Survey Form V06 is a free, 166 KB HTML/CSS/JavaScript template for a short weekly team pulse survey. It has five five-point agreement questions and an anonymous-by-default option. It can run on its own or be added to a Bootstrap 4, 5, or 6 project, but it does not send or save answers until you connect a backend or form service.
What Survey Form V06 includes
Colorlib describes V06 as a weekly employee-feedback form. Its five statements use a scale from “Strongly disagree” to “Strongly agree,” shown as dots along a rose-to-green track. A counter displays how many statements have been answered.

Colorlib describes the download as plain HTML and CSS with a small amount of vanilla JavaScript. It does not require jQuery, Bootstrap files, or plugins, and is described as usable by itself or within a Bootstrap 4, 5, or 6 project. Bootstrap is therefore optional: compatibility does not mean the template needs Bootstrap to run.

The source page describes native radio groups inside fieldsets and an anonymity switch using role="switch" and aria-checked. It also says scale labels remain beneath their points on phones. Those are publisher descriptions; they do not establish an independent accessibility audit or device test.
Is Survey Form V06 anonymous?
Anonymity is enabled by default in the form interface. Turning the switch off reveals required name and team fields. That setting describes the form’s front end; it is not a guarantee about privacy or data handling once the form is connected to a service. For a real deployment, explain to respondents what information is collected and how it will be used.

Where the answers go
Nowhere, unless you connect the form to a backend or form service. The download is a front end: Colorlib says its demo shows a thank-you state rather than transmitting responses. It does not provide a complete survey platform or store answers by itself. A live survey needs a receiving service or server-side implementation, and responses should be validated on the server before you rely on them. Colorlib’s survey-template roundup also describes this limitation.
License and reuse
The V06 page lists a CC BY 3.0 license. Colorlib’s roundup says its snippets are free for personal and commercial projects if the credit link is retained. Check the current license and attribution information on the template page before reuse, since those details govern how you may distribute or adapt the download.

When this template fits—and when to adapt it
V06 is shaped for a brief recurring team check-in: five agreement-scale statements can suit a pulse survey that should be quick to answer. If you expand the questionnaire, put the most important question first, consider making less important questions optional, and show progress when a survey takes more than one screen. For repeated rating questions, a matrix can be compact on desktop, but should be adapted into phone-friendly cards on smaller screens. These are general design recommendations from Colorlib, not measured findings about V06.
