These are third-party guide figures, not official statistics, so confirm the requirements shown for your platform before relying on them. The guide is at XboxAchievements: The Medium achievement guide.
TrueAchievements also calls out cumulative goals involving Insight, time spent outside the body, running and object interactions. Its guide gives thresholds of 15 minutes using Insight and 10 minutes outside the body, plus interaction milestones of 25, 50 and 100. Treat these as that guide’s achievement-planning details, not universal requirements across every platform or version. If you want these goals, use the relevant abilities and interact with objects consistently rather than leaving all of the accumulation until the end.

Which edition should you choose?
Choose according to the platform you play on and whether you want bonus material; the extras are not needed to complete the story or collect achievements. The official site presents PS5 and Xbox purchase options, while Steam describes a Deluxe Edition that bundles the game with the soundtrack and art book. Store contents and availability can change, so check the current listing for your platform before buying: The Medium on Steam.
The official site credits Akira Yamaoka and Arkadiusz Reikowski as co-composers. Soundtrack or art products are optional extras, not gameplay requirements.
