It presents a 0–10 recommendation scale and changes its follow-up question according to the respondent’s score category. The template can display a survey, but its demo does not transmit answers: to collect responses, connect the form to a server endpoint, CMS handler, or form service and validate submissions on the server.
What Survey Form V02 includes
Colorlib describes V02 as a dark, in-app survey intended for SaaS dashboards, accounting products, and relationship surveys. Its main interaction is a single 0–10 recommendation question, followed by a category-specific prompt for detractors, passives, or promoters. The template page lists HTML, CSS, and vanilla JavaScript as its building blocks. Colorlib’s V02 page identifies Aigars Silkalns as the author and lists a September 2026 release and a 249 KB download.

Colorlib also describes an optional email field with inline format validation, a success state that shows the selected score and receives focus, a polite announcement when the follow-up changes, and a two-row scale on mobile. Those are publisher descriptions, not independently verified test results.
How the score categories work
The familiar NPS grouping assigns scores of 0–6 to detractors, 7–8 to passives, and 9–10 to promoters. V02 uses those three respondent categories to determine which follow-up question appears. The category follow-up can make the prompt more relevant to the answer, but it does not turn the snippet into a complete survey platform or a data store.

Does V02 require Bootstrap?
No. Despite “Bootstrap” in the page title, Colorlib describes the snippet as plain HTML and CSS with a few lines of vanilla JavaScript; it does not bundle Bootstrap files, jQuery, or plugins. The publisher says it can be used on its own or placed in projects using Bootstrap 4, 5, or 6. Treat that as compatibility guidance, not a requirement to install Bootstrap. See the template’s stated setup details.
How to collect real survey responses
The supplied demo is front-end only. Colorlib’s roundup says its templates reach a thank-you state without sending data. A visible success message therefore confirms only that the browser-side interaction completed; it does not mean the response was saved. Colorlib’s survey-template roundup describes the front-end limitation and the need to connect a collection mechanism.

- Choose where submissions will go. Connect the form to a server endpoint, a CMS form handler, or a form service that can accept and store submissions.
- Map the fields you need. Ensure the submission includes the selected score and the appropriate follow-up answer. If the optional email field is enabled, decide whether it is necessary for the survey and how it will be handled.
- Validate on the server. Browser-side checks, including inline email-format validation, can improve the interface but do not replace server-side validation.
- Make success reflect the actual result. Show a saved or thank-you state only after the receiving system accepts the response; handle failed submissions with a clear recovery message.
- Test the complete path. Verify that each score category produces the intended follow-up and that a submitted answer reaches the destination. Check keyboard and screen-reader behavior as well as the mobile layout.
Colorlib’s V02 information does not establish a particular backend, integration recipe, or data-retention policy for the template. Those depend on the collection system you choose.

Is V02 the right survey format?
V02 is a reasonable fit when the goal is a focused recommendation score with one adaptive follow-up, such as an in-app prompt or a periodic relationship survey. Choose a different interaction when the survey needs more steps or a different response structure. Colorlib’s 2026 roundup describes these alternatives:
- V01: combines star satisfaction, NPS, and comments for a richer post-purchase feedback prompt.
- V03: uses multiple pages, progress, and a review page for a longer questionnaire.
- V04: gathers beta feedback and can accept a screenshot.
- V05: uses a session-rating matrix.
- V06: is designed as an employee pulse survey.
- V07: is an instant poll.
- V08: presents one question at a time in a conversational format.
- V10: is an exit-intent checkout feedback card.
Compare the audience and timing, the number and structure of questions, any need for file upload or a matrix, and the experience on phones and with keyboard or screen-reader interaction. Colorlib describes these formats but does not provide comparative performance measurements. Its advice that a single NPS prompt can get more answers than a long questionnaire is publisher guidance, not a quantified study result.
