Repair Windows errors before they cause bigger problemsFix Now →Scan for outdated or missing drivers - takes under a minuteDriver Scan →If a bug report comes back as “cannot reproduce,” the next report should let someone follow the same path and see the same difference without filling in missing details. Use the one-page template below to state the problem, starting conditions, exact steps, expected and actual results, and relevant environment. Add screenshots or isolation checks only when they help explain the failure.
A one-page bug report template
Copy this structure into your issue tracker or report form. Replace each prompt with what you know; mark relevant unknowns as unknown instead of guessing.

Title: [Action] causes [observed failure]
What happened?
[Describe one problem, including the page, feature, or task. Copy any error text exactly.]
Starting conditions
[Account role or state, required data, settings, feature flags, and whether this is a fresh install/profile. Include only conditions relevant to reaching the problem.]
Steps to reproduce
1. [Start from the conditions above.]
2. [Perform one action; include the specific input or choice that matters.]
3. [Continue in order.]
4. [End at the point where the behavior differs.]
Expected result
[What should have happened?]
Actual result
[What happened instead? Include the exact message or visible state, if available.]
Environment
[Product and version/build; operating system and version; browser and version or device, when relevant. Add other runtime details only when they plausibly affect this issue or the project requests them.]
How often? / last observed
[Every time, intermittently, or once; when last observed.]
Evidence and isolation checks (as relevant)
[Attach a clarifying screenshot or minimal test case. For a browser issue, note useful checks such as another browser, a private window, disabling extensions, or clearing cache.]
How to fill it in so someone else can follow it
Give the report one focused title and one issue
Name the action and the observed failure, rather than writing only “broken” or “doesn’t work.” Keep separate problems in separate reports: Mozilla Bugzilla’s guidance puts it plainly, “Open a new bug report for each issue!” A narrow report gives the reader one behavior to reproduce and one expected-versus-actual comparison to evaluate.

Write steps as a sequence, not a summary
Start at a defined state and list actions in the order they happen. Include the page or feature, the choice or input that matters, and the last step before the problem appears. “Submit the form and it fails” leaves unanswered which form, what was entered, and what failure looked like. Chromium’s guidance is to “Mention detailed steps to replicate the issue.”

- report writing template for law enforcement
Keep each step to one action where possible. If a particular role, setting, record, or setup is needed, put it under starting conditions so the steps do not depend on hidden assumptions. If you do not know a relevant detail, say so; an honest unknown is more useful than an invented setup.
Separate expected behavior from actual behavior
Write what should have happened in one field and what you observed in another. Be concrete: for example, “After selecting Save, the edited name should appear on the profile” versus “The Save button becomes disabled and the previous name remains.” If an error appears, copy its wording exactly rather than paraphrasing it. These fields help distinguish a reproducible defect from a mismatch in what the reporter expected.
Record the environment that could affect the failure
Include the product and version or build, plus the operating system and version. For a browser-based problem, include the browser and version; for a device-specific issue, identify the device. Other details—such as account role, runtime, settings, or enabled features—belong in the report when they could plausibly change the behavior. There is no single environment checklist that fits every product.
Describe frequency without implying certainty
Say whether the problem happens every time, intermittently, or only once, and note when you last saw it if that helps someone check current behavior. These are useful practical fields, not a universal requirement stated by the project guidance. Do not call a bug “always reproducible” if you have only seen it once.

When to add a screenshot or a minimal test case
Use evidence to make a particular state or failure easier to understand, not as a substitute for written steps. A screenshot can show the page, visible message, or unexpected state; avoid relying on it to communicate actions that occurred before the image. A minimal test case is a smaller example that still reproduces the issue. MDN describes creating one as the first step in identifying a problem’s source.
For browser issues, a few targeted checks may help isolate the cause: try another browser, test in a private window, disable extensions, or clear the cache. Note what you tried and what changed. These are diagnostic options for relevant browser problems, not mandatory steps for every bug report.
Rank #4
What “cannot reproduce” tells you—and what to do next
It means the person checking the report could not see the described behavior using the information or setup available to them; it does not establish that your experience did not happen. Chromium’s triage guidance describes follow-up questions about details such as the operating system, a helpful screenshot, or checking against a current version. Treat the response as a prompt to compare setups and clarify the reproduction path.

Check whether the account state, data, settings, version, or platform you used differs from what the other person tried.
- Make the path explicit. Add the missing action, input, or prerequisite instead of repeating “it still fails.”
- Clarify the observed result. Supply the exact error text or describe the visible state at the point of failure.
- Report useful checks. If you tested another version or, for a browser issue, another browser or a private window, state the result rather than just saying you investigated.
A public Microsoft GitHub issue template offers one project-specific example of fields for description, reproduction steps, expected behavior, screenshots, device details, and additional context. It is an example, not a universal reporting standard; follow the project’s own form when it asks for different information.
